- چکیده انگلیسی: The preoperative care of patients undergoing orthopedic surgery and treated with biologic agents is of great significance.
Perioperative use of biologic agents could lead to such complications as infection and delayed postoperative wound
healing. This narrative review aimed to evaluate the current information on the use of biologic agents in patients
undergoing orthopedic surgery, determine the rate of associated postoperative complications, and identify the
appropriate time for the continuation or discontinuation of biologic therapy in these patients. It can be stated that
all biologic agents increase the risk of infections depending on their half-lives. Biologic agents are suggested to be
withheld for at least twice their half-lives before major surgeries. However, in case of minor operations, they can be
continued given the low risk of infection and impaired wound healing in these cases.

- چکیده انگلیسی: Autoimmune diseases are characterized by the attack of the immune system to normal tissues. Patients with autoimmune diseases usually have the deficiency of dietary factors that may be related to the etiology of these conditions. Given the role of vitamin E as a physiologic stabilizer of lysosomal membranes, its deficiency can initiate the process of autoimmune diseases or accelerate its progress. It is supposed that vitamin E could reduce oxidative stress, which is an important factor in the pathogenesis of autoimmune diseases. The literature review is indicative of a decrease in the serum levels of vitamin E in almost all autoimmune diseases. Furthermore, there is evidence regarding the possible therapeutic value of vitamin E in the management of autoimmune diseases. Owing to the anti-inflammatory and protective effect of vitamin E against free radicals, and also its important effect on cytokines levels, this vitamin may play a powerful role in the prevention and treatment of rheumatoid arthritis, as well as joint inflammation and damage. Moreover, increased vitamin E intake might decrease the incidence and severity of certain autoimmune diseases through the regulation of the immune system.

- چکیده انگلیسی: Sydenham’s chorea is a delayed complication of group Aßhaemolytic streptococcal infections and forms one of the majorcriteria of acute rheumatic fever.It is characterised bychorea, muscular weakness, and a number of neuropsychiatricsymptoms. It is considered to be an autoantibody mediated disorderwith the evidence suggesting that patients with Sydenham’schorea produce antibodies that cross react with streptococcal,caudate, and subthalamic nuclei.However, documented evidenceof previous streptococcal infection is found in only 20%–30%of cases. It has a goodprognosis for full recovery so treatment is not warranted inmost cases. Following case is a high school student presented with progressive changes in his handwriting during two months before. Chief complaint of this 15-year-old boy was difficulty in writing at classroom. His physics notebook has been shown as interesting figures.

- چکیده انگلیسی: Background: Rheumatoid arthritis (RA) is described as a systemic and chronic autoimmune disease characterized by inflammatory polyarthritis. Lymphocyte-activation gene 3 (LAG3) is a membrane glycoprotein expressed on activated, exhausted, and regulatory T cells. LAG3 plays a major role in the function of Treg cells. LAG3 also has a soluble form (sLAG3) with a controversial role. Objective: To evaluate the serum level of sLAG3 in rheumatoid arthritis patients in comparison with healthy subjects and assess its association with the disease activity. Methods: This cross-sectional study was performed on 105 patients with RA referred to Ghaem hospital of Mashhad, Iran. We divided the participants into four groups: 1) 35 untreated patients with newly diagnosed RA, 2) 35 active RA patients, 3) 35 patients in the remission phase of the disease, and 4) 35 healthy individuals matched in terms of age and sex. After completing the interview and questionnaire, the sLAG3 was evaluated by commercial ELISA. Results: The serum level of sLAG3 significantly increased in RA patients (76.78 ng/ml) as compared with the healthy participants (51.67, p=0.002). However, there was no significant difference between RA patients in the remission phase of the disease (114.11 ng/ml) and those with moderate to high disease activity (63.06 ng/ml, p=0.076). Conclusion: This study provided insights into the role of sLAG3 in the immunopathogenesis of RA disease, but further investigations are also warranted.

- چکیده انگلیسی: Objective(s): Juvenile idiopathic arthritis (JIA) is one of the most common chronic rheumatic diseases in children. The complex nature of this immune-mediated disease owes itself to several predisposing genes and environmental factors affecting its pathogenesis. Conducted in Iran, this study was originally intended to investigate every possible association between HLA DRB1 alleles and a susceptibility to JIA.
Materials and Methods: In this case-control study, 45 patients with a definite diagnosis of JIA based on International League against Rheumatism (ILAR) criteria were compared against 46 healthy controls. DNA samples taken from both groups were analyzed using PCR-sequence specific primers (PCR-SSP) method. Data analysis including parametric and nonparametric test and multivariate analysis was undertaken using the SPSS 11.5 software. A P-value< 0.05 was regarded as statistically significant.
Results: Mean ages in case group and healthy controls were 14.64±6.21 and 13.73±6.39, respectively with no significant difference between the two groups (P=0.515). Sex difference between JIA group and healthy controls was also not significant (P=0.068) .The frequency of HLA-DRB1*01 was found the most frequent HLA-RB1 in our patients (33.3%). No significant statistical correlation between various HLA-DRB1 alleles and clinical subtypes of the disease could be established from the data. HLA-DRB1*11 was shown to raise protection to JIA (P=0.035, OR=2.755, 95% CI=0.963-8.055) in northeastern Iran. In addition, we found that HLA-RB1*09 is nominally associated with an increased risk of JIA (P=0.56, OR=2, 05, 95% CI=0.18-23.63).
Conclusion: HLA-DRB1*11 was shown to raise protection to JIA in northeastern Iran. The disparity of findings in other ethnicities prompts further investigations with larger sample sizes.- انتشار مقاله: 27-04-1396

- چکیده انگلیسی: Objective(s) Recent studies on human indicate that the introduction of therapeutic use of tolerogenic dendritic cell (DC) for chronic inflammatory conditions is imminent. For the purpose of defining CGRP potency in tolerogenic DC production, we investigated the phenotype and IL-'2 production of DCs generated from the monocytes of rheumatoid arthritis (RA) patients in the presence of the calcitonin gene-related peptide (CGRP), as a multifunctional neuropeptide. Materials and Methods DCs were generated from isolated monocytes from four resistant and two early female RA patients using IL- 4, GM-CSF, and CGRP at concentrations of 0, ', and '00 nM. Then, the phenotype of neuropeptide-treated or untreated DCs was determined using flow cytometry and the IL-'2 production was measured by ELISA. Results Our study showed that, on the last day of the culture, at a concentration of ' nM CGRP, the mean fluorescence intensity (MFI) for CD80 increased ('4.'3%) and the MFIs for CD83, CD86, and HLA-DR decreased ('4.57%, 5.28%, and 6.88% respectively). Moreover, at '00 nM CGRP concentration, the MFI for CD80 increased (''.'0%) and the MFIs for CD83, CD86, and HLA-DR decreased (4.27%, '8.60%, and '9.75% respectively). In addition, our results indicated that the mean concentrations of IL-'2 produced at 0, ', and '00 nm CGRP concentrations measured '3.72±2.4', ''.0'±'.6', and 7±'.34 pg/ml respectively. Conclusion Decreased CD83, CD86, and HLA-DR expression and reduced IL-'2 production by CGRP were found in the RA patients' monocyte-derived DCs. CD83 is a well-defined DC activation marker. HLA-DR and CD86 are appropriate molecules for inducing an immune response. IL-'2 promotes cell-mediated immunity. Therefore we suggest that CGRP may be used as an inducer in the production of tolerogenic DCs.

H syndrome is an autosomal recessive genodermatosis with a low prevalence which is caused by a mutation in SLC29A3 gene. This disorder is characterized by sclerotic, hyperpigmented, hypertrichotic cutaneous plaques with systemic involvement including: hepatosplenomegaly, heart anomalies, hearing loss, hypogonadism, low height, and hyperglycemia.
Case Presentation
Here we have presented two cases of H syndrome that have been misdiagnosed and mismanaged as rheumatologic disease. The first case had been represented with sclerotic skin lesions and diagnosed as morphea, and second one with chronic and recalcitrant to treatment arthritis as juvenile idiopathic arthritis.
Conclusion
H syndrome is an autosomal recessive genodermatosis that has been recently recognized with a variety of manifestations and overlapping features with other diseases. Increase the knowledge of physicians for wide spectrum manifestations of this syndrome along with reporting the misdiagnosis of this condition can increase the accuracy of physicians for its better identification. - چکیده انگلیسی: Objective(s): Systemic lupus erythematosus (SLE) is an autoimmune disease with a wide range of clinical manifestations. Cognitive dysfunction is one of the manifestations that could present prior to the emergence of any other neuropsychiatric involvements in SLE. Cognitive dysfunction is a subtle condition occurring with a
high frequency. However, there is no data on the correlation of cognitive dysfunction with central nervous system (CNS) imaging findings, in particular single-photon emission computed tomography scan (SPECT) and functional MRI. We decided to perform a systematic review of brain SPECT and fMRI in SLE patients with cognitive dysfunction.
Methods: PubMed, Scopus, and Google Scholar databases were searched until April 2017 with the following keywords: “SLE OR systemic lupus erythematous OR lupus” AND “functional MRI OR functional magnetic resonance imaging OR fMRI OR SPECT or SCAN”. A total of 1,767articles were found. Two rheumatologists
reviewed the articles and finally 14 articles were selected for the final systematic review.
Results: The fMRI and SPECT imaging techniques could provide valuable information regarding the SLE patients with cognitive dysfunction at the early stages of the disease.
Conclusion: Brain SPECT scan and fMRI are used as functional imaging tools in SLE. Both of these diagnostic modalities are sensitive in reflecting the subtle brain damages in SLE patients with cognitive dysfunction. Brain fMRI and SPECT scan could be significantly beneficial in the diagnosis and initial management of cognitive dysfunction in SLE. Nevertheless, prospective studies could be useful in confirming the application of these diagnostic modalities in the clinical setting.- انتشار مقاله: 01-07-1396
