- چکیده انگلیسی: Background: The prevalence of allergic diseases is increasing worldwide. The effective role of omega-3 fatty acids in the maturation of immune system and protection against atopic diseases has been well discussed. However, previous studies revealed conflicting results. This study was conducted to investigate the effect of fish oil consumption by nursing atopic mothers on the level of interferon gamma (IFN-γ) in their breast milk and incidence of allergic disease in their infants.
Methods: This randomized clinical trial was conducted on 94 atopic mothers, who were assigned to two equal groups receiving either 1000 mg fish oil capsules or placebo for 60 days after delivery. The breast milk was collected 120 days after delivery, and IFN-γ level was measured. The history of the symptoms of atopic disease in infants was collected from their mothers through ISAAC questionnaire; in addition, the infants’ growth was evaluated.
Results: Sixty mother-infant pairs completed the trial. The mean values of IFN-γ in breast milk was higher in the fish oil group (1.11±1.15 Pg/ml) than in the placebo group (0.81±0.86 Pg/ml), but the difference was not significant (P=0.288). In addition, the incidence of allergic symptoms of infants was not significantly different between the two groups (P=0.84).
Conclusion: In this trial, the consumption of fish oil by lactating mothers did not have a significant effect on IFN-γ level in their breast milk and the incidence of allergic symptoms in their infants. Future studies with longer follow-ups are necessary in this regard.- انتشار مقاله: 09-02-1397

- چکیده انگلیسی: Background: Type 1 diabetes mellitus (T1DM) is by far the most common metabolic disease in children. Asthma is the most common chronic disease in pediatric population, and its prevalence has increased in the last decades. In this study, the prevalence of asthma among these children with T1DM has been described and its association with the demographic and clinical characteristics of Iranian children has been evaluated.
Materials and Methods: A cross sectional study was carried out on children with T1DM who referred to pediatric endocrinology clinics of Isfahan University of Medical Sciences. The participants were classified as a group with asthma and the second group as diabetic children without asthma. After selection and evaluation of the diabetic patients with asthma, their glycemic status was evaluated three times in the past year. All data were analyzed using the SPSS version 23.0 statistical software package.
Results: A total of 419 patients (49.4% male and 50.6% female) diagnosed with T1DM were included in the study. The mean age of patients at the time of recruitment was 12.65 ± 3.9 years with a range from 3.8 to 18 years. The mean of disease duration was 5.3 ± 2.7 years. Among all participants, asthma was detected in 24 children with T1DM (5.7%). Glycemic control was significantly poorer among asthmatic patients with diabetes compared with diabetic patients without asthma.
Conclusion: The study demonstrated a lower prevalence of asthma among T1DM children rather healthy ones. Hence, diabetic child patients with asthma experience poorer glycemic control in comparison with T1DM patients without asthma.- انتشار مقاله: 07-02-1396

Asthma is the most chronic inflammatory disorder of the airways in children and asthmatic patients can experience cardiac dysfunction, pulmonary hypertension and finally cor pulmonale later in life. We aimed to investigate Right Ventricular (RV) functions in asthmatic children by conventional and tissue Doppler echocardiography (TDE).
Materials and Methods
Pulmonary function tests, conventional and TDE examinations were performed on 42 asthmatic and 42 age- and gender matched healthy controls subjects (n=42).
Results
Compared with healthy children the RV wall was statistically thicker among asthmatic patients (P= 0.01). Conventional echocardiography had not significant difference between cases and controls, but TDE had significant difference between these two groups. Peak E’ velocity, A’ velocity, E’/A’ ratio and S’ in lateral and medial sites of tricuspid annulus valve, were significantly differ from control group in our patients (P<0.01). Isovolemic relaxation time and isovolemic contraction time were higher in asthmatic patients than the controls (P<0.01). Tie index or Myocardial performance index (MPI) had higher value in asthmatic patients (P<0.01). There was no difference in deceleration time between patients and cases (P=0.12).
Conclusion
Our study showed that although the conventional echocardiography and clinical finding were apparently normal in asthmatic children, TDE could showed subclinical dysfunction of the right ventricle in these patients. Our findings signify the diagnostic value of tissue Doppler- echocardiography superior to conventional echocardiography for the early evaluation, detection and monitoring of ventricular dysfunction among asthmatic patients.- انتشار مقاله: 19-08-1395

Allergic rhinitis (AR) is a common form of nose inflammatory disease that occurs when the mucosal immune system overreacts to environmental allergens especially in the air. It’s absolutely clear that the prevalence of AR varies between different countries, and even between the various areas of a country. The purpose of present investigation is to determine the prevalence of AR, as most common allergic disorders, in the children of middle schools in Isfahan, Iran.
Materials and Methods
This investigation performed from February 2016 to July 2016, thoroughly, 1191 Junior high school children aged 13-14 years old were screened in our study. Authenticated ISAAC questionnaire was interviewer-administered to gather data on parentally informed symptoms, clinician diagnoses and environmental exposures.
Results: The students were averagely 13.57+ 0.5 years old; 779 (65.4%) of them had been suffering from AR and 696 (58.4%) of them had the symptoms of rhinitis over the preceding year. Symptoms of conjunctivitis such as Itch and watery eyes have been reported in 35.1% of cases. Among examined students, 644/1191 (54.1%) were females (P= 0.000). The relationship of allergic rhinitis and exposure to parental smoking, domestic and type of feeding in infancy were not significant (P= 0.655, 0.459, 0.946, respectively), but exposure to house planting was statistically significant (P=0.000). Daily activity was affected in 43(3.6%).
Conclusion
Prevalence of allergic rhinitis was considerably high in Junior high school students of Isfahan and frequently is undiagnosed. In addition the association of allergic with gender and keeping house plants was significant.- انتشار مقاله: 30-01-1397

- چکیده انگلیسی: NEMO (NF-κB essential modulator) is a regulatory factor involved in signaling pathways of innate and adaptive immune system. Hypomorphic mutation of IKBKG gene on X chromosome leads to X-linked recessive anhidrotic ectodermal dysplasia with immunodeficiency. The affected boys presented developmental phenotype with hypotrichosis, hypohidrosis; hypodontia with conical incisors; and susceptibility to pyogenic bacteria, mycobacteria, and viruses. Most of them have impaired antibody response to polysaccharide antigens. Here, we presented the case report of 7 years old Iranian boy with NEMO-deficiency and unusual Aspergillus infection.

- چکیده انگلیسی: Ataxia-telangiectasia is a rare primary immunodeficiency and multisystem DNA repair disorder which is caused by mutation in ataxia telangiectasia mutated (ATM) gene. The ATM protein plays a critical role in sensing DNA double-strand breaks (DSB), oxidative stress and other genetic stresses. The ATM can directly mention DNA ends in repair complexes and directly involved in the repair of DSBs induced during T cell and B cell rearrangement. Therefore, increase serum IgM level and recurrent infection mainly sinopulmonary, indistinguishable from hyper IgM syndrome can be a presentation of some AT patients. AT patients with class-switched defect are more prone to severe infections, autoimmunity and lymphoproliferative disorders. Herein we present an AT patient with characteristic feature of hyper IgM phenotype and lymphoproliferation.

- چکیده انگلیسی: Primary immunodeficiency (PID) is a group of more than 400 distinct genetic disorders affecting both children and adults. As signs and symptoms of PID are usually heterogeneous and unspecific, based on the available human resources and laboratory facilities, diagnosis and follow-up of these patients can be challenging. In line with this notion, management and treatment of PID are practiced divergently in different PID centers. In order to reach a distinct national protocol and given the little evidence on appropriate or universal guidelines to improve the current status of management of the disease, the Iranian PID network designed a consensus suitable for regional resources. This review summarizes this PID guideline based on the importance of different clinical complications and the level of medical authority visiting those at the first line. Further, for each complication, appropriate interventions are mentioned to improve the approach.
