- چکیده انگلیسی: Background & objective: Breast cancer is the leading cause of cancer related death in females. Sphingosine kinase 1 (SPHK1) and its product sphingosine-1-phosphate (S1P) are the essential key regulator molecules in breast cancer through their ability to promote cell proliferation, angiogenesis, cell proliferation, and lymphagiogenesis. SPHK1 is overexpressed in multiple types of cancer including breast cancer and is associated with resistance to treatment. The current study aimed at investigating the expression of SPHK1 in estrogen and progesterone receptors (ER, PR) negative in comparison to ER, and PR positive breast cancer and their normal controls, and also finding the relationship between SPHK1 expression and high body index (BMI) in the selected groups with breast cancer.
Methods: A total of 120 human breast cancer tissue specimens were analyzed for SPHK1 expression using quantitative real–time polymerase chain reaction (q RT-PCR) assay. Detection of hormonal status of breast cancer tissue samples was conducted by immunohistochemical assay.
Result: The current study findings showed that the level of SPHK1expression in the breast cancer tissue was significantly higher in patients with estrogen and progesterone negative receptors, compared to the ones without them (P-value< 0.05). The obtained data confirmed that the obesity in patients with ER negative was higher than the ones with positive receptors (BMI> 25).
Conclusion: The current study showed that expression of SPHK1gene was higher in the patients with ER and PR negative breast cancer and high BMI, compared with other groups.- انتشار مقاله: 25-10-1394

- چکیده انگلیسی: Background & Objective: There is a complicated interaction between leishmaniasis and the host immune cells, and also between the host immune cells. These interactions have fundamental effects on the outcome of the disease.
The current study aimed at characterizing the number, distribution, co-localization, and interrelation of 4 types of inflammatory cells in different clinical forms of dry-type cutaneous leishmaniasis (CL).
Methods: Thirty-nine cases of CL were studied. The cases were classified clinically as 14 cases of acute leishmaniasis with indurated papules, nodules, and plaques with central crust formation < 2 years, 7 cases of chronic type with non-healing lesions > 2 years, and 12 cases of lupoid leishmaniasis with characteristic papules around previous scars of CL > 2 years. Paraffin-embedded blocks were stained with hematoxylin and eosin (H&E) and also stained immunohistochemically for CD4, CD8, CD68, and CD1a.
Results: In acute CL, there was a significant correlation between CD68+ macrophages and CD1a+ epidermal dendritic cells (DCs); the population of CD68+ macrophages and CD1a+ epidermal DCs increased in parallel.
In lupoid CL, there was a significant correlation between CD1a+ epidermal DCs, and CD1a+ dermal DCs and population of CD1a+ epidermal DCs; the number of CD1a+ dermal DCs increased in parallel.
Conclusions: The result of the current study could be used as a baseline to design and study the new targeted therapy of synergistic effects of macrophages and DCs to phagocytizing leishmania bodies; and/or suggestion planning of individualizing setup of vaccine by autologous interaction of macrophages and DC in CL.- انتشار مقاله: 25-12-1394

Their usual locations are soft tissue , cutaneous or subcutaneous tissue and bone especially vertebra(2).However, intradural extramedullary hemangiomas are rare and most of them fall in cavernous type category. Hereby, authors report a woman with spinal intradural extramedullary capillary hemangioma which is exceedingly rare.

- چکیده انگلیسی: Leishmaniasis include several clinical manifestations, mostly cutaneous, visceral and mucosal (1, 2). Various species can lead to diverse clinical presentations. Thus species identification contribute to proper management (3). Localized Leishmania Lymphadenitis (LLL) is a distinct entity in clinicopathologic field presenting with isolated lymphadenitis and possible cocomitant cutaneous lesion in the absence of systemic visceral involvement (4). Species identification has been acceptable by conventional microscopic evaluation, serologic methods such as isoenzyme and monoclonal antibody detection and particularly PCR. However, definite identification and confirmation of parasite without gene sequencing have always been doubted (5).

- چکیده انگلیسی: Background: The breast cancer is the most prevalent cancer among women, on the other hand absence of myoepithelial cells play a pivotal role in pathogenesis of this cancer. Thus we aimed to investigate the possible abilities of the molecular assay technique to find a relationship between mammary serine protease inhibitor (Maspin) gene expression possibly secreted by myoepithelial cells, grade of breast cancer and other prognostics factors (ER, PR, and c-erb-B2).
Methods: Paraffin embedded blocks of 31 breast cancer patients together with two normal breast tissues were used for IHC staining and Maspin gene RNA detection uses the real-time PCR method. Applying QIAGEN kit, we were able to measure Maspin RNA and Extract the cDNA of different samples for evaluating the Maspin RNA level.
Results: We found that the RNA level was considerably lowerin these cancer samples compared with normal samples. In addition, different grades of breast cancer in the obtained results adopt some distinguishable values. The Maspin expression in samples with grades II and III is much lower than the ones in normal group (P<0.05) which could be considered as a promising way in diagnosing of this disease. The results showed no considerable differences in Maspin gene expression of the c-erb-B2 scores in the tumor group except the samples having score 0. The other observation of this research study confirmed that Maspin gene expression couldn't show any differences between the values of both ER and PR in different scores of the tumor group. On the other hand, the cDNA of these patients showed lower values compared with normal samples.
Conclusion: Maspin expression was reduced in samples with grade II& III of invasive ductal carcinoma. Based on expression of Maspin Inc-erb-B2, it seems that more expression happened in normal group comparing with different scores of it. We could suggest that there was a reverse relationship between tumor formation and Maspin gene expression. These results showed possible role of Maspin as prognostic factor.- انتشار مقاله: 15-11-1393

- چکیده انگلیسی: Background & Objectives: Chronicity of lupus nephritis (LN) should be considered for interaction of cell mediated immunity (CMI) and dendritic cells in glomeruli and tubulointerstitial areas. In this study establishment of immunohistopathological changes of dendritic cells and other immune effector cells in lupus nephritis comparing with non-lupus nephritis was performed. Materials & Methods: Renal needle biopsies of 35 cases of lupus nephritis and 35 cases of other causes of persistent proteinuria were compared for immunohistochemistry for plasmacytoid (CD123), myeloid (CD11c) dendritic cells, macrophages (CD68) and lymphocytes (CD4) markers. Statistical analysis of the data was performed using Spearman and Pearson correlation or ANOVA and t- student test (P < 0.05). Results:Significant difference of glomerular and interstitial spaces for presence of myeloid-plasmacytoid dendritic cells and lymphocytes except macrophages between lupus nephritis and other causes of persistent proteinuria have found (P<0.001). Positive significant correlations were observed between glomerular presentation of myeloid dendritic cells and chronicity index but not with other markers in lupus nephritis (P <0.001). Statistically significant changes between presence of all markers and activity index were not observed (P >0.05). Conclusions: The myeloid dendritic cells might have synergistic role with other immune cells in pathogenesis and progression or chronicity of lupus nephritis.

- چکیده انگلیسی: We present unusual retained remnants of hidden conjoined twin of possible abdominal pagus after 21 years who presented with abdomenpain and incidentally have found his forbidden conjoined twin. He presented for many years with a palpable epigastric mass, and recent abdominal pain, early satiety, nausea and vomiting. The mass was a like deformed fetus weighing 815 gr who was anencephalic and deprived of viscera but there was a distinct vascular connection through the rig limb. Microscopic examination revealed a normal epidermis with melanocytes overlying dermis with mature adnexa. The fetus plain film after surgery showed a rather complete skeletal system. All above findings were in favor of fetus in fetus, a rare congenital entity in which a nonviable parasitic fetus grows within the body of its twin.

- چکیده انگلیسی: Background and Objectives: Kerman Province, especially city of Bam in the southeast part of Iran, is epidemics for dry type cutaneous leishmaniasis (DTCL). This study was conducted to compare the effect of different treatments on parasite DNA load following therapies using Real-Time PCR method. Materials and Methods: Fifteen patients were divided into three groups under therapy with intralesional meglumine antimoniate, topical imiquimod and combination of both drugs. After obtaining consent from patients, punch biopsies were taken before and after treatment .To compare the amount of DNA load a relative quantitative Real-Time PCR method was designed and set up using Leishmania tropica ITS (internal transcribed spacer) gene. After doing PCR, the obtained results were analyzed using 2^-ΔΔCT method and relativity of DNA load before and after treatment were calculated. Results: The highest falling of DNA load was for imiquimod (mean 4/7 cases), glucantime (mean 2/2 cases) and combination therapy (mean 2/4). From clinical point of view combination therapy had the best response. On the other hand, the overall IHC findings showed good response based on decreased CD1a epidermal, increased CD1a dermal, decreased CD68 macrophages and increased CD3 and CD20 of dermis. Conclusion: We set up a new method to compare Leishmania DNA load using the stable human gene of beta actin for normalization. We concluded that imiquimod was immune modulator and had synergistic effects on the best parasitocidal drug of glucantime for better response.

- چکیده انگلیسی: Inflammatory myofibroblastic tumor is an uncommon benign neoplasm and its presentation in small bowel is rare. Due to clinical manifestation, laboratory data and radiologic results, there is an increased risk of over diagnosis as malignant tumors particularly malignant lymphoma in childhood therefore may be resulting in wrong therapeutic approach. However pathologic findings could be demonstrated definite diagnosis based on markedly proliferation of bland looking spindle cells which their presence can be confirmed by immunohistochemistry staining to show myofibroblastic differentiation. We describe here an unusual presentation of inflammatory myofibroblastic tumor of the terminal ileum in a 19-month-old girl who presented with anemia, eosinophilia, intestinal obstruction, focal ossification and dystrophic calcification within the tumor.
