- چکیده انگلیسی: Background: Morning reports (MRs) are commonly used as an efficient technique in Medical Education. This study was intended to assess the developmental process following Iranian definable standards in the Internal Medicine Department, Imam Reza General Hospital, Mashhad, Iran.
Methods: Following an initial one-month assessment through direct observation of morning reports held in Imam Reza Hospital Internal Medicine Department, workshops were run for 6 weeks aiming at rectifying the flaws and reforming the trends practiced contrary to current standards. Checklists were filled by the attending researcher, subsequent to which feedback was given regarding possible flaws and/or challenges to the attending physicians. Reforms as well as alterations were urged to improve the status quo, which were eventually accepted and implemented by the Head of Department. Reassessment was conducted six weeks afterwards, using checklists having been prepared in advance. An equal number of MR Sessions (n=25) was evaluated prior to and following the Reform Scheme.
Results: Significant differences can be seen in the level of participation by nephrologists, infectious disease specialists and clinical pharmacologists after the scheme (P<0.001). Better arrangements were made between the coordinator and the resident in charge prior to the MR session, mainly via short text messages (44%). This encompassed the case selection, number of cases to be presented and the chief objective behind these presentations. Of the total of 65 patients presented, 50 (77%) were complicated ones whereas common disorders only reported in 6% of the cases. Presentations became growingly shorter in case of the first cases (P=0.022) while second and third ones took as much time as prior to the Reformation Scheme. There could be seen no considerable improvement in the accuracy of the final diagnoses yet punctuality was reported to have improved significantly as morning reports routinely and regularly commenced at 8 a.m. following alterations(P=0.025). A significant rise in the number of cases presented and discussed in every meeting (p=0.006).
Conclusions: Training and feedback seem to have improved the quality of morning reports in different respects, especially when augmented by applying national as well as international standards used in this and other studies.- انتشار مقاله: 04-05-1393

- چکیده انگلیسی: Background: Running standard journal clubs is one of the important goals of postgraduate education. Students should update their knowledge and learn critical appraisal skill. This study aimed to compare quality of journal clubs in Mashhad Dental School in order to improve educational and appraisal structure of journal clubs.
Methods: This quasi-experimental study was conducted on journal clubs of Mashhad dental school. Journal clubs were evaluated and compared using a valid and reliable (r= 0.74) checklist before and after an educational workshop. Data were analyzed using SPSS: 11.5 by paired T-Test (CI= 95 %)
Results: There was a significant difference before and after intervention in terms of educational structure, however no critical appraisal was conducted.
Conclusion: As to the results, it is recommended to refresh faculty members and postgraduate students in terms of educational and appraisal structure of journal clubs. It seems teaching critical appraisal is more important.- انتشار مقاله: 17-02-1392

- چکیده انگلیسی: Background: Lead is a toxic element which causes acute, subacute or chronic poisoning through environmental and occupational exposure. The aim of this study was to investigate clinical and laboratory abnormalities of chronic lead poisoning among workers of a car battery industry.
Methods: Questionnaires and forms were designed and used to record demographic data, past medical histories and clinical manifestations of lead poisoning. Blood samples were taken to determine biochemical (using Auto Analyzer; Model BT3000) and hematologic (using Cell Counter Sysmex; Model KX21N) parameters. An atomic absorption spectrometer (Perkin-Elmer, Model 3030, USA) was used to determine lead concentration in blood and urine by heated graphite atomization technique.
Results: A total of 112 men mean age 28.78±5.17 years, who worked in a car battery industry were recruited in the present study. The most common signs/symptoms of lead poisoning included increased excitability 41.9%, arthralgia 41.0%, fatigue 40.1%, dental grey discoloration 44.6%, lead line 24.1%, increased deep tendon reflexes (DTR) 22.3%, and decreased DTR (18.7%). Blood lead concentration (BLC) was 398.95 µg/L±177.40, which was significantly correlated with duration of work (P=0.044) but not with the clinical manifestations of lead poisoning. However, BLC was significantly correlated with urine lead concentration (83.67 µg/L±49.78; r2=0.711; P<0.001), mean corpuscular hemoglobin (r=-0.280; P=0.011), mean corpuscular hemoglobin concentration (r=-0.304; P=0.006) and fasting blood sugar or FBS (r=-0.258; P=0.010). Conclusion: Neuropsychiatric and skeletal findings were common manifestations of chronic occupational lead poisoning. BLC was significantly correlated with duration of work, urine lead concentration, two hemoglobin indices and FBS.- انتشار مقاله: 09-09-1392

- چکیده انگلیسی: Objective(s) Herbal waters are extensively used in most parts of Iran including Mashhad. Visiting a patient with total blindness due to daily ingestion of around 200 ml of herbal water (Plant forty water) per day for six months was the rational for methanol determination in all herbal waters available in Mashhad markets. Materials and Methods A total of two hundred and nineteen bottles of herbal waters were randomly bought from Mashhad market. Methanol concentration was determined by gas chromatography, using a Flame Ionized Detector. Benzene (1000 mg/l) was applied as the internal standard. Collected data was analyzed by SPSS software (version 11.5), using appropriate descriptive statistical tests. Results Forty six different herbal waters from three main producing factories (A, B and C) were tested. Highest methanol concentration was measured in dill water of A (1208±202.74 mg/l), concentrated rose water of A (1017.41±59.68 mg/l) and concentrated rose water of B (978.52±92.81 mg/l). Lowest methanol concentration was determined in Trachyspermum copticum water of B (18.93±1.04 mg/l), cinnamon and ginger water of B (29.64±10.88 mg/l) and rice skin water of A (41.33±7.85 mg/l). Mean methanol concentrations of herbal waters including ginger, cinnamon, dill, peppermint, alfalfa, and plant forty from A, B and C were 374.69, 209.81 and 280.12 mg/l, respectively (P< 0.001). Conclusion Methanol concentration in all herbal waters, especially rose water of the three producers was very high that may induce toxicity in people taking these products regularly for a long time.

- چکیده انگلیسی: This paper presents a novel topology of permanent magnet brushless DC motors. Brushless DC motors usually experience torque ripple mainly caused by cogging torque. In the proposed motor, the whole structure of the rotor’spermanent magnets hasbeen changed so that the cogging torque is considerably mitigated. The philosophy behind this modification depends on the way of flux-path production in the rotor structure and it should be similar to the way of motor phases. Aninitial electromagnetic analysis is, first,carried out using the finite element method. Asensitivity analysis is, next,included to obtain the most important design parameters for the proposed structure. The different performance parameters of the motor are calculated and compared betweenthe proposed structures and the conventional BLDC structures. The results revealthat the proposed motor has a considerably lowertorque ripple retaining an average value of the produced torque. The proposed structure is also compared with an asymmetrical V-type structure and the results further show the effectiveness of the proposed structure.

- چکیده انگلیسی: Objective: Organophosphorus compounds (OPs) are common causes of poisonings. Atropine and oximes are pharmacological antidotes of OPs. However, because of their adverse effects and insufficient performance, several other compounds have been evaluated as adjuvant therapy. HESA-A is a herbal-marine drug that contains material from Carum carvi (Persian cumin), Penaeus latisculatus (king prawn), and Apium graveolens (celery) with anti-inflammatory and antioxidants properties, which has shown useful effects as adjuvant therapy on some diseases. We have evaluated the effect of HESA-A on 69 moderate to severe acute OPs poisoned patients (44 HESA-A treated and 25 controls) as an adjuvant drug.
Materials and Methods: Two randomized age and sex matched groups of OPs poisoned patients were treated in Medical Toxicology Center of Imam Reza hospital, Mashhad, by conventional therapy with or without HESA-A (50 mg/kg/day orally). The evaluation criteria were total administrated doses of atropine and pralidoxime, intensive care unit (ICU) admission rate, mechanical respiration need, number of hospitalization days and mortality.
Results: There were no significant differences between the morbidity and mortality rate criteria of the two groups; moreover, we did not observe significant adverse effects for HESA-A.
Conclusion: HESA-A did not reduce morbidity and mortality of OPs poisoning and did not induce any major side effect in the patients.- انتشار مقاله: 26-11-1397
