The term E-professionalism or electronic professionalism is the manner in which one involves himself online in connection with his profession and it incorporates an individual’s attachment to professional codes of conduct. In other words, if one acts and behaves in a professional way on electronic or digital platforms, he has observed e-professionalism. One issue which is of paramount importance is that confidentiality must be properly deemed in e-professionalism in all fields in general and in medical practice in particular. If confidentiality is not regarded in patients’ Electronic Health Record (EHR), patients will not reveal correct information to their physicians and the professional image of health care providers will be damaged.

- چکیده انگلیسی: Dear Editor,In spite of recent advances in treatment of infection and emphasis on adequate nutritional intake, patients with cystic fibrosis (CF) frequently have less than ideal body weight and adults are often shorter than their target height.1Owing to a state of relative growth hormone (GH) insensitivity, the anabolic and growth promoting effects of recombinant human GH (rhGH) treatment have been explored in children with CF.2 A number of open-labeled uncontrolled studies of small numbers of undernourished prepubertal children with CF documented improvements in weight and height with rhGH treatment.3We studied 12 prepubertal children with CF (diagnose is based on clinical presentation and sweat chloride more than 60 meq/L for 2 times) in three divided stages. In the first 3 months (stage 1) they received nutritional guides and supplements in addition to pancreatine. In the second stage all patients received rhGH 0.05 mg/kg/day, 6 days per week for 6 months, and in the last 3 months (stage 3) patients were followed the same as stage 1. All patients were evaluated for growth parameters (weight, height, bone age), monthly.There were 8 boys and 4 girls with mean age of 5.4 years. The mean weight and height velocities during the first stage (before rhGH treatment) were 116 gr/month and 0.64 cm/month and increased to 417 gr/month and 1.51 cm/month respectively, during the second stage (rhGH treatment), the differences were statistically significant (P<0.05).Also the mean weight and height Z scores before rhGH treatment were -1.46 and -1.7, but increased to -1.25 and -1.3 during rhGH therapy, respectively, the differences were statistically significant (P=0.002) also. The mean bone age before and after treatment with rhGH were 51 and 59 month, respectively, the difference was statistically significant (P=0.011).Growth is a dynamic process that starts at conception and ends after full pubertal development.Failure to thrive is one of the common presenting features of CF after neonatal period, and poor weight gain and impaired linear growth are well recognized in these patients4. Therefore assessment of growth in children with CF is essential for monitoring well being, disease activity and response to treatment.4The CF foundation reported that 30% of children are bellow the 10th percentile for height and 26% are bellow the 10th percentile for weight.5 Of the 13116 children with CF studied by Lai and co-workers in the US, around one third were bellow the 10th percentile and less than 25% were above the 50th percentile for height and weight.6In a randomized clinical trial over one year of 19 prepubertal CF patients whose initial height and weight were below 10th percentile despite adequate caloric intake, Hardin and colleagues reported greater height and weight velocity in those who had rhGH compared with those who did not.1A significant number of our patients had growth failure at presentation and rhGH therapy significantly improved their height and weight velocity.We conclude that rhGH therapy is safe and effective for enhancing growth in children with CF who do not achieve ideal growth on standard enteral nutritional supplementation.

- چکیده انگلیسی: Background: Atopic dermatitis is a major public health problem, often starting in early childhood and sometimes followed by other allergic diseases. Although hypersensitivity to foods is assumed to play an essential role in the development of atopic dermatitis in some patients, little is known about common food allergens in Iranian children with atopic dermatitis.
Objectives: This study was designed to identify probable food allergens in Iranian children with atopic dermatitis and find the relationship between food sensitization and the severity of atopic dermatitis.
Methods: This study included 90 children aged 2-48 months with atopic dermatitis. Skin prick tests for cow’s milk, hen’s egg, almond, potato and soybean were done. Serum specific IgE to 20 food allergens was also screened.
Results: Among children with atopic dermatitis, the frequency of food sensitization was 40% by skin prick test and 51% by food-specific IgE. Children with atopic dermatitis were most commonly sensitized to cow’s milk (31%), hen’s egg (17.7%), tree nuts (17.7%), wheat (12.2%), potato (11.1%), tomato (8.8%) and peanut (8.8%). In 42 children with moderate to severe eczema, sensitivity to food allergens was 78.5% by skin prick test and 88% by serum specific IgE evaluation.
Conclusion: Our results showed that cow’s milk, hen’s egg and tree nuts were the most common food allergens in Iranian children with atopic dermatitis. Sensitization to foods was much higher in patients with moderate to severe atopic dermatitis. Objectives: This study was done to determine the prevalence of IgG aCL and its subclasses in relation to APS symptoms, in a group of juvenile rheumatoid arthritis (JRA) and juvenile systemic lupus erythematosus (SLE) patients.
Methods: In this prospective study, 28 JRA and 16 SLE patients, aged 3-18 years, were enrolled. IgG aCL was assayed by standard aCL ELISA. IgG subclasses were also assayed by ELISA on sera with medium to high titers of aCL. ACL assay was performed on at least two occasions for each patient, over 3-6 months period of follow up.
Results: 29% (8/28) of JRA patients and 44% (7/16) of SLE patients had aCL. Six of SLE patients displayed APS related manifestations: hemolytic anemia, thrombocytopenia, arterial occlusion, valvular heart disease, livedo reticularis and pulmonary hypertension, but none of them had persistant medium or high titer of aCL. The lack of association of high titer of aCL with APS related symptoms was observed in two patients. The IgG subclasses were primarily IgG1 and IgG3.

- چکیده انگلیسی: Introduction: The purpose of this study was to describe clinical features and serological findings of children with idiopathic arthritis in south-western Iran.
Methods: This descriptive study included 60 patients with juvenile idiopathic arthritis who were referred to a pediatric rheumatology clinic at a university hospital during 6-month period. Initial manifestations, first laboratory tests and clinical course of patients were reviewed.
Results: Sixty children (32 boys and 28 girls) with idiopathic arthritis ranged in age from 1.5 to 16 years. The mean age at the first presentation was 4.92 years (SD= 3.68). Oligoarthritis was the most common subtype in 27 (45%), followed by systemic- onset in 17 (28.3%) and polyarthritis in 16 (26.7%) of patients. The most commonly involved joints were knee 53(88.3%), ankle 28(46.6%) and wrist 27(45%). Uveitis was detected in two patients, and positivity for ANA titer was revealed in one patient.
Conclusions: In this study, the pattern of most clinical features in different subtypes of juvenile idiopathic arthritis resembles to other studies. Positive ANA was less; however, the low numbers of Iranian patients with uveitis was noteworthy.- انتشار مقاله: 02-06-1393

- چکیده انگلیسی: Primary immunodeficiency (PID) is a group of more than 400 distinct genetic disorders affecting both children and adults. As signs and symptoms of PID are usually heterogeneous and unspecific, based on the available human resources and laboratory facilities, diagnosis and follow-up of these patients can be challenging. In line with this notion, management and treatment of PID are practiced divergently in different PID centers. In order to reach a distinct national protocol and given the little evidence on appropriate or universal guidelines to improve the current status of management of the disease, the Iranian PID network designed a consensus suitable for regional resources. This review summarizes this PID guideline based on the importance of different clinical complications and the level of medical authority visiting those at the first line. Further, for each complication, appropriate interventions are mentioned to improve the approach.

Gut microbiota is a major component of the intestinal luminal environment and plays important roles in colorectal cancer.
Object:
systematically review all the existing literature on the association of mucosa-associated and fecal microbiota with incidence, location, and stage of colorectal adenoma and carcinoma.
Methods:
The scientific search was done up to July 2018. The search was limited to the English language with predefined and proper keywords. Among 616 articles some of them were eliminated due to some reasons. The inclusion and exclusion criteria were defined. In the next step two reviewers (M.M and Z.K) independently scanned the titles of all retrieved articles, removed duplicates, and identified potentially relevant abstracts for further assessment. The Newcastle-Ottawa Scale (NOS) for assessing the Quality was used for quality control.
Result:
Finally, 54 articles were entered into the study. Fusobacteria 39 (72%), Firmicutes 22(40%), Bacteroidetes 20 (37%), Proteobacteria 15(27%), Actinobacteria 10(18%) was the most prevalent phylum which was found in colorectal cancer patients. Among these taxa some of them were increased in colorectal cancer patients compared to the control; on the other hand, some taxon was declined in colorectal cancer patients. Besides this, in some taxon there were controversies among articles.
Conclusion:
Early detection of CRC is essential because patients whose cancer are detected at an early stage have more chance of survival. Until now there are several studies have demonstrated the potential rule of gut microbiota to be used for detection of CRC, but there is not any predefining protocol for screening. Although we found lots of articles which were published in this area, for defining a precise microbiota profile we need large multicenter case-control studies, where can show the effect of most important confounding factors like nutrition, ethnicity, physical activity, smoking consumption, and genetic background.- انتشار مقاله: 21-01-1399
