- چکیده انگلیسی: Introduction: Unpleasant incidents are one of the biggest problems in health system and error reporting system is an efficient way to improve patient’s safety. Therefore, this study was conducted with the purpose of assess reporting error system in one of the pediatric hospitals in Tehran and finally a proposed model for health centers was presented.
Materials and Methods: This was qualitative study. The procedures of study included four phases such as identifying status of selected hospitals in the error management system, identifying the status of error reporting systems in selected countries, identifying the features, requirements and benefits of setting up an error reporting system and develop a proposed model for health centers. Triangular method was used to collect information and descriptive content analysis and consensus opinion was used to analyze the data.
Results:Demographic, cultural, organizational, legal-regulatory and financial factors have been introduced as the main barriers of error reporting in the respective hospital. Structural and process criteria should be strengthened to establish a desired reporting system. The process of incidents detection, incidents drawing, incident analysis, review and share errors, implementing solutions and monitoring and evaluation are recommended to design an error reporting system.
Conclusion: The competitive key to manage medical’s error successfully is to establish a voluntary reporting system, commitment of senior managers, change the culture of blame and removing barriers of reporting errors. So, healthcare system needs to provide an error reporting system with learning and proactive approach to improve quality of services and safety of infrastructure.- انتشار مقاله: 11-07-1396

- چکیده انگلیسی: Introduction: Customer Quality (CQ) refers to customer’s characteristics and is related to the consumer knowledge, skills and self confidence in active participating in care process and life style improvement. This study was aimed to assess customer quality among people with Rheumatoid Arthritis (RA) according to the patients’ perspective.
Materials and Methods:This cross-sectional study was carried out on 170 patients who received care from specialist clinics of Isfahan University of Medical Sciences in 2013. Customer Quality was assessed using Comprehensive Quality Measurement in Health care questionnaire (CQMH_CQ). Questionnaire content validity was reviewed and confirmed by 10 experts and its reliability was confirmed based on Chronbach's alpha index (α=0.803). SPSS-17 statistical software was used to analyze the data. Independent Samples T test and ANOVA were conducted to compare CQ score between categorical variables.
Results: The average CQ score was 70.25 (13.20). According to self-reported customer quality score, all participants achieved the scores at the level of stage one. Some (9.2%) of participants didn’t reach the action stage (stag three), then 90.8% took action in facing with health related problem and finally only 19.8% of participant achieved highest level of Self-management. Customer Quality score of employed patients (P=0.026) and patient who had active disease
(P =0.030) were significantly. Customer quality scores of illiterate participants were lower than that of the educated (P= 0.001).
Conclusion: According to overall score of customer quality, findings indicate the necessity of patients' involvement in care process and self-thought behavior-change skills for ongoing self-management and enhancing their self care abilities in daily life.- انتشار مقاله: 25-02-1393

- چکیده انگلیسی: In this study, an unsteady aerodynamic simulation is performed to realize the influences of platform surge motion on the aerodynamic performance of a high capacity offshore floating wind turbine. A dynamic model with pitch angle control system is utilized to propose a more realistic model of wind turbine and also achieve the rated condition of the rotor. The transient effect of platform surge motion on power coefficient, thrust coefficient and blade pitch angle also is investigated. The 5 MW NREL wind turbine is selected for the simulations. The unsteady aerodynamic model contains unsteady blade element momentum method, dynamic stall and dynamic inflow models. The in-home aerodynamic code and the control system model are implemented in MATLAB/SIMULINK software. It is revealed that reduction in mean power coefficient at tip speed ratios less that 7 is expected by amount of 12-15 % at surge amplitude of 2m and frequency of 0.1 Hz. For high tip speed ratios, the trend is reverse with respect to fixed-platform case. The mean thrust coefficient is also reduced for many tip speed ratios with maximum loss of 32 %. The mean blade control pitch angle is increased due to the surge motion. Since the influence of changing amplitude and frequency of disturbances depends on the tip speed ratio, therefore the special bound of this parameter is being proposed.

- چکیده انگلیسی: Inflation targeting is a prevalent economic policy that is applied increasingly by countries. In addition, the effect of inflation targeting on macroeconomic variables is also been considered recently which mostly uses econometric models. One of the most important issues in this regard is that evaluating the effectiveness of inflation targeting usually confronted with bias in econometric models. In order to solve this problem, it is suggested the Propensity Score Matching Method (PSM) which has been used recently by economists. In this paper, it is tried to investigate the impact of inflation targeting on direct taxes and its components in a selected collection of two groups of oil importer and exporter countries by Propensity Score Matching Method (PSM) during 1990- 2016 years. The results show that adopting inflation targeting framework has a positive and significant effect on tax revenue in oil importer countries; whereas the impact of this policy in oil exporter countries is statistically insignificant and its direction is also ambiguous.

egional unbalanced growth and the factors affecting it are one of the most important economic issues in developing countries. In this research, after evaluating the regional inequality at the provincial level of Iran, in order to examine the effect of economic factors on it, it was considered as a spatial econometric model using panel data during the period of 2001 to 2015. The evaluation of Theil inequality indices and Population-Weighted Coefficient of Variation (PW-CV) shows that Iranian provinces are extremely unequal. The results of the estimation of the research model indicate the spatial dependence among the provinces. So that the regional inequality index of each province is affected by neighboring provinces with a coefficient of 34%. In the analysis of economic factors affecting regional inequality; the increase of foreign direct investment and the ratio of industrial production, reduce the inequality of the provinces. But increased provincial spending will increase regional inequality and the tax policy has failed to succeed in redistributing revenue. The results of the study of the direct and indirect effects of explanatory variables on the PW-CV shows that regional inequality is mainly influenced by direct effects, and the effect of the total coefficients on the inequality index is significant.

- چکیده انگلیسی: Abstract T his paper is to convince the usage of the nonlinear unit root tests when dealing with a nonlinear model. To do so, the stationary test for variables in a model titles “Fiscal Reaction Function in Iran” has been applied according to both the ordinary and the Nonlinear Dickey-Fuller (NDF) tests. Results show that while variables under investigation are stationary in a nonlinear form, augmented Dickey-Fuller test indicates tendency to fail and reject the null hypothesis of a unit root in the presence of nonlinear dynamics. Therefore based on the results of Nonlinear Dickey-Fuller (NDF), the paper estimates the fiscal reaction function (FRF) in Iran. The estimated nonlinear regression supports a threshold behavior of two regimes in applying the fiscal reaction. Finally, findings confirm that fiscal policy in Iran is countercyclical though not sensitive in order to react to accumulation of the government debt.

- چکیده انگلیسی: Three-dimensional surface geometry of rock discontinuities and its evolution with shearing are of great importance in understanding the deformability and hydro-mechanical behavior of rock masses. In the present research, surfaces of three natural rock fractures were digitized and studied before and after the direct shear test. The variography analysis of the surfaces indicated a strong non-linear trend in the data. Therefore, the spatial variability of rock fracture surfaces was decomposed to one deterministic component characterized by a base polynomial function, and one stochastic component described by the variogram of residuals. By using an image-processing technique, 343 damaged zones with different sizes, shapes, initial roughness characteristics, local stress fields, and asperity strength values were spatially located and clustered. In order to characterize the overall spatial structure of the degraded zones, the concept of ‘pseudo-zonal variogram’ was introduced. The results showed that the spatial continuity at the damage locations increased due to asperity degradation. The increase in the variogram range was anisotropic and tended to be higher in the shear direction; thus, the direction of maximum continuity rotated towards the shear direction. Finally, the regression-kriging method was used to reconstruct the morphology of the intact surfaces and degraded areas. The cross-validation error of interpolation for the damaged zones was found smaller than that obtained for the intact surface.

- چکیده انگلیسی: Background and Objectives: Induced demand in healthcare is referred to as provision of unnecessary services or the patient by health services providers, while the patient is not aware of their unnecessity. Apart from being unethical, this practice can potentially disturb the supply and demand balance in the health market, pose financial load on the patient, thread the patient’s health by imposing possible side-effects, and lead to waste of the limited national health resources. This study, thus, was aimed at investigating the nature of the phenomenon in Iran, as perceived by the healthcare experts. Methods: A qualitative research design was adopted. Data was collected using semi-structure interview. Participations were selected by purposive sampling method. Thematic analysis was used for extracting and categorizing the major domains of induced demand. Findings: Four major categories of health services in which induced demand occurs were extracted from the interview data, including para-clinical services, medical services, surgical services, and pharmaceutical services. These health services domains account for nineteen specific health services with the potential of induced demand. Conclusions: The study identified the health services domains in which induced demand frequently takes place. This information can help policy-makers to devise strategies for alleviating the problem.

- چکیده انگلیسی: In this work, a new iterative method is proposed for obtaining the approximate solution of a class of Hammerstein type Integral Equations System. The main structure of this method is based on the Richardson iterative method for solving an algebraic linear system of equations. Some conditions for existence and unique solution of this type equations are imposed. Convergence analysis and error bound estimation of the new iterative method are also discussed. Finally, some numerical examples are given to compare the performance of the proposed method with the existing methods.
