- چکیده انگلیسی: Background: The prevalence of allergic diseases is increasing worldwide. The effective role of omega-3 fatty acids in the maturation of immune system and protection against atopic diseases has been well discussed. However, previous studies revealed conflicting results. This study was conducted to investigate the effect of fish oil consumption by nursing atopic mothers on the level of interferon gamma (IFN-γ) in their breast milk and incidence of allergic disease in their infants.
Methods: This randomized clinical trial was conducted on 94 atopic mothers, who were assigned to two equal groups receiving either 1000 mg fish oil capsules or placebo for 60 days after delivery. The breast milk was collected 120 days after delivery, and IFN-γ level was measured. The history of the symptoms of atopic disease in infants was collected from their mothers through ISAAC questionnaire; in addition, the infants’ growth was evaluated.
Results: Sixty mother-infant pairs completed the trial. The mean values of IFN-γ in breast milk was higher in the fish oil group (1.11±1.15 Pg/ml) than in the placebo group (0.81±0.86 Pg/ml), but the difference was not significant (P=0.288). In addition, the incidence of allergic symptoms of infants was not significantly different between the two groups (P=0.84).
Conclusion: In this trial, the consumption of fish oil by lactating mothers did not have a significant effect on IFN-γ level in their breast milk and the incidence of allergic symptoms in their infants. Future studies with longer follow-ups are necessary in this regard.- انتشار مقاله: 09-02-1397

- چکیده انگلیسی: Background: Thyroid function disorders, particularly congenital hypothyroidism (CHT), are important endocrine dysfunctions associated with permanent morbidities. CHT is more prevalent among preterm low-birth-weight neonates compared to term infants with normal weight.
Methods: This prospective cohort study was conducted on 126 very-low-birth-weight (VLBW) neonates referred to the neonatal intensive care units (NICUs) of two tertiary referral hospitals affiliated to Isfahan University of Medical Sciences in Isfahan, Iran during 2012-2014. On day five of birth and two, four, and six weeks after birth, blood samples were collected from the infants to determine thyroid function disorders, including transient hypothyroxinemia, neonatal hypothyroidism, transient primary neonatal hypothyroidism, and transient hyperthyrotropinemia.
Results: In total, 126 infants with mean gestational age of 30.5±2.29 weeks and mean birth weight of 1246.90±193.58 g were enrolled in this study. Thyroid-stimulating hormone (TSH) level of Conclusion: According to the results of this study, thyroid function disorders are relatively common in preterm VLBW neonates, and serum T4 level is correlated with gestational age in these infants. Therefore, thyroid function tests with a consistent protocol are required for premature infants. - چکیده انگلیسی: Background: Health-related quality of life (HRQoL) has become a major concern in the field of children’s health research. We assessed HRQoL among Iranian children and adolescents according to the socioeconomic status (SES) of their living region.Methods: Via multistage cluster sampling from rural and urban school students aged 6 to 18 years, this nationwide study was conducted from 2011 to 2012. HRQoL was assessed using the adolescent core version of the Pediatric Quality of Life questionnaire. Through survey data analysis methods, the data were compared according to the SES of the living region, sex, and the living area.Results: Overall, 23043 students participated in the survey (participation rate=92.2%). The mean age of the participants was 12.55±3.31 years. Boys accounted for 50.8% of the study population, and 73.4% were from urban areas. At national level, the mean of the HRQoL total score was 81.7 (95% CI: 81.3 to 82.1) with a mean of 83.5 (95% CI: 83.0 to 84.1) for the boys and 79.8 (95% CI: 79.1 to 80.5) for the girls. The highest and the lowest scores, respectively, belonged to social functioning (90.0 [95% CI: 89.7 to 90.3]) and emotional functioning (78.2 [95% CI: 77.7 to 78.7]). The highest total HRQoL score belonged to the second highest SES region of the country (mean=83.1; 95% CI: 82.5 to 83.7). The association between total HRQoL and the score of all the subscales and SES in the living area was statistically significant (P<0.001).Conclusion: The results of the present study showed that in the children and adolescents, SES was associated with HRQoL. Accordingly, HRQoL and the related SES differences should be considered one of the priorities in health research and health policy.

- چکیده انگلیسی: The glomerular filtration rate (GFR) is widely considered the best overall index of renal function. The Schwartz equations are designed for measuring the GFR in children between 1 and 16 years of age. In the present study, we investigated the percentiles of the GFR in a general population of Iranian children with no known renal disease via the 2009 Schwartz equations (updated and combined equations). Between 2010 and 2011, we selected 687 children aged 7–16 years from the Iranian province of Isfahan. Blood samples were analyzed for blood urea nitrogen, creatinine, and cystatin C. For each child, we calculated the GFR using 2 Schwartz equations. The Wilcoxon test was applied to examine the differences in the estimated GFRs between the equations. To determine the correlation between the GFRs obtained via the updated and combined Schwartz equations in the boys and the girls and also for the correlation between age and the GFR, we performed the Pearson or Spearman correlation coefficients. The statistical analyses were conducted using MedCalc, version 12.1.4.0 (MedCalc Software, Mariakerke, Belgium). The mean GFR was 100.06±19.80 mL/min/1.73 m2 based on the updated equation and 96.10±18.44 mL/min/1.73 m2 according to the combined equation. No significant differences were observed between these equations in estimating the GFRs. The correlation analysis for determining the association between age and the GFR estimated by the updated (r=0.05, P=0.1) and combined (r=0.06, P=0.09) Schwartz equations was not statistically significant. In conclusion, in the current study we showed that the updated and combined Schwartz equations exhibited high concordance in estimating GFR values in this age group.

- چکیده انگلیسی: Objective(s): This study aims to investigate joint association between cholesterol ester transfer protein (CETP) polymorphisms and body mass index (BMI) or birth weight with the risk of dyslipidemia in Iranian children and adolescents.
Materials and Methods:This study was conducted as a sub-study of the “school-based nationwide health survey” (CASPIAN-III). We randomly selected 750 samples from the whole blood samples. Real-time PCR and high resolution melt (HRM) analysis were performed to determine Taq1B (rs708272) and A373P (rs5880) polymorphisms.
Results:Taq1B polymorphism increased HDL-C, and total cholesterol (TC) as well as decreased triglyceride and LDL-C concentrations. LDL-C and triglyceride levels were significantly higher and HDL-C and TC levels were significantly lower among those with A373P polymorphism. CT/TT genotype in Taq1B polymorphism showed a protective effect on dyslipidemia (OR= 0.12, 95%CI: 0.07-0.20). G allele of A373P polymorphism increased the risk of dyslipidemia (OR=4.10, 95%CI: 2.14, 7.83) after adjusting the confounders. We observed interactive effects of CETP gene polymorphisms and BMI or birth weight on dyslipidemia.
Conclusion:Findings showed Taq1B polymorphism might have a protective effect and A373P polymorphism had deleterious effect on dyslipidemia in Iranian children and adolescents. These associations interacted with BMI and birth weight.- انتشار مقاله: 10-09-1394

- چکیده انگلیسی: Background: Cystic fibrosis (CF) is the most common lethal genetic disorder of Cystic Fibrosis Trans-membrane Conductance (CFTR) Regulator gene mutations. We aimed to investigate common mutations in CF patients and to assess its possible relationship with clinical presentations.
Materials and Methods: This cross sectional study was conducted on 36 CF patients who were referred to a tertiary pediatric hospital in Isfahan, Iran. They were evaluated for 34 common mutations in CFTR gene by using reverse dot blot strip assay. Other parameters such as the age of diagnosis, the sweat chloride level, and clinical manifestations due to lung involvement and pancreatic insufficiency were also assessed. According to genotype mutations, children were divided in three groups: ΔF508 mutation (group 1), non-ΔF508 mutation (group 2), without current mutations (group 3). Finally, genotype, and phenotype relationship were reported.
Results: The mean age of patients was 8.1+2.3 months, and 23 of them (63%) were male. CFTR mutations were found in fourteen patients (38.8%). ΔF508 mutation has the highest prevalence in the studied samples with allele frequency of 15.27%, and c. 2183 AA>G was in the second standing. Furthermore, p.R553X, p.G542X, C.1766+1, p.N1303K mutated alleles also were obtained in lower level. Mean age at the diagnosis time of CF, sweat chloride level and pancreatic insufficiency were not different between groups but lung complications were significant in children with common mutations.
Conclusion: Our findings showed that commercial kit designed to identify 34 common CFTR mutations failed to detect 61.2% of alleles of our patients. This necessitates designing local diagnostic kits for proper diagnosis of CF in Iranian children.- انتشار مقاله: 27-08-1397

- چکیده انگلیسی: Background: Nutritional habits and its determinants, especially in children and adolescents have recently turned into the one of the major concerns of health researches. We examine the diet contribution inequality in according to socio-demographic factors, age, gender, physical activity and body image to alleviate this gap in Iranian children and adolescents.
Materials and Methods: Study sample was comprised of 14,880 students aged 6-18 years who selected from urban and rural districts of 30 provinces of Iran via stratified multi-stage sampling method. A short food frequency questionnaire was used for estimating the food group consumption. The Blinder-Oaxaca method was applied to investigate the inequality in the prevalence of healthy and junk foods consumption between the first and fifth socio-economic status (SES) quintiles.
Results: The frequency of healthy and junk foods consumption showed considerable differences between the SES quintiles. The highest differences were found in the frequency of fresh fruit (25.38%), vegetable (12.92%), and milk (10.74%) consumption, respectively. The daily consumption of vegetables, and fresh and dried fruits increased linearly by increasing the SES quintiles. The highest absolute difference was seen in the frequency of fresh fruit consumption between the bottom and top of the socioeconomic groups (SII value=-32%). The estimated SII was statistically significant for the consumption of all healthy and junk foods except for fast foods and milk consumption. The estimated C index for consumption of healthy and junk foods was positive and negative, respectively.
Conclusion: This study provides the considerable information on the consumption of healthy and junk foods and its determinants among Iranian children and adolescents for better programming, developing health policies, and future complementary analyses.- انتشار مقاله: 28-09-1397

- چکیده انگلیسی: Background: This study investigates the association of perceived weight status and health-related quality of life (HRQOL) in a representative sample of Iranian children and adolescents. Materials and Methods: In this study, 6-18-year-old students were selected from 30 provinces of Iran. Weight status and perceived weight status of students were determined by physical examination and using the validated questionnaire of the World Health Organization-Global school-based student health survey, respectively. The students’ HRQL was evaluated by the Persian version of the Pediatric Quality of Life inventory (PedsQL™ 4.0TM 4.0) Generic Core Scales. Results: The participants consisted of 23,043 students with mean (standard deviation [SD]) age of 12.55(3.31) years. Underweight was significantly less prevalent than perceived underweight (30.8%vs 10.0%, respectively, P<0.001), whereas normal weight was significantly more prevalent than perceived normal weight (70.8% vs. 52.5%, respectively P<0.001). The mean total PedsQL™ score and its subscales in different categories of perceived weight did not have significant difference (P>0.05). After adjustment for age, gender, region, socio-economic status, physical activity, screen time, and body mass index (BMI), significant negative association was documented between perceived overweight and total score of HRQOL. School functioning was negatively associated with perceived overweight. Positive significant association existed between perceived overweight and psychosocial functioning. Conclusion: Our findings indicated that weight underestimation is the most common form of weight misperception in Iranian children and adolescents, which could be a cause of concern especially for obesity treatment and prevention programs. Furthermore, considering the negative association between weight overestimation and total HRQOL and school functioning, it is suggested that social standards and norms impacts on weight perception and consequently HRQOL in children and adolescents.

- چکیده انگلیسی: Background: At least half the world’s population is colonized the stomach by Helicobacter pylori (H. pylori) which are a key constituent of the human microbiome. The aim of this study was to investigate the association of cardiometabolic risk factors with H. pylori infection in Iranian adolescents. Materials and Methods: The current study was conducted along with the third survey of a national school-based surveillance system in Iran, entitled "Childhood and Adolescence Surveillance and Prevention of Adult Non-communicable Disease" (CASPIAN) study. Detailed questionnaires on demographic, socioeconomic, dietary, and health-related information of the participants were filled by one of the parents under supervision of trained health-care workers. Trained healthcare professionals measured anthropometric indices, blood pressures according to standard protocols. Fasting venous blood was examined for fasting blood sugar and lipid profile.
Results: Overall 882 serum samples were suitable for testing. H. pylori antibody was found in 643 serum samples (72.9%). Among cardiometabolic risk factors, only the mean weight of participants was different between two groups (44.6±11.8 in H. pylori positive and 42.8±11.3 in H. pylori negative group; p=0.04). Overall, 5.1% of adolescents with positive H. pylori tests were overweight or obese, while 1.7% of negative ones were so (p=0.02). In the multivariate regression model, H. pyloriseropositivity increased the risk of overweight (OR, 3.3; 95%CI, 1.2-9.3; p= 0.03). In the multivariate model, association of other cardiometabolic risk factors with H. pylori infection was not statistically significant (p>0.05).
Conclusion: Results of present study showed that H. pylori infection was associated with excess weight in adolescents. H. pylori eradication may be decrease the risk of obesity.- انتشار مقاله: 29-03-1396

Background: Health status is an individual’s relative level of wellness and illness. Self-Perceived Health (SPH) is a single item considered as a health indicator for national and international survey. The aim of this study was to evaluate the SPH measure and its determinants in a National sample of Iranian children and adolescents.
Materials and Methods: This National study was performed as the fourth National survey of a school‑based surveillance program entitled the Childhood and Adolescence Surveillance and Prevention of Adult Non-communicable Disease (CASPIAN‑IV) study. 14,880 students aged 6-18 years old selected from 30 provinces of Iran by a multistage Cluster and stratified sampling method. Two sets of valid and reliable questionnaires were completed for students and their parents.
Results : Overall 13,846 participated in the present studywith 90.6 % participation. In this study, 80.5 % of boys and 79.4% of girls reported good SPH (95% confidence interval (CI), 79.3-81.6 versus 78.1-80.6, respectively). According to the living area, 79.5% from urban and 81.1% from rural area declared good SPH (95% CI, 78.6-80.5 versus 79.2-82.8, respectively). In the multivariate model, the subjects who had healthy weight compared with excess weight had significantly higher SPH (OR: 1.36; 95% CI, 1.17-1.59). Also, the subjects with high Socio-economic status (SES) had higher odds of SPH (odds ratio [OR], 1.23; 95% CI, 1.06-1.41).
Conclusion: Our results showed that SPH can be influenced by both demographic and life style related characteristics among study population. This association was stronger for SPH and academic success, talking with both parents and having normal body image respectively.
