- چکیده انگلیسی: Introduction: Lack of a clear policy for the development of healthhuman resources has created inconsistencies. These imbalancesare threats to the health system to achieve its goals. Therefore, thedevelopment of human resources through proper performance ofhigher education health system is an important part of the policydevelopment process of the health sector. The present paper aimsto introduce the methods applied for the compilation of evolutionand innovation program of medical sciences training as well asthe most important directions for evolution and innovation. Methods: In this study, we evaluated the methodology fordesigning packages of Iran’s higher education health systemevolution and innovation. For this purpose, the evaluation of thepolicy process was conducted based on Michelle and Scott’s policyprocess models. This policy evaluation model starts by problemidentification and definition and continue by agenda setting, policyformation, legitimation, implementation, evaluation, and policymodification, using the proper feedback. Qualitative contentanalysis method was used as a research method for subjectiveinterpretation of the content of the text data. Results: Twelve policies, 68 strategies and their translation in thehealth system were adopted in a comprehensive plan for higherhealth education. Eleven practical packages were also developedin order to implement these policies as packages for reform andinnovation in medical education. These packages were organizedbased on the IPOCC pattern. Conclusion: The lack of a comprehensive look at each projector program could bring about irreparableness consequences.However, the MoHME of Iran, relying on the integration of healthhigher education with health care system and comprehensivemethod used for transformation and innovation plan in the fieldof health higher education could take an important step towardsimproving the nation’s health.

- چکیده انگلیسی: Background and Objectives: The Iranian Ministry of Health and Medical Education has introduced clinical governance as the accepted model of hospital healthcare improvement. Subsequently, a growing movement for implementing clinical governance in health facilities has emerged. This study aimed to explore the opinions of the relevant experts and executives to provide insight into current challenges, barriers and inadequacies in implementation of clinical governance in Iranian health settings. Methods: A qualitative approach was adopted. A purposeful sample of 17 participants was interviewed in the spring of 2012. The study sample was selected from among clinical governance executives of teaching hospitals affiliated with Kerman University of Medical Sciences and the academicians involved in administration of the clinical governance. The Framework method was adopted for data analysis. Findings: Seven themes explain challenges of implementing clinical governance, including human resources, management, communication, training, culture, resources, and monitoring and regulations. Adequate quality human resources, particularly experts in clinical governance should be dedicated. Leadership commitment to support implementation of clinical governance should be improved. Administrators need to get more familiarized with the concept and requirements of clinical governance. High ranking authorities should avoid rapid turnover of management teams. Adequate communication regarding of clinical governance is needed to be established among different deputies of the Ministry of Health and Medical Education. Hospitals must be encouraged to share their experiences in clinical governance. Training programs should be based on needs assessment, have definite goals, and focus more on practical aspects of clinical governance. The felt need for change must be promoted among hospitals’ administrations and staff. Teamwork between staff must be promoted. Expectation for rapid achievements and early frustration in time taking reforms must be addressed by appropriate training. Appropriate infrastructure and mechanisms for reporting, interpretation, and analysis of quality indicator data should be developed. Adequate financial and physical resources should be appropriated. Efficient monitoring and assessment systems must be implemented. Progress in implementation of clinical governance should be appropriately supervised and evaluated. Constant feedback on staff performance and outcomes of interventions should be provided. Adequate supportive laws and regulations, legally guaranteeing implementation of clinical governance should be developed. Conclusions: Considering inter-relation of the identified themes and subthemes, our study recommends that a systems approach should be adopted for successful implementation of clinical governance. While fundamental solutions to the identified challenges require long-term reforms in the health system, some obstacles such as inadequate leadership support, rapid rotation of managers, lack of financial and physical resources, inefficient monitoring system, and inefficient training programs may be addressed in a relatively short run.

- چکیده انگلیسی: Background and Objectives: There is a growing tendency in Iran’s health sector for adoption of standard excellence models for improving quality in healthcare organizations. The European Foundation of Quality Management (EFQM) model is a widely used framework for continuous quality management within healthcare facilities. The purpose of this article is to extend the view of current status of quality management in Iran’s hospital industry by reporting the details of an EFQM-based self-assessment of quality management in hospitals of Kerman University of Medical Sciences. Methods: A cross-sectional study was conducted over the period of October to December 2011 by enrolling heads of departments and chief executive officers of all the four university hospitals of Kerman, the capital of Kerman province in southern Iran (n = 69). The standard EFQM questionnaire was used to measure quality management. Reliability of the assessment tool was determined by calculating Chronbach’s alpha coefficient. The data was analyzed using descriptive statistics. Findings: The criteria ‘Process’ and ‘Customer results’ scored above 60%, and other criteria scored within the range of 50% to 60%. While ‘Process’ gained the highest score of 62.9%, the lowest score of 52.1% was received by ‘Key performance results’. Scores appeared to be overestimated compared to those gained by pioneer healthcare settings in developed countries. However, the scores were found more realistic in comparison with those in precedent domestic studies, suggesting a trend of gradual improvement of quality management knowledge and self-assessment skill. Conclusions: The data suggest that the criteria ‘People’ and ‘Resource and Partnership’ are of a high priority for improvement in order to reach satisfactory ‘Key performance results’.
