- چکیده انگلیسی: Background Justice has gained much attention in social and human studies and has many consequences on employees and the organizations, especially on health system workers such as nurses who are among the key factors in health care systems. Objectives The purpose of this study was to investigate perception of organizational justice among nurses in educational hospitals of Shiraz University of Medical Sciences (SUMS), and to compare the results of general and specialty hospitals. Materials and Methods In this research, 400 nurses at SUMS hospitals were selected by random sampling method. A 19-item questionnaire was applied to measure distributive, procedural and interactional justice. Data analysis was performed using descriptive statistics, including percentage, frequency, mean, and standard deviation. Also, the t-test and one way ANOVA were used to measure the differences between different hospitals and wards. Results Of 400 nurses, 66% perceived a high level of organizational justice. In this study the mean scores of total perceived organizational justice (P = 0.035), procedural justice (P = 0.031), and interactional justice (P = 0.046) in specialty hospitals were higher than general ones. Furthermore, the mean score of interactional justice was higher than the other components of organizational justice, respectively 3.58 ± 1.02 for general and 3.76 ± 0.86 for specialty hospitals. Significant differences were observed between overall perceived justice (P = 0.013) and its components (P = 0.024, P = 0.013, and P = 0.036) in different wards. Conclusions Most nurses who participated in this study had a high perception of organizational justice. The mean score of organizational justice was higher in specialty hospitals. Health care policy makers and hospital managers should support their employees, especially nurses through fairness in distributions, procedures, and interactions.

- چکیده انگلیسی: Introduction: Despite the provision of health care in Iran, brucellosis is still an endemic disease, so this study was carried out to determine the epidemiology of brucellosis in Andimeshk city during 2001-2016.
Materials and Methods: This cross-sectional study was conducted on patients with brucellosis, which were reported by governmental and non-governmental departments and received relevant diagnostic and laboratory information after receiving reports from patients. The obtained information was entered in the survey form. Sampling was done by census. SPSS version 20 was applied for analysis of data using descriptive statistics as well as the chi-square and independent t-test; a significant level of 0.05 was considered.
Results: In the study period, there were 713 patients with brucellosis in Andimeshk. There were 390 males and 324 females with a male to female ratio of 1.02:1. The age of the patients ranged from 1 to 74 years with a mean of 31.19±17.03 years. The annual incidence rate per 100,000 persons was 24.42. There was a statistically significant association (p <0.05) between sex and exposure to infected milk and cattle. Also, the results showed that there was no statistically significant association (p>0.05) between the infected milk and job, year, season and month.
Conclusion: In conclusion, the findings of this study showed that the incidence of brucellosis was higher among males, young people, and dairy farmers. Therefore, continuous surveillance and efforts are required to further decrease the cases of brucellosis.- انتشار مقاله: 28-07-1396

- چکیده انگلیسی: Background/Objectives: Selecting the best set of input and output indicators and allocation of correct weights to them is a sensitive step in any efficiency evaluation study. Therefore, the present study aims to determine and rank the efficiency indicators of hospitals.
Method: This mixed-method study was carried out in three steps: comprehensive literature review, application of the Delphi method to determine the best indicators for efficiency evaluation of the hospitals, and utilization of a fuzzy analytic hierarchical process (FAHP) for weighting of final indicators and ranking them.
Results: 8 input and 9 output indicators were selected for efficiency evaluation of the hospitals which were weighted by FAHP. Among the input indicators, the number of physicians and active beds and among the output indicators, length of stay and number of surgeries were identified to be the most important indicators.
Conclusion: According to the proposed indicators and their accurate weights, efficiency evaluation of hospitals can be done more accurately, reliably, and comprehensively.

- چکیده انگلیسی: Introduction: Actions of healthcare workers paired with the use of complex forms of technology can have positive diagnostic and restorative effects, but also can produce negative outcomes. The aim of this study was to implement and then assess the WHO’s 9 patient safety solutions. We also attempted to identify ways to improve compliance in our hospital’s pediatric cardiology and neurology departments.
Methodology: In this study, a form was designed to collect information regarding the current state of implementation for each of the 9 patient safety solutions. Forms were distributed among Nemazi Hospital nurses for initial evaluation, and analyzed by SPSS Statistics, Version 18.0. The two solutions with the lowest compliance scores were selected for intervention. These included “assuring medication accuracy at transitions in care” and “avoiding catheter and tubing misconnections.” To improve these two solutions, an interventional program was designed.
Results: Compliance scores from the cardiology department concerning “assuring medication accuracy at transitions in care” solution increased from 34.16% to 75.00% after intervention. In the neurology department compliance scores increased from 36.66% to 68.33%. Regarding the “avoiding catheter and tubing misconnections” solution, scores increased from 46.87% to 72.91% in the cardiology ward and 40.27% to 67.53% in the neurology department.
Conclusion: Results indicate that interventions, such as training courses, checklists and reporting forms concerning medication reconciliation improved compliance. This was also true for use of checklists to improve compliance in the proper catheter and tubing connections solution area.- انتشار مقاله: 26-11-1398

- چکیده انگلیسی: Introduction: Hospitals are complex organizations faced with continuous fundamental changes complicated by expertise diversity. The aim of this study was to investigate the factors influencing hospital employees’ resistance to change.Methods: This cross-sectional study included 510 employees working in hospitals affiliated to Shiraz University of Medical Sciences; they were selected using stratified random sampling. Data were collected through a questionnaire. A structural equation modeling technique using LISREL Version 8.8 tested the hypotheses and conceptual model. Results: Results indicated that dispositional resistance to change and involvement with change directly affected the employees’ resistance to change. Perceived benefits of change to involvement in change were significant (T-values >1.96) and the hypotheses related to these paths were accepted. Perceived benefits of change indirectly affected the employees’ resistance to change by influencing involvement. Dispositional resistance to change and involvement in change (mediator variables) were estimated 0.89, 0.03, and 0.47, respectively. Conclusion: The study results indicated that hospital managers should encourage the employees to actively participate in their change program by strengthening perception of the benefits. - چکیده انگلیسی: Introduction: Efficiency evaluation of universities and facultiesis one of the tools that help managers to identify the departments’strengths and weakness. The main objective of the presentresearch was to measure and compare the technical efficiency ofShiraz school of medicine departments using Data EnvelopmentAnalysis (DEA) technique.Methods: This cross-sectional and retrospective study wasperformed on clinical and non-clinical departments in researchand education domains over the period of 2006 to 2011. Differentinputs and outputs were considered for research and educationaldomain separately. Efficiency was measured based on theobserved optimal performance.Results: Findings showed that pathology and anatomydepartments achieved the score of 100 in technical efficiency ineducation during 2006 to 2011. During this period, parasitology,psychiatric and pediatrics department’s achieved the score of100 for technical efficiency in research domain. The lowestmean of relative educational efficiency belonged to orthopedicdepartment; as to relative research efficiency, the lowest meanwas shown in orthopedics and genetics departments. The meantechnical efficiency of non-medical departments in education andresearch domain was 91.93 and 76.08, respectively, while the meantechnical efficiency of the clinical department in educational andresearch fields was 91.02 and 82.23, respectively.Conclusion: Using multiple input and output in DEA techniqueprovided a comprehensive evaluation of efficiency in Shiraz schoolof medicine departments. The DEA could successfully estimatethe technical efficiency of the departments in research andeducational fields. Moreover, the deficiency in each departmentwas found; this could help them to plan for improvement.Keywords: Education; Medical school; Efficiency; Resource allocation

- چکیده انگلیسی: Introduction: Hospital managers, as one of the most important health care workers, are the subject of continuous education. In some countries around the world, health management education is an important part of health reforms. The aim of present research is to perform evidence-based studies based on assessing hospital managers’ needs. Therefore, it should be considered that educational need assessment regarding hospital managers is essential in today’s health care management. Methods: A total of 26 hospital managers were surveyed using a data gathering form (questionnaire) including 59 open questions designed in order to obtain data in 3 different areas. These included managers’ insight towards job duties and the capabilities required to fulfill their duty along with educational needs determined by managers. The next step involved presenting standard description of job duties to hospitals managers and asking them to document their educational needs regarding capabilities required to fulfill their duties. The standard description of job duties originated from ISO 9001 certified hospitals. For each manager, a score ranging from 0 to 19 was attained. For each correct matched statement, a positive point was considered and in the case of unmatched statements, managers received no points. Results: Out of 26 participating managers, only 20 did state the educational needs of hospitals managers. In describing job duties of hospital managers, the mean scores regarding hospital managers of public sector was 10.27 while it was 8 with private hospital managers. Conclusion: The findings of our study indicate that hospital managers as the leaders of the most sophisticated health care facilities delivering expensive complicated services need proper training regarding job competency. Therefore, it seems that continuous need assessment in this area should be carried out.Keywords: Health care, Hospital, Manager, Need assessment

- چکیده انگلیسی: The importance of health indicators in the recent years has created challenges in resource allocation. Balanced and fair distribution of health resources is one of the main principles in achieving equity. The goal of this cross-sectional descriptive study, conducted in 2010, was to classify health structural indicators in the Fars province using the scalogram technique. Health structural indicators were selected and classified in three categories; namely institutional, human resources, and rural health. The data were obtained from the statistical yearbook of Iran and was analyzed according to the scalogram technique. The distribution map of the Fars province was drawn using ArcGIS (geographic information system). The results showed an interesting health structural indicator map across the province. Our findings revealed that the city of Mohr with 85 and Zarindasht with 36 had the highest and the lowest scores, respectively. This information is valuable to provincial health policymakers to plan appropriately based on factual data and minimize chaos in allocating health resources. Based on such data and reflecting on the local needs, one could develop equity based resource allocation policies and prevent inequality. It is concluded that, as top priority, the provincial policymakers should place dedicated deprivation programs for Farashband, Eghlid and Zaindasht regions.

- چکیده انگلیسی: Background: In recent years use of family physicians has been determined as a start point of health system reform to achieve more productive health services. In this study we aimed to assess the cost-efficiency of the implementation of this plan in Fars province, southern Iran.
Methods: This cross-sectional descriptive study was done in 2007 in 18 provincial health centers as well as 224 rural health centers in Fars province. Data were collected using forms, statistics, and available evidence and analyzed by expert opinion and ratio techniques, control of process statistics, and multi indicator decision model.
Results: Although in the family physician plan more attention is paid to patients and the level of health training, availability, and equity has improved and the best services are presented, it has not only decreased the costs, but also increased the referrals to pharmacies, laboratories, and radiology clinics and the costs of healthcare.
Conclusion: Although the family physician plan has led to more regular service delivery, it has increased the patients’ referral to pharmacies, laboratories, and radiology centers and more referrals to family physicians. It seems that the possibility of setting regularity in health system can be gained in the following years of the family physician program mainly via planning, appropriate management and organizing correct health plans according to need assessments, and continual supervision on activities, which would happen according to current experiences in this plan.- انتشار مقاله: 13-09-1392

- چکیده انگلیسی: Introduction: Human resources impose a vast expense on health organizations. Therefore, improvement of the productivity of human resources is of considerable concern to executive managers of every country.
Methods: In the present study, first, the knowledge workers’ productivity assessment questionnaire was localized. Then, the knowledge workers of the central field of Shiraz University of Medical Sciences were investigated regarding productivity and affecting factors thereof.
Results: In this analytic and cross-sectional study, the questionnaire designed by Antikainen et al, was used as the pattern. 300 knowledge workers of the central field of this University were selected through the stratified random sampling in June 2011. Moreover, the data were analyzed through factor analysis, and etc.
Conclusion: Factor analysis led to the identification of eight main components of the knowledge workers’ productivity. The reliability of the new version of the questionnaire was confirmed by the Cronbach’s alpha coefficient of 0.945. Additionally, in this sample, productivity level of 19.3% of employees was low and 80.7% was favorable.
