- چکیده انگلیسی: Introduction: Annually, millions of Muslims all over the world observe the fasting rules based on its measures; this highlights the importance of studies in this field as a worthwhile model for intermittent fasting. It is obvious that changes in lifestyle over fasting have outstanding effects on physiological parameters. The current study was carried out to investigate the IOP and serum electrolytes as two important factors that are influenced by human lifestyle. Methods: Eighty-nine fasting and healthy participants including men and women with mean age of 34.97 were included in our study based on the inclusion and exclusion parameters. During this project, Ramadan coincided with the summer (between June and July 2015). All participants were monitored by an expert examiner and blood samples were collected and IOP was measured by tonometer (Topcon, 1-75, Hasunuma-cho, Itabashi-KU, Tokyo, Japan 2014). The participants were examined one week before and one week after Ramadan. Results: There was a significant reduction in physiological IOP in healthy people after Ramadan. In line with this, the serum electrolytes were altered by fasting so that Na, K, Se experienced a significant decrease while serum phosphorous increased (P<0.05). Conclusion: Prolonged intermittent fasting in Ramadan changed IOP and serum electrolytes in healthy people within a normal range.

- چکیده انگلیسی: Introduction: Bread is the staple food of most Muslims and can be considered to be a component with a remarkable effect on satiety and appetite during Ramadan fasting. This study aimed to present the recent advances in investigating the effect of different types of bread and enrichments on satiety and appetite. Methods: In this paper, articles focusing on the effect of various bread types (including enriched bread) on satiety and appetite, particularly during fasting were reviewed. Articles were found in databases such as ISI, PubMed and Google Scholar. Results: Different bread types with lower glycemic index are recommended for Ramadan fasting, especially for the Sahur meal, due to better satiety and sglycemic control. In addition, fermentable dietary fibers, such as arabinoxylans, β-glucan, fructans, and resistant starch, can influence appetite through fermentation in the colon by saccharolytic bacteria and gastrointestinal tract releasing hormones changes. Consumption of wholemeal bread results in the moderation of satiety and starvation. Barely, oat and rye breads demonstrate the better improvement of satiety compared to white wheat bread due to their higher fiber content, probiotic ingredients and steadier glycemia. On the other hand, use of protein-rich breads can result in delayed gastric emptying, steadier insulin levels and higher satiety. Beta-glucan enrichment shows similar significant results in terms of reducing hunger and increasing satiety by influencing the appetite and satiety and regulating hormones such as insulin, ghrelin and PYY. Fiber enrichment and probiotics (Fructo-oligosaccharides/Inulin) could also be considered in this regard. Conclusion: During Ramadan fasting, barley bread, oat bread and wholegrain wheat bread could be suggested as the preferred bread types to be incorporated into the main meals to improve satiety and decrease hunger feeling.

- چکیده انگلیسی: Ramadan is considered as holy month of Muslims worldwide. During this month, most able-bodied Muslim adults refrain from foods, drinks, smoking, and medications from dawn until sunset. In Islamic rules, patients and those for whom fasting may have major health risks are exempted from fasting during Ramadan. There is still no consensus regarding the effects of Ramadan fasting, as a unique opportunity for lifestyle modifications, on cardiovascular risk factors in the Muslim population. The aim of this scientific literature review was to gather comprehensive results with regard to the effects of Ramadan fasting on major cardiovascular risk factors. This literature review focused on the effects of Ramadan fasting on four main factors, including body weight, blood glucose level, blood pressure, and plasma lipoproteins. According to the results, fasting during Ramadan is a good opportunity for patients and healthy subjects for control and prevent from diabetes, hypertension, hyperlipidemia and perhaps body fat mass as major risk factors of cardiovascular diseases, however diabetic patients need to know about physical recommendations for fasting during. However, limitations of the conducted studies should be considered in specific populations while interpreting the results.

- چکیده انگلیسی: Introduction: During Ramadan, adult Muslims abstain from drinking and eating from sunrise to sunset. This religious practice influences individuals’ lifestyle factors such as eating behavior, meal schedule, and sleep pattern. These changes may affect endocrine and neuroendocrine circadian patterns, and consequently, cardiovascular indices. This study was performed to investigate the effects of Ramadan fasting on serum high-sensitivity C-reactive protein (Hs-CRP) and homocysteine as the risk factors for cardiovascular disease and body composition in the Iranian population. Methods: Healthy volunteers who fasted at least during 20 days of Ramadan were included in the study. Body composition and biochemical markers were measured pre- and post-Ramadan fasting. For normally distributed parameters, paired samples t-test was performed for analyzing the differences between the results, and Wilcoxon Signed Ranks test was run for non-normally distributed parameters. All the data was analyzed by SPSS, version 11.5. Results: Fifty-one healthy participants with the mean age of 36±10 years were enrolled in this study. Our analyses showed a reduction in body mass index (BMI) and fat mass pre- and post-Ramadan fasting. However, lean body mass and total body water remained unchanged by fasting. Variation in the serum Hs-CRP and homocysteine were not statistically significant. The results were the same across genders. Conclusion: Our study demonstrated that Ramadan fasting may lower fat mass in fasting volunteers with no adverse effects on inflammatory biomarkers of cardiovascular disease.

- چکیده انگلیسی: Introduction: Nonalcoholic fatty liver disease (NAFLD) is a global disease which its prevalence is about 10-35%. Several factors are involved in the pathogenesis of the disease. The present study was conducted to evaluate the effect of fasting during Ramadan on plasma free fatty acids in patients with NAFLD. Methods: This cross-sectional study was performed during the month of Ramadan in June-July, 2014 (Islamic year: 1435) with 50 patients who were living in Mashhad, Iran. The participants were recruited from 18-65 years old patients. The inclusion criteria were 1) patients with NAFLD that diagnosed fatty liver by ultrasonography and 2) being at least 10 hours fasting. Levels of plasma free fatty acids (Palmitic, Elaidic and Oleic fatty acid) were analyzed in blood sample of all patients by gas chromatography apparatus equipped with a flame ionization detector (GC-FID). Result: results indicated that there was no significant changes were observed in plasma levels of Palmitic, Elaidic and Oleic fatty acids in overweight patients (BMI 25-30 ), but plasma levels of Elaidic acid significantly increased in obese patients (P

- چکیده انگلیسی: Considering the fact that many Muslims with diabetes prefer to fast in spite of inhibitory advises and to answer several conflicts and controversies regarding diabetes and Ramadan, IDF in collaboration with Diabetes and Ramadan (DAR) International in Alliance decided to develop a comprehensive guideline to be used by health care professionals. This guideline was then developed on April 2016 by 12 main authors and 20 co-authors from all around the world, and published by International Diabetes Federation. The guideline is provided in nine chapters: 1) Introduction to the IDF-DAR Practical Guidelines, 2) Epidemiology of Diabetes and Ramadan Fasting, 3) Physiology of Ramadan Fasting, 4) Risk Stratification of Individuals with Diabetes before Ramadan, 5) Diabetes and Ramadan: A Medico-religious Perspective, 6) Pre-Ramadan Education, 7) Ramadan Nutrition Plan (RNP) for Patients with Diabetes, 8) Management of Diabetes during Ramadan and 9) Identifying and Overcoming Barriers to Guideline Implementation. This guideline tries to answer three main questions: 1) Is fasting during Ramadan associated with a significant risk? 2) What are the criteria that predispose patients with diabetes to increased risk during fasting? 3) What is the most appropriate oral anti-diabetic drug(s) or type and regimen of insulin for patients with type 2 diabetes who fast? - چکیده انگلیسی: The month of Ramadan, as a model of intermittent fasting, is a valuable opportunity to investigate the effects of dietary modifications on human metabolism. Fasting improves insulin sensitivity, reduces atherogenic risk, oxidative stress, and inflammation. Inflammation plays a key role in the pathogenesis of different disorders including atherosclerosis, metabolic syndrome and cardiovascular diseases. Ramadan fasting can positively modulate cardiovascular risks and improves the metabolic syndrome features through suppression of inflammatory responses. In this review we attempt to present recent studies that addressed the regulatory role(s) of this nutritional status on inflammation in patients with inflammatory diseases. These studies suggest that the anti-inflammatory effect of fasting is significant and could be considered as a complementary therapeutic approach in treatment of inflammatory disorders in patients.

- چکیده انگلیسی: The 17th Congress of Parenteral and Enteral Nutrition Society of Asia (PENSA 2016) and the first Symposium on Fasting and Health will be held on 6th to 8th December 2016 at Olympic Hotel, Tehran, Iran. The congress is hosted by Tehran University of Medical Sciences, Semnan University of Medical Sciences and Scientific Committee of Iranian Nutrition Society. This congress is organized with the partnership of Mashhad and Kermanshah Universities of Medical Sciences, Journal of Fasting and health, Nutrition and Health technology Assessment (HTA) Departments in Iran Ministry of Health and Medical Education as well as National Nutrition & Food Technology Research Institute (NNFTRI). The 17th congress of PENSA presents the first symposium on "Fasting and Health", which includes all relevant research reports about fasting and its effects on human health, fasting and diseases and update guidelines related to fasting. Dr. Nematy who is chairperson of “Journal of Fasting and Health” is the manager of this symposium and several experts from different countries are co-chairpersons. Accepted abstracts in the symposium will be published in winter issue of Journal of Fasting and Health.

- چکیده انگلیسی: Background & Aims: Nonalcoholic steatohepatitis (NASH) is part of nonalcoholic fatty liver disease (NAFLD). No special medical treatment is known for this disease, and lifestyle modification is the best known method of treatment. We aimed to compare the effect of diet and aerobic exercise with that of diet alone on the quality of life of patients with NASH. Methods: In the present study, 25 patients within the age of range of 18-55 years were randomly divided into two groups of diet along with aerobic exercise (n = 12) and diet alone (n = 13). The low-calorie diet in both groups included 500 kilocalories of energy less than the estimated daily energy requirement. In addition to diet, the first group participated in aerobic exercise for a period of 12 weeks, 3 days a week with 55–60% heart rate reserve. Quality of life score was measured by the short form-36 (SF-36) and ultrasonography at the beginning and the end of the study. Results: In assessment of quality of life in diet along with aerobic exercise group, physical function, performance limitations due to illness, physical component score, general health, and vitality showed significant changes. In the diet alone group, general health and vitality improved after the intervention. Moreover, a significant reduction was observed in ultrasonographic features of fatty liver of those who also had aerobic exercise. Conclusion: Results of this study showed that addition of aerobic exercise to low-calorie diet was more effective in the improvement of quality of life and ultrasonographic features of patients with NASH

- چکیده انگلیسی: Background: The optimum nutritional support of critically ill neonates is considered an essential aspect of their medical management since they are susceptible to rapid nutritional depletion, loss of fat-free mass, organ failure, delayed wound healing, and diminished immune function in the post-gastrointestinal surgery state. Providing appropriate nutritional support for these high-risk patients is a very complex and critical process accompanied by many potential errors. To the best of our knowledge, this study has been the first attempt investigating the energy and protein adequacy and probable barriers to the achievement of nutritional goals in such patients.
Methods: The present study was carried out at Akbar Children’s Hospital in Mashhad, Khorasan Razavi province, Iran, during 8 months in 2019. All the gastrointestinal surgical patients admitted to the neonatal intensive care unit (NICU) for at least 72 h were eligible for enrollment in the study. The information on age, gender, primary surgical diagnosis, route of nutritional support, adequacy of received energy and protein, probable barriers to the achievement of nutritional goals, and clinical outcomes were collected in this study.
Results: Totally, 59 eligible neonates were included in the study 59.3% (n=35) of whom were male. Among different methods of nutritional support, enteral nutrition (47.5%) was the most frequently used feeding route. Energy and protein adequacy was observed in 35.5% and 79.7% of the patients, respectively. The fluid restriction was the major barrier to the provision of optimum nutritional support, affecting 38.9% of the patients. Finally, nutritional adequacy was observed to be significantly associated with decreased infection rate and length of hospital stay.
Conclusion: While 64.5% of the studied neonates did not receive adequate energy, almost 80% of them had adequate protein intake during their post-gastrointestinal surgery state. The awareness of the fluid restriction and non-availability of calorie-dense solutions as the most frequent barriers to the achievement of nutritional goals may lead to making reasonable and realistic decisions on the customized protocols of the NICU patients as well as medical management and insurance coverage of required nutritional products.- انتشار مقاله: 16-11-1398
