The phenomenon of neglected foreign bodies is a significant cause of morbidity in soft tissue injuries and may present to dermatologists as delayed wound healing, localized cellulitis and inflammation, abscess formation, or foreign body sensation. Localization and removal of neglected soft tissue foreign bodies (STFBs) is complex due to possible inflammation, indurations, granulated tissue, and fibrotic scar. This paper describes a simple method for the quick localization and (surgical) removal of neglected STFBs using two 23-gauge needles without ultrasonographic or fluoroscopic guidance.
Materials and Methods:
A technique based on the use of two 23-gauge needles was used in 41 neglected STFBs in order to achieve proper localization and fixation of foreign bodies during surgery.
Results:
Surgical removal was successful in 38 of 41 neglected STFBs (ranging from 2–13mm in diameter).
Conclusion:
The cross-needle-guided technique is an office-based procedure that allows the successful surgical removal of STFBs using minimal soft tissue exploration and dissection via proper localization, fixation, and propulsion of the foreign body toward the surface of the skin.- انتشار مقاله: 18-03-1391

- چکیده انگلیسی: Lichen planus pigmentosus (LPP) develops as an asymptomatic and dusky pigmentation of the facial skin with no effective treatment. Herein we report a 54-year-old lady with clinical and histopathologic confirmation of LPP who was scheduled to be treated with CO2 laser resurfacing. After tumescent anesthesia, the whole facial skin was treated with three passes of Ultrapulse conventional CO2 ablation. The energy used in the first, second, and third passes were 6.6, 5.4, and 4.8 mJ, respectively. The mandibular area was treated with two passes (4.2 and 3.6 mJ). The eye contours were treated with two passes (3.9 and 3.3 mJ). The patient’s face remained edematous for a week, with the facial skin oozing within the first three days before it became crusted. The crust shed out from day three to six. After a week, smooth and erythematous skin appeared. The erythema persisted for up to four months, after which uniform skin with considerable bleaching and rejuvenation developed. No recurrence occurred within 18 months of follow-up. We conclude that LPP can be treated with CO2 laser resurfacing without any complications.

- چکیده انگلیسی: The patients with universal vitiligo and residual pigmented patches usually do not benefit from available repigmenting methods and are better scheduled for depigmentation by medical, laser,
and or freezing procedures. A 37-year-old lady with universal vitiligo and remaining pigmentation in the left anterior chest wall, both ventral forearms, and diffuse mottled depigmentation of the face was scheduled for facial depigmentation by 532 nm Q-switched Nd-YAG laser. All residual pigmented facial areas were treated uniformly. Two months after laser therapy, we paradoxically observed considerable repigmentation of the treated areas with only some remaining scattered and round depigmented patches. Through an unknown mechanism, a true reverse Koebner phenomenon with its resultant repigmentation rather than depigmentation was observed in our patient.- انتشار مقاله: 04-10-1398

- چکیده انگلیسی: Background: Lichen simplex chronicus (LSC), nodular prurigo (NP), and neurotic excoriation (NE) are considered as psychogenic pruritus disorders. Atopic dermatitis (AD) is also classified by some as a psychocutaneous disorder to point out the influence of psychologic factors in its recurrence, persistence and precipitation. AD and psychogenic pruritus share many common features including immunohistology, clinical course and their psychiatry. The aim of this assay was to study the correlation between psychogenic pruritic disorders and atopy.
Methods: Ninty-two patients with psychogenic pruritus, including 57 cases of LSC, 27 cases of NE and 8 cases of NP, were referred to us within a period of 18 months. The patients were studied in our private Dermatology-Psychiatry liaison Clinic in order to find out the correlation of those disorders with atopy. Ninty-two healthy individuals with no apparent dermatologic disoder were selected from the general population as controls. Patients were considered atopic if they had the history of dermatitis, asthma or hay fever with the typical age of onset, distribution, seasonal variation, and history of remission and recurrence. Atopy was investigated by physical examination and thorough history by completing a questionnaire containing all reported major and minor criteria in the literature.
Results: Forty five out of 92 patients with psychogenic pruritic disorders (48.9%) were atopic which was significant in comparison with the control group (20.6%) (p<0.0001, odd ratio = 3.68 (3.29 – 4.24), and CI = 99%).
Conclusion: This study showed that atopic state seemed to be more common in patients with psychogenic pruritic disorders.- انتشار مقاله: 11-10-1348
