- چکیده انگلیسی: Background: Inflammatory bowel disease, an autoimmune disease, has two clinical manifestations including Crohn’s disease and ulcerative colitis (UC). IL-17 has been the target of intensive research in autoimmune diseases. The influence of Toll like receptor 4 (TLR-4) gene polymorphisms on IL-17 production has also been revealed in UC patients and tissue inflammation in mice.
Objectives: To investigate the association between the TLR-4 gene polymorphisms, Asp299Gly and Thr399Ile and IL-17 serum levels with ulcerative colitis. Additionally, we aimed to study modulation effects of forenamed gene polymorphisms on IL-17 serum levels in UC patients and controls.
Methods: A total of 256 healthy controls and 85 UC patients enrolled in our study. DNA was extracted and PCR-RFLP technique was employed to determine Asp299Gly and Thr399Ile polymorphisms in TLR-4 gene and IL-17 serum levels were measured by ELISA method.
Results: There was no significant difference between the frequency of Asp299Gly A>G and Thr399Ile C>T in UC patients and controls. While IL-17 serum levels in UC patients were significantly higher than controls (p=0.003), no significant difference in IL-17 levels between different genotypes existed. Additionally, a significant inverse relationship was observed between hemoglobin level and IL-17 serum levels in UC patients (p=0.039).
Conclusions: Increased IL-17 serum levels in our UC patients might be explained through the synergistic activity of IL-17/IL-23 axis and pro-inflammatory cytokines, causing severe clinical outcome in patients with IBD. The prolonged excretion of blood in stool driven by inflammatory process which causes iron metabolism disorder and anemia may elucidate the inverse correlation between hemoglobin and IL-17 serum levels in UC patients. Lack of association between the TLR-4 gene polymorphisms and UC in our study was consistent with the results from other Caucasian populations.- انتشار مقاله: 15-05-1395

- چکیده انگلیسی: Background: Explaining the risk factors of waterpipe smoking (WS) is very necessary in prevention programs. The aim of this study was determining the risk factors and the prevalence of WS in adolescents based on the social cognitive theory (SCT).
Materials and Methods: This cross-sectional study was conducted among urban adolescents in two provinces located in the South East of Iran. We recruited 1,218 adolescents (girls=595 and boys=623) through multistage sampling during the period October 2017 to February 2018. The data collection tool was a self-administered standardized questionnaire that included basic baseline characteristics, SCT constructs items and questions about WS behavior in the participants. The data were analysed using SPSS software version 22.0.
Results
The ever use prevalence of WS in boys was 43.8% (n= 264), and in girls 27% (n=160). WS in family member (OR=1.87, CI: 1.32-2.63; p<0.001) and intimate friends (OR=2.34, CI: 1.70- 2.26; p<0.001) were the most important risk factors for adolescents. In the relation of WS with constructs of SCT: outcome expectations (OR=0.64, 95% CI: 0.50-0.84; p<0.001), outcome expectancy, (OR=0.65, 95% CI: 0.55-0.85; p=0.002) self-efficacy, (OR=0.30, 95% CI: 0.23- 0.41; p<0.001), and situational perception (OR=0.63, 95% CI: 0.47- 0.84: p=0.002) were the most important and related factors of WS. Boys were 1.92 times (OR: 1.06 – 2.43; p<0.001) more exposed to water-pipe smoking than girls.
Conclusion
The prevalence of WS was notable in students especially in boys. Peer group and family are important risk factors for tendency toward WS in adolescence. Also, lower score in SCT constructs such as knowledge, self-efficacy and situational perception in adolescents are important risk factors for WS in participants.- انتشار مقاله: 04-03-1398

Providing population-based data on awareness, attitude and practice of drug and stimulant use has policy implications. A national study was conducted among Iranian general population to explore life time prevalence, awareness and attitudes toward opioids and stimulant use.
Methods
We recruited subjects from 5 provinces with heterogenic pattern of drug use. Participants were selected using stratified multistage cluster sampling. Data were collected using a validated self-administered questionnaire. Logistic regression model was applied to identify the variables that are associated with drug and stimulant use.
Results
In total 2065 respondents including 1155 men (33.96 ± 10.40 years old) and 910 women (35.45 ± 12.21 years old) were recruited. Two-third of respondents had good awareness about adverse effects of opioid use. Corresponding figure in terms of stimulants was 81.4%. Almost 95% of participants reported a negative attitude towards either opioid or stimulant use. The lifetime prevalence of opioid use and stimulant use were 12.9% (men: 21.5%, women: 4.0%) and 7.3% (men: 9.6%, women: 4.9%), respectively. Gender (adjusted odds ratio [AOR]M/W = 6.92; 95% CI: 2.92, 16.42), education (AORundergraduate/diploma or less = 0.49; 95% CI: 0.26, 0.90), and marital status (AORothers/single = 2.13; 95% CI: 1.36, 3.33) were significantly related with opioid use. With respect to stimulant use, age was negatively associated with the outcome (AOR60+/20-29 years = 0.08: 95% CI; 0.01, 0.98) and men were 2 times more likely than women to use stimulants (ORM/W=2.15: 95% CI: 0.83, 5.56). In addition, marital status (AOROthers/singles = 3.45; 95% CI: 1.09, 10.93), and awareness (AORWeak and moderate/good = 0.40; 95% CI: 0.25, 0.61) were independently correlated with stimulants use.
Conclusion
While the attitude of Iranian adults toward opioid and stimulant use was negative, their awareness was not that adequate to prevent the drug use. Men and those with lower socio-economic status (SES) should be the focus of health promotion programs regarding opioid use. However, regarding stimulants use, promotion programs should target younger age groups and those with higher SES status.- انتشار مقاله: 05-09-1396

In Islamic countries alcohol consumption is considered as against religious values. Therefore, estimation of frequency of alcohol consumptions using direct methods is prone to different biases. In this study, we indirectly estimated the frequency of alcohol use in Iran, in network of a representative sample using network scale up (NSU) method.
Methods
In a national survey, about 400 participants aged above 18 at each province, around 12 000 in total, were recruited. In a gender-match face to face interview, respondents were asked about the number of those who used alcohol (even one episode) in previous year in their active social network, classified by age and gender. The results were corrected for the level of visibility of alcohol consumption.
Results
The relative frequency of alcohol use at least once in previous year, among general population aged above 15, was estimated at 2.31% (95% CI: 2.12%, 2.53%). The relative frequency among males was about 8 times higher than females (4.13% versus 0.56%). The relative frequency among those aged 18 to 30 was 3 times higher than those aged above 30 (3.97% versus 1.36%). The relative frequency among male aged 18 to 30 was about 7%.
Conclusion
It seems that the NSU is a feasible method to monitor the relative frequency of alcohol use in Iran, and possibly in countries with similar culture. Alcohol use was lower than non-Muslim countries, however, its relative frequency, in particular in young males, was noticeable.- انتشار مقاله: 18-09-1394

The aim of this study was to examine the impacts of multiple indicators of Socio-economic Status (SES) on Congestive Heart Failure (CHF) related readmission.
Methods
A prospective study consisting of 315 patients without the history of admission due to CHF was carried out in Tehran during 2010 and 2011. They were classified into quartiles based on their SES applying Principal Component Analysis (PCA), and followed up for one year. Using stratified Cox regression analysis, Hazard Ratios (HRs) were computed to assess the impact of SES on the readmission due to CHF.
Results
During the 12 months follow-up, 122 (40%) were readmitted at least once. HR of lowest SES patients vs. the highest SES patients (the fourth versus first quartile) was 2.66 (95% CI= 1.51–4.66). Variables including abnormal ejection fraction (<40%), poor physical activity, poor drug adherence, and hypertension were also identified as significant independent predictors of readmission.
Conclusion
The results showed low SES is a significant contributing factor to increased readmission due to CHF. It seems that the outcome of CHF depends on the SES of patients even after adjusting for some of main intermediate factors.- انتشار مقاله: 04-03-1393

Road traffic accidents and their related deaths have become a major concern, particularly in developing countries. Iran has adopted a series of policies and interventions to control the high number of accidents occurring over the past few years. In this study we used a time series model to understand the trend of accidents, and ascertain the viability of applying ARIMA models on data from Taybad city.
Methods
This study is a cross-sectional study. We used data from accidents occurring in Taybad between 2007 and 2011. We obtained the data from the Ministry of Health (MOH) and used the time series method with a time lag of one month. After plotting the trend, non stationary data in mean and variance were removed using Box-Cox transformation and a differencing method respectively. The ACF and PACF plots were used to control the stationary situation.
Results
The traffic accidents in our study had an increasing trend over the five years of study. Based on ACF and PACF plots gained after applying Box-Cox transformation and differencing, data did not fit to a time series model. Therefore, neither ARIMA model nor seasonality were observed.
Conclusion
Traffic accidents in Taybad have an upward trend. In addition, we expected either the AR model, MA model or ARIMA model to have a seasonal trend, yet this was not observed in this analysis. Several reasons may have contributed to this situation, such as uncertainty of the quality of data, weather changes, and behavioural factors that are not taken into account by time series analysis.- انتشار مقاله: 23-01-1392

Policy makers need models to be able to detect groups at high risk of HIV infection. Incomplete records and dirty data are frequently seen in national data sets. Presence of missing data challenges the practice of model development. Several studies suggested that performance of imputation methods is acceptable when missing rate is moderate. One of the issues which was of less concern, to be addressed here, is the role of the pattern of missing data.
Methods
We used information of 2720 prisoners. Results derived from fitting regression model to whole data were served as gold standard. Missing data were then generated so that 10%, 20% and 50% of data were lost. In scenario 1, we generated missing values, at above rates, in one variable which was significant in gold model (age). In scenario 2, a small proportion of each of independent variable was dropped out. Four imputation methods, under different Event Per Variable (EPV) values, were compared in terms of selection of important variables and parameter estimation.
Results
In scenario 2, bias in estimates was low and performances of all methods for handing missing data were similar. All methods at all missing rates were able to detect significance of age. In scenario 1, biases in estimations were increased, in particular at 50% missing rate. Here at EPVs of 10 and 5, imputation methods failed to capture effect of age.
Conclusion
In scenario 2, all imputation methods at all missing rates, were able to detect age as being significant. This was not the case in scenario 1. Our results showed that performance of imputation methods depends on the pattern of missing data.- انتشار مقاله: 11-02-1392

- چکیده انگلیسی: Background: Breast cancer is a top biomedical research priority, and it is a major health problem. Therefore, the present study aimed to determine the prognostic factors of breast cancer survival using cure models. Methods: In this retrospective cohort analytic study, data of 140 breast cancer patients were collected from Ali Ibn Abi Taleb hospital, Rafsanjan, Southeastern Iran. Since in this study, a part of the population had long-term survival, cure models were used and evaluated using DIC index. The data were analyzed using Openbugs Software. Results: In this study, of 140 breast cancer patients, 23 (16.4%) cases died of breast cancer. Based on the findings, the Bayesian nonmixture cure model, with type I Dagum distribution, was the best fitted model. The variables of BMI, number of children, number of natural deliveries, tumor size, metastasis, consumption of canned food, tobacco use, and breastfeeding affected patients’ survival based on type I Dagum distribution. Conclusion: The results of the present study demonstrated that the Bayesian nonmixture cure model, with type I Dagum distribution, can be a good model to determine factors affecting the survival of patients when there is the possibility of a fraction of cure. In this study, it was found that adapting a healthy lifestyle (eg, avoiding canned foods and smoking) can improve the survival of breast cancer patients.

- چکیده انگلیسی: Background: Heavy vehicle drivers spend a great deal of time away from their families. This issue and other difficulties around their job may increase risky behaviors among them. The current study aims to investigate the prevalence of opium drugs, stimulants, cannabis, and alcohol use 12 hours before driving among heavy vehicle drivers. Methods: We selected two sites that were in charge of medical examination of drivers and recruited 363 drivers of heavy vehicles (trucks, trailers, and buses). We asked drivers about total number of drivers they knew and number of drivers who experienced use of different types of drugs. The data were analyzed using Network Scale Up Method (NSUM). Findings: Mean of age and job experience was 43.28 ± 10.04 years and 16.07 ± 9.67 years, respectively. The highest and lowest prevalence of drug use related to opium-based drugs at 12.8% to 14.0% and simulants at 1.97% to 2.84%, respectively. The prevalence of alcohol use 12 hours before driving was 4%. Conclusion: 12-hour before driving prevalence of opium-based drugs among drivers was high. This might put them in higher risk of road accidents. There is a need to design appropriate educational programs for them.

- چکیده انگلیسی: Background: Alcohol use and drug injection are prevalent among homeless youths. The aim of this study was
to identify the associated factors of alcohol consumption and drug injection among homeless youths aged
18-29 years.
Methods: Data on 202 homeless youths (111 males and 91 females) were collected using a standardized
questionnaire and face-to-face interview. Lasso logistic regression was applied to determine the impact of
associated factors on alcohol consumption and drug injection.
Findings: The mean age of the participants was 26.30 ± 3.19 years. Also, the prevalence of alcohol
consumption and drug injection was 33.0% [95% confidence interval (CI): 30-36] and 4.0% (95% CI: 0-8),
respectively; 6 people (3.0%) consumed alcohol and injected drugs at the same time. Correlates of alcohol
consumption and drug injection were male sex [odds ratio (OR)Alc = 5.7], age (ORAlc = 0.96 and ORDI = 0.98),
bachelor or higher education level (ORAlc = 1.34), non-Iranian nationality (ORAlc = 0.05 and ORDI = 0.18),
food score (ORDI = 0.92), smoking (ORAlc = 2.05), substance use (ORAlc = 1.12), opposite sex relationship
(ORAlc = 1.6), homosexual relationship (ORAlc = 3.56 and ORDI = 2.69), and mental disorder (ORAlc = 0.99).
Conclusion: Based on our findings, it seems that the homeless youth are more desired to use alcohol and drug
injection, whereas the prevalence of alcohol consumption and drug injection in homeless youth was higher
than general youth population in Iran. Therefore, some suitable solutions are needed to prevent the
homelessness. Also, the effective variables that were identified in this study for alcohol use and drug injection
can help design and implement beneficial interventions.
