Patient safety is one of the main goals in a health system trying to reduce patient problems during hospitalization. These problems can be due to unintentional errors made by clinical staff the recognition and management of which are very important. The present study aimed to investigate the registered medical errors in Alavi hospital in Ardabil, Iran, during 2017-2018.
Materials and Methods:
In this cross-sectional study, all the wards were studied in Alavi hospital, and the data were collected by the evaluation of reported errors made by clinical staff and completion of the registered medical error forms. The collected data were analyzed through descriptive statistics.
Results:
During the study period, 1,050 registered errors were evaluated. Most errors (n=482; 45.9%) were detected in the women’s surgery ward. According to the number of errors made by nurses in the hospital in comparison to those reported for other clinical groups, the nurses with 464 cases (44%) had the highest frequency of medical errors, and the caring errors (29%) were the most frequently committed type of errors.
Conclusion:
Based on the obtained results of the current study, it was shown that caring errors were the most frequently committed errors, and the highest frequency of medical errors occurred in the women’s surgery ward. Therefore, providing educational training, emphasizing on doing hospital process, reducing the duration of work shifts, and increasing the number of personnel in hospitals, could be effective in decreasing errors and associated consequences.- انتشار مقاله: 13-12-1398

- چکیده انگلیسی: In this article, a new structure is presented for MOS (Metal Oxide Semiconductor)-like junctionless carbon nanotube field effect transistor (MOS-like J-CNTFET), in which dual material gate with different work-functions are used. In the aforementioned structure, the size of the gates near the source and the drain are 14 and 6 nm, respectively, and the work-functions are equal and 0.5 eV less than the work-function of the intrinsic carbon nanotube. The simulation is carried out in the ballistic regime using the non-equilibrium Green's function (NEGF) in the mode space approach. The simulation results show that the proposed structure has a better am-bipolar behavior and less OFF current compared to a conventional junctionless structure with the same dimensions. In the new structure, the hot carrier effect is also reduced due to the reduced electric field near the drain, and with regard to a peak in the electric field curve at the junction of two gates, the gate control on the channel will be increased.

- چکیده انگلیسی: Background: Due to wide clinical differences in the various pathological types of breast cancer and also close
associations between disease prognosis and molecular subtypes, relationships of the latter with traditional risk factors
have been suggested. Hence, the present study aimed to assess any associations. Methods: This bi-center cross-sectional
study was performed on 800 consecutive women with known breast cancer referred to two Comprehensive Cancer Centers
in Tehran between 2006 and 2016. Baseline information related to reproductive risk profiles as well as pathological
tumor diagnosis and molecular subtypes determined using immunohistochemical analysis by immune-staining for
ER, PR, and HER2 molecules were collected by reviewing hospital records. Results: Of 800 samples included for
immunohistochemical analysis, 314 (39.3%) were diagnosed as of Luminal A subtype, 107 (13.4%) as Luminal B
subtype, 153 (19.1%) as HER-2 over-expressing, and 226 (28.3%) as triple negative. Among all reproductive risk
factors initially assessed, young age was associated with HER-2 over-expression, greater tumor size and a history of
abortion with the luminal B subtype, lower age at pregnancy with the luminal A subtype, and lower gravidity and
a shorter duration of breastfeeding with the triple negative subtype. Conclusion: Each molecular subtype of breast
