- چکیده انگلیسی: Research shows that 1% of children experience swallowing difficulties, though the incidence is higher in some, such as those with cerebral palsy and brain injury. One of the causes of pharyngeal phase dysphagia is the absent gag reflex. The aim of this case study was to investigate the effects of sensory stimulation combined with Neuro Muscular Electrical Stimulation (NMES) on dysphagia and gag reflex.
This study was conducted on a 2.5-year-old child with midbrain disorders and absence of oral swallowing and gag reflex. At the beginning of the treatment, sensory-tactile stimulation were provided which induced successful oral swallowing. The treatment was then continued with NMES along with stimulations of the sensory-tactile receptor. Over 26 sessions of NMES, the gag reflex was induced by a 2-second stimulation of the posterior pharyngeal wall, and the time interval between suctions increased to two hours. The present Study showed that NMES activates and strengthens the muscles involved in swallowing.- انتشار مقاله: 24-07-1398

- چکیده انگلیسی: Background:Contrast bath is a thermal agent that is used as alternative heat and cold water on a target limb. Type 2 diabetes mellitus in long term leads to vascular insufficiency. It seems that contrast bath is useful for patients with diabetes mellitus due to the increase in blood flow and vascular pumping. The purpose of this study is to investigate the effect(s) of contrast bath on circulation of contralateral lower limb in women with type 2diabetes mellitus compared to healthy subjects.Methods:Fifteen diabetic females aged between 30-60 years, who were diagnosed with type 2 diabetes, and 30 matched control females participated in this research. The oral temperature, skin temperature of the first web space of the foot, and the pulse of dorsalis pedis artery were measured and recorded. These results were evaluated prior to, immediately and 10 minutes after the contrast bath. The contrast bath protocol included the immersion of the right foot in the warm bath (38-440C) for four minutes and then in the cold bath (10-180C) for one minute. These processes were repeated five times as the total duration of the intervention was 24 minutes. The treatment was started and terminated with warm water. Repeated measures and independent t-test were used for data analysis.Results:The dorsalis pedis pulse was significantly higher at all times of the measurements except between immediately post the contrast bath and after 10 minutes of applying it in the diabetic group (P=0.58). The results of the skin temperature measurement demonstrated incremental changes in the healthy and the diabetic groups. The oral temperature at all times was not significantly different in the normal and the diabetic groups (P > 0.05).Conclusion:The results of this study showed that contrast bath can increase circulation in the contralateral limb. However, its influence on superficial and deep blood flow is uncertain.

- چکیده انگلیسی: Background: Osteoarthritis is one of the most common diseases in synovial joints. Dueto high loading forces during weight-bearing activities, the knee joint is prone to degenerative processes. This study aimed to compare the flexibility of muscles around the knee joint in patients with knee osteoarthritis with those of healthy individuals.Methods: Twenty-three women with bilateral knee osteoarthritis and 23 healthy women matched with the patient group for age, height and weight participated in this study. Flexibility of the quadriceps, hamstring, iliotibial band, adductor and gastrocnemius muscles was evaluated with a goniometric device. Pain intensity was assessed with a visual analogue scale. The data were analyzed with independent t-tests to compare the two groups, and Pearson correlation coefficient was calculated to correlate muscle flexibility and pain intensity.Results: significant lower flexibility of the bilateral quadriceps muscles was found in patients compared to healthy women (P<0.05). No significant differences were detected between groups regarding other muscles. Quadriceps flexibility correlated significantly with pain intensity in patients (P<0.001).Conclusion: In patients with osteoarthritis, quadriceps flexibility may be decreased although other muscles may remain unaffected. Quadriceps stretching is thus a potentially important component of treatment, which may influence pain reduction in these patients.

- چکیده انگلیسی: Background: Lateral epicondylitis, also known as tennis elbow, is the most common overuse syndrome of the elbow. The severity of pain may not be directly caused by tendinopathy of wrist extensors since trigger points of the shoulder muscles have a referral zone in the arm and elbow. Therefore the aim of this study was to evaluate the effect of dry needling of shoulder myofascial trigger points on wrist extensors muscles pain and function.Methods: Fourteen female patients with tennis elbow (aged 20 - 45 years old) were recruited after primary evaluation by an orthopedist. They entered the study if they had pain in the lateral aspect of elbow of the dominant hand for more than 3 months along with the presence of myofascial trigger points in any muscles of supra spinatus, infra spinatus, sub scapularis or scalenes. Pain pressure threshold, maximal grip force and pain intensity of the hand extensors on lateral epicondyle of elbow were measured before and after treatment. Pain intensity was measured on a one to ten scale of visual analogue scale (VAS). A hand dynamometer used to measure the maximal grip force value of the affected hand in 0˚shoulder flexion/abduction, 90˚ elbow extension and mid-poison of forearm in sitting position .A pressure algometer was applied on hand extensor muscles to define their trigger point sensitivity. For the control group, treatment regimens consisted of routine physical therapy of tennis elbow. This regime was accompanied by dry needling of mentioned muscles for the intervention group. Wilcoxon and Mann-Whitney non-parametric tests were used for statistical analysis.Results: Comparison of the results after intervention showed that the patients’ pain significantly decreased in both groups (P <0.001); but the patient’s PPT and grip force significantly increased solely in the intervention group (P <0.05). Mann Whitney test showed significant pain differences in both groups (P = 0.001).The comparison of differences showed that the grip force and PPT statistically improved in the intervention group (P <0.05).Conclusion: Dry needling of affected shoulder muscles along with routine physical therapy has a more significant effect on improvement of pain, sensitivity and grip power of patient’s with tennis elbow syndrome.
