- چکیده انگلیسی: Toxoplasma gondii is widely prevalent in sheep and their products pose a risk to public health. The aim of this study was to identify the seroprevalence and risk factors associated with T. gondii infection in sheep in Veracruz State, Mexico. The study was cross-sectional and it was carried out in thirteen municipalities distributed in three regions of Veracruz State. A total of 414 blood samples were collected from four districts of Veracruz State and analyzed for T. gondii antibodies using enzyme-linked immunosorbent assay. Total seroprevalence was 35.90% (149/414; 95.00% CI = 31.40-40.80). Seroprevalence by the municipality was 10.50% to 85.70% and for the district was 28.80% to 47.80%, respectively. Age, breed and productive status were identified as risk factors associated with T. gondii infection significantly. The infection by T. gondii is widely present in the districts of the Veracruz State with a high seroprevalence and risk factors associated with infection.

Background and Aims: It has been demonstrated that homozygote and heterozygote mutant allele carriers for thiopurine S-methyltransferase (TPMT) are at high risk of developing myelosuppression after receiving standard doses of 6-mercaptopurine (6-MP). The aim of this study was to determine the frequency of TPMT deficient alleles in children with acute lymphoblastic leukemia (ALL) in Jordan and to compare it with other ethnic groups. Methods: We included 52 ALL childhood cases from King Hussein Cancer Research Center in Jordan. Genotyping of the rs1800460, rs1800462, and rs1142345 SNPs was performed by polymerase chain reaction (PCR) followed by sequencing. Comparisons were made with historical data for controls and for both volunteers and cases from other middle-eastern countries. Results: Mutant TPMT alleles were present in 3.8% (2/52) of patients. Allelic frequencies were 1.0% for both TPMT*B and TPMT*C. None of the patients were heterozygous or homozygous for TPMT*3A or TPMT *2. We did not find statistically significant differences in the distribution of mutant alleles between Jordan and other middle-eastern countries for both healthy volunteers or ALL patients. Conclusions: The overall frequency of TPMT mutant alleles was low and did not exhibit differences compared to other middle-eastern countries, including Jordanian studies assessing TPMT mutant alleles in healthy volunteers. Objective: To determine the impact of the trial on surgeon practice patterns at our institution. Methodology: A comparison of patients undergoing surgery for early breast cancer before and after the implementation of the new guidelines was done. We adopted the new guidelines in April 2015. Patients meeting Z0011 inclusion criteria were identified. For group A (Pre Z0011) patients operated between Jan to Dec 2013 were studied. And for Group B (Post Z0011) patients operated between July 2014 to Jun 2015 were included. Clinicopathologic data were compared between the two groups. Results: There were 318 patients with clinical T1-2 tumors planned for breast conservation. 68% patients had T1 tumor and 32% had T2. 92% of the patients had IDCa. There were 150 patients in the pre-Z0011 group and 168 post-Z0011. 68% of the patients in Group A were ER+ve while 70% in group B. 38 (25.7 %) patients were sentinel lymph node (SLN) positive in the pre-Z0011 group versus 34 (21 %) post-Z0011 (p = 0.392). Before Z0011 100 % (38/38) of SLN-positive patients underwent axillary node dissection (ALND) versus 17 % (6/34) after Z0011 (p < 0.01). Median no of SLNs identified in group A were 1.3 and group B were 1.4. There was a decrease in median operative times of the two groups (80 vs. 60 min, p < 0.01). There was a significant decrease in the overall hospital stay of sentinel lymph node positive patients in between the two groups (2.1 days vs 1.3 days p value < 0.01). Conclusions: Implemention of Z0011 guidelines has resulted in significant short term advantages in terms of reduced axiilary dissections, shorter operative times and shoter hospital stays.- انتشار مقاله: 30-10-1395

Introduction: Superwarfarins are highly lethal rodenticides which are much more potent than the warfarin owing to various structural changes in the parent compound that confers high half-life and increase affinity to vitamin K epoxide.
Case Presentation: We report a case of a 19 years old female who presented bleeding prolonged PT and APTT. Factor assay showed significantly lower levels vitamin K dependent coagulation factors. She was given conventional doses of FFP and vitamin K for two consecutive days, although she responded clinically but her coagulation profile failed to show any improvement. Hence superwarfarin poisoning was considered. Based on this assumption patient was cross questioned repeatedly, and subsequently she admitted that she had ingested rodenticide with the suicidal intent. Hence she was given higher doses of vitamin K and FFP for several days after which her PT and APTT shortened significantly.
Discussion: Superwarfarin toxicity clinically presents with bleeding and deranged coagulation profile. Differentiating it from other causes is challenging but very crucial, as it does not responds to usual doses of vitamin K and plasma.
Conclusion: This case highlights the importance of maintaining high index of suspicion of superwarfarin toxicity in coagulopathy of unknown etiology that fails to respond to conventional doses of treatment.
