- چکیده انگلیسی: Background: Consumption of non-commercial alcoholic drinks is the main cause of methanol poisoning in the world. Non-standardized production methods or deliberate spiking of methanol in non-commercial and homemade alcoholic beverages have been reported, globally. The analysis of toxic alcohol contents in illegally produced alcohol beverages is necessary for prevention and early diagnosis of methanol poisoning especially during alcohol mass poisoning episodes. In this study, we analyzed methanol, ethanol and higher alcohols content in seized illegal alcoholic beverages during methanol mass poisoning outbreak in Bojnourd City (northeast of Iran) in 2018.
Method: During the methanol mass poisoning outbreak in Bojnourd city (northeast of Iran), happened in one –month period from September 22 to October 22, 2018, samples of all illegal alcoholic beverages either found consumed by poisoned patients who admitted to the hospitals or seized by the law enforcement were analyzed. Qualitative and quantitative analysis of ethanol, methanol, 1-propanol, 2-propanol and acetone were performed using GC-FID method.
Results: In the samples collected from a total of 116 seized illegally produced alcohol beverages; methanol was detected in 39.6% with mean concentration of 77.6 %v/v and the range from 12 to 97% v/v. Ethanol was detected in 77 (66.4%) samples with mean concentration of 32.3%v/v. Only 8 (6.9%) samples had a mixture of ethanol and methanol. 1-propanol, 2-propanol and acetone were not detected in any samples.
Conclusion: Methanol is the main toxic chemical component in illegal alcoholic beverages with toxic level and could be considered as a cause of mass poisoning during the alcohol mass poisoning in Bojnourd City. - چکیده انگلیسی: Abstract Background: Synthetic cannabinoids (SCs) are highly abused of New Psychoactive Substances (NPS). SCs has known under street names such as “Spice”, “herbal incense” and “K2”, act as endocannabinoids (CB) receptor full agonists and have unpredictable toxicity and abuse potential. This narrative review was conducted to update the present evidence about the clinical and forensic toxicological aspects of SCs. Methods: PubMed, Scopus and Google Scholar databases from 2015 to 2020 (up to 1st May) were searched using the terms “synthetic cannabinoids”, “synthetic cannabimimetics”, “ K2”, “Spice”, “clinical toxicology”, “forensic toxicology”, “poisoning”, “toxicity”, “abuse” , “addiction “analysis” and “determination” to identify the relevant articles. In addition, a manual search of reference lists of the retrieved articles was conducted. Results: ADB-FUBINACA , XRL-11, 5F-ADB, 5F-PB-22, MDMB-CHMICA and MMB-2201 are the commonly reported SCs analogues among acute toxicities and fatalities cases. Adverse reactions and toxic effects of SCs includes psychoneurological, cardiovascular, renal and gastrointestinal involvements. Deaths related to SCs have been reported due to stroke and cardiac dysrhythmia. Analysis of SCs in biological samples in the clinical and postmortem setting needs sophisticated analytical instruments. Liquid gas chromatography tandem mass spectrometry (LC-MS/MS) has a crucial role for detection of SCs and their metabolites in biological samples. Conclusion: Unlike natural cannabinoids, the SCs abuse/poisoning has serious and life-threatening effects in abuser. Also, analysis of SCs is not included in the routine forensic urine drug testing. Therefore, suitable measures of informing the public and health care professionals for prevention of SCs abuse are recommended.

- چکیده انگلیسی: Background: In Iran, methadone has been used for methadone maintenance treatment (MMT) as well as analgesic treatment in pain clinics. Recently, there are some reports regarding accidental and intentional methadone poisonings and deaths. The aim of this study was to evaluate the trend of methadone poisonings and deaths during a 10-year period in Tehran, Iran.
Methods: This was a retrospective cross-sectional study over 2000 to 2010. Patients with a documented methadone poisoning who were admitted in Loghman Hakim Hospital Poison Center in Tehran, Iran were identified and included in the study. The data including patients’ age, gender, ingested dose, co-ingestants, intention of ingestion and outcome were extracted from the patients’ medical records.
Results: During the study period, 1426 cases of methadone poisoning were recorded, of which, 1041 cases (73%) were men. Thirty-six cases (2.5%) died. Mean age of the patients was 29.9 ± 17 years. In 476 cases, the intention of poisoning could not be determined, and in the remaining, the intention was misuse (n = 273, 28.7%), suicide (n = 254, 26.7%), accidental (n = 245, 25.8%) and abuse (n = 178, 18.8%). Mean of the ingested dose of methadone was 120.6 ± 306.8 mg. The incidence of acute methadone poisoning per one million population of Tehran was 0.43 in 2000 that rose to 37.62 in 2010.
Conclusion: The results indicate that methadone poisoning and deaths have increased in Tehran. MMT clinics should be strictly run according to the national guideline to prevent methadone poisoning. With regard to high frequency of poly-drug use in methadone poisoning, it seems important to warn health care providers against prescription of other drugs with methadone.- انتشار مقاله: 15-03-1393
