- چکیده انگلیسی: Introduction: Esophageal atresia (EA) with or without tracheoesophageal fistula (TEF) is defined as a congenital malformation characterized with the interruption or obstruction of esophagus. Affected neonates may present with cyanosis during breast feeding, sialorrhea, coughing and difficulty in respiration. The defect should be corrected by surgery; otherwise, the condition can be life-threatening. Little is known about the long-term nutritional problems of children with EA. Our aim was to assess growth status and nutritional difficulties in children with EA. Methods: This was a retrospective study on 32 children with EA between 2007 and 2016. Nutritional status and feeding problems were assessed by a questionnaire. Results: The most common EA-associated complications were esophageal stricture (84.4%), dysphagia (46.9%), gastroesophageal reflux disease (37.5%), and respiratory infections (25%). Majority of our patients showed normal growth parameters, and 96.9% of them had experienced at least one complication or feeding problem including needing to drink water to swallow food (25%), coughing during feeding (34.4%), vomiting (12.5%), and abdominal pain (34.4%). There were no significant associations between feeding problems and growth parameters. Conclusions: Feeding problems are relatively common in children with EA. There is a need to provide nutritional consults to these patients to prevent or mitigate these problems.

- چکیده انگلیسی: Background: There is limited knowledge on the potential applicability of fecal calprotectin (f-CP) as an inflammatory screening parameter in infantile colic (IC). This study aimed to evaluate f-CP in neonates with IC as a useful diagnostic indicator regarding this condition.
Methods: The present study was conducted on 100 cases, including 50 newborns with IC and 50 non-colicky neonates. The diagnosis of IC was fulfilled by the Wessel Criteria. The level of f-CP was determined by a specific enzyme-linked immunosorbent assay kit (Calprotectin ELISA, EuroImmun, Germany). The statistical analysis was performed in SPSS software (version 19).
Results: Out of 100 neonates, 57 cases were male and 43 subjects were female. The age spectrum ranged from 19-90 days (40.4±15.9). Colicky newborns were slightly younger (P=0.06) with higher birth weight compared to the infants without colic (P<0.0001). The level of f-CP was significantly higher in colicky neonates (113.7±98.2 µg/g) than non-colicky cases (71.4±45.5 µg/g) (P=0.007). Overall, 37%, 30%, 26%, and 7% of the newborns showed f-CP levels<50, 50-100, 100-200, and >200 µg/g, respectively. There was a significant difference regarding the distribution of these f-CP categories between neonates with IC and the cases without IC (P=0.02). There were no significant correlations between the f-CP and newborn age, pregnancy age, present or birth weights, and number of pregnancies. Receiver operating characteristic analysis rendered an area under the curve of 0.642 (95% CI: 0.534-0.748) (P=0.01). At the cut-off value of 74 µg/g, f-CP showed sensitivity of 60% and specificity of 59% for the detection of IC.
Conclusion: The results of this study revealed that the f-CP might be useful in the diagnosis of the IC.- انتشار مقاله: 10-09-1397

Celiac disease (CD) is an autoimmune mediated gluten sensitive enteropathy and characterized by a wide range of clinical manifestations. The study aimed to compare Doppler tissue echocardiography findings between children with celiac disease and healthy children.
Materials and Methods
This case-control study was performed on 186 celiac and healthy children aged 6- 19 years. Celiac diagnosed by clinical findings and laboratory's tests (tTGIgA) confirmed by intestinal biopsy. Controls consisted of voluntary individuals free of any diseases. Echocardiography performed on participants. Data analyzed utilizing SPSS software version 20.0.
Results
Left Peak A, left S', right ICT', right IRT', right S', EF, FS, left ET', left IRT', right E'/A' left S' were higher in case when Right ET was lower. Patients with left S' < 8 were lower in left A', higher in right ICT', lower level in right S', higher in left E/E'. - چکیده انگلیسی: Background: Dilated cardiomyopathy (DCM) is revealed with the left ventricular dilatation and systolic dysfunction. This study was performed to determine the level of Calcitonin Gene Related Peptide (CGRP) and Brain Natriuretic Peptide (BNP) in children with dilated cardiomyopathy and controls and comparison of these two biomarkers in patients.
Materials and Methods: This case-control study was performed from April 2014 to March 2015 on patients with DCM. The levels of BNP and CGRP were measured by ELISA and final amounts of biomarkers were compared with echocardiographic finding.37 DCM patients compared with 30 healthy children selected randomly from those who referred to the hospital for routine checkup.
Results: In this study the mean age was 10.567± 5.50 and 12.135 ± 4.626 years for controls and cases, respectively (P=0.321). The majority of echocardiography indices in the left and right heart had different means in case and controls (P<0.05). Means of BNP were 213.814 ± 309.601 and 2.76± 1.013 for case and control groups, respectively (P< 0.001). Means of CGRP were 2.278±1.586 and 1.488±0.501 for case and control groups, respectively (P = 0.001). In the dilated cardiomyopathy children however, no significant relationship was observed between CGRP level and Ross classification, but observed a significant association between Ross classification and BNP (Chi square = 15.85 and P=0.001).
Conclusion:The present research was performed on DCM patients and showed that most echocardiographic parameters, mean of CGRP and mean of BNP increased in patients compared to healthy children. The severity of illness based on the Ross classification showed significant and positive correlation with BNP level but not with CGRP. Probably could be concluded that, BNP would be a better biomarker in DCM patients.- انتشار مقاله: 10-04-1396

This study aimed to assess Brain Natriuretic Peptide in early diagnose of cardiac function in children with Congenital Heart Diseases.
Methodology
This study was performed on children with CHD and healthy. Severity of heart failure modified Ross classification system was used for grouping patients in four classes. For all participants, height, weight and head circumference were measured. From participants 3 cc blood samples was taken and after separation, the plasma placed at temperature of -80 c. After collecting all samples they were in testing using an ELISA BNP kit. For 50 children in control group statistical methods used to generate RV and PA pressure randomly. The data were analyzed using SPSS for Windows, Version, 15.0 (Chicago,SPSS Inc. USA) with 0.05 for the level of significant.
Results
BNP, RV pressure, PA pressure, HB and O2 saturation had different means in cyanotic, Acyanotic and and control significantly. BNP, RV, PA , HB and O2Sat had different means in case and control significantly. , RV, HB and O2 saturation had different means significantly acordance with the sevirity of disease.BNP had correlation with PA and PA had correlation with Qp/Qs ratio ,Rp/Rs ratio , RV and O2Sat significantly .
Conclusion
At the end from the results of the study concluded that BNP levels in children with chd increased.

- چکیده انگلیسی: There are few reports on the therapeutic role of colonoscopy in correcting intussusception of childhood. In the present report, 15 children (14 boys and 1 girl) presented with acute intussusception and successfully treated by colonoscopy were presented. The patients were referred to the gastroenterology clinic of Ami-Al-Momenin Hospital, Zabol, Iran from 2015 to 2018. The mean age of the patients was 36.6±28.7 months. After stablishing the diagnosis, they underwent colonoscopic reduction of intussusception by an experienced gastroenterologist. Following 24 hours of the procedure, follow up sonography was performed. Overall, 6 recurrences were identified in 4 (26.7%) patients which were managed by a second colonoscopy. A second recurrence was detected in two patients who finally underwent surgical reduction. Regarding the role of colonoscopy in providing a visual and real-time setting, this can be used as either a primary option or an alternative to radiologic and surgical procedures of intussusception reduction in children.
