- چکیده انگلیسی: Introduction: Inefficient management of hospitals leads to the substantial waste of resources. Therefore, hospital efficiency has recently been a major concern among researchers. The present study aimed to evaluate the association of the management competency and technical efficiency in the general hospitals of Tehran, Iran. Materials and Methods: This descriptive-analytical, cross-sectional study was conducted in 28 general hospitals in Tehran, Iran in 2015. In the first step, a survey was performed among 224 managers at different levels in the hospitals using a researcher-made management competency self-assessment questionnaire. In the second step, technical efficiency in the selected hospitals was measured by the Data Envelopment Analysis (DEA). Data analysis was performed using SPSS, Enterprise Management Software (EMS), and Deap1, 2. Results:The average of competency was relatively high among the hospitals managers. However, more than half of the hospital performances were observed to be inefficient. Moreover, the results indicated that the competency of hospital managers was significantly correlated with their field of education, participation in training courses, and managerial experience. Also, a significant, direct association was observed between the competency of managers and technical efficiency in the studied hospitals. Conclusion: According to the results, although the level of technical efficiency was favorable in the studied hospitals, the hospital managers with high competency should further develop and improve their performance and efficiency in these hospitals.

- چکیده انگلیسی: Introduction: Quality improvement and patient safety are the most important aspects of health care delivery systems. Improving quality and safety in health care organizations is assured through accreditation.
To evaluate patient safety in Farabi Eye Hospital by Joint Commission International (JCI) accreditation standards for quality improvement.
Materials and Methods:This descriptive-analytical study was carried out in 2014. The data collection instrument was the translated version of JCI checklist, the validity of which was confirmed by a panel of Iranian experts. The data were obtained through interviews with all heads of departments (n=80) and observation of documentation in each department by appraisers in Farabi Hospital. To analyze the data, Spearman's rank correlation coefficient and Mann-Whitney U tests were run using SPSS Version 16.
Results: The highest rate of compliance with JCI standards was 91.1%, pertaining to infection control and adopting an approach to reduce contamination. The lowest rate was associated with management monitoring of patients’ demographic information. In general, Farabi Eye Hospital observed the standards to a high level.
Conclusion: Our findings exhibited that Farabi Eye Hospital is relatively efficient as to patient safety as a quality improvement factor. However, there were some shortcomings regarding some standards, which indicate deficient compliance with the JCI standards in this hospital. Absence of comprehensive training programs and defective policies and documentation are the key factors for patient safety quality improvement.- انتشار مقاله: 26-11-1394

- چکیده:
- چکیده انگلیسی: Background and Objectives: Stress among students of the health sciences can lead to reduced performance of future healthcare human resources. To address this threat there is a need to develop a robust understanding of the nature and intensity of stress in these professionals. To help approaching this goal, the present study assessed stress-induced life change in students of Tehran University of Medical Sciences by quantifying their stressful life events. Methods: A cross-sectional descriptive-analytical study was carried out. Using a cluster sampling method, 248 students were randomly selected from the students of medicine, nursing, dental medicine, pharmacy, allied-medicine, health, midwifery, medical management and information, and rehabilitation disciplines. Based on the concept of Life Change Units (LCU), a questionnaire was developed to quantify the stressful events in student life. The questionnaire contained 54 weighed items about stressful life events related to four groups of interpersonal (10 items), personal (16 items), academic (14 items), and environmental (14 items) stress sources. Validity of the questionnaire was determined by expert opinion. The questionnaire reliability was ensured by Cronbach’s alpha of 0.88. The mean LCU loading of demographic groups was compared using t test and ANOVA. Findings: The average LCU loading in student was 71. Twenty seven percent of the students reported an average LCU loading < 50, 46%, LCU loading between 150-300, and 27%, LCU loading > 300. The highest reported life changes were related to personal factors (86), followed by interpersonal (79), environmental (63), and academic (55) factors. Male students showed significantly higher life change as compared with their female counterparts (P < 0.01). Students of medical and graduate courses jointly expressed significantly higher life change as compared with the under graduate students (P < 0.01). Students of nursing showed significantly higher life change in comparison to other students except medical students (P < 0.01). Upper-year students reported higher life change as compared with freshman students (P < 0.01). LCU loading was found significantly higher in students with sleeplessness (P < 0.05) and muscle spasm (P < 0.05), whereas no significant effect was observed for other clinical symptoms. Conclusions: Our study indicated that a considerable percentage of students are exposed to a high risk of health problems. This observation points out the urgent need for implementing effective stress management strategies to assist students in coping with stress. Such a strategy should primarily focus on enhancing students’ personal life management and communication. Male and upper-year students need to receive particular attention. Stress in students of medical and nursing disciplines needs to be specifically characterized and addressed.

- چکیده انگلیسی: Background and Objectives: Today, management is one of the most important factors in hospital effectiveness and performance. The objective of this study was to assess the management skills of administrators in hospitals of Tehran University of Medical Sciences and to identify how these skills are related to effective management. Methods: A cross-sectional descriptive study was carried out over the period of February to April 2011 by enrolling 15 administrators from hospitals of Tehran University of Medical Sciences (TUMS). The tool for data collection was a self-constructed questionnaire of managerial skills containing the measures of leadership ability, effective use of organizational teams, employee motivation, change management, and effective management. The validity of the questionnaire was confirmed by expert opinion method. The reliability of the scale was also confirmed by obtaining a Chronbach’s alpha of 0.81 for all items. The collected data was analyzed using correlation analysis. Findings: Our results identified relatively high self-assessment score on leadership ability and effective management, but the scores for effective use of organizational teams, employee motivation and change management were low. In addition, among considered dimensions of management skills, only leadership ability showed significant correlation with effective management. Conclusions: A lack of relationship between effective management and most assessed managerial skills depicts an unsatisfactory picture of the overall management skills in hospitals’ administrators, which in turn suggests the need for relevant training programs.

- چکیده انگلیسی: Introduction: Lung cancer is the most common cause of cancer-related death among males and females. The
diagnosis of lung cancer is of great importance for clinical considerations and follow-up treatment. This study aimed
to examine the expression of CEA, LUNX, and CK19 biomarkers in the cancerous and healthy tissues of patients
suffering from NSCLC. Methods: In this study, 30 patients with NSCLCs referring to Masih Daneshvari Hospital
in Tehran were voluntarily selected prior to taking any treatment. A tissue sample from the center and a sample of
healthy tissues close to the cancerous masses were prepared by a specialist in the bronchoscopy sector and tested using
real-time RT-PCR. Results: Positive CEA mRNA was observed in cancerous tissues in the center of tumors of 25 out
of 30 cases. In the healthy tissue group, the same was found in 10 out of 30 cases (P<0.001). The markers CK19 and
LUNX mRNAs showed to be positive in cancerous samples in the center of tumors of 15 and 22 out of 30 cases, and in
the healthy tissue group, the expression was observed in 5 and 4 out of 30 cases, respectively(P<0.001). Conclusion:
This study confirms that the aformentioed markers are the ones with a relatively appropriate sensitivity and specificity
for the diagnosis of lung cancer.- انتشار مقاله: 20-07-1397
