The present study was applied in terms of purpose, qualitative in terms of method and content analysis in terms of research method. Participants in this study, based on the rule and principle of theoretical saturation, included 8 experts available in the field of talent management in Tehran, who were selected by purposive sampling. The data collection tool is a semi-structured interview. Data were analyzed using MAXQDA 2018 software. Data validation was performed by participants using the verification method. Based on the research findings, the dimensions and components of talent management in schools include; Talent-supporting laws and regulations, having a talent management perspective, talent management financial resources, creating an atmosphere of talent support, establishing soft infrastructure, establishing hard infrastructure, scientific and academic system, quality of teaching and learning and talent management.
Today, the school and its activities are constantly being judged by public opinion. The school is the only organization that has a direct relationship with the external environment and its customers are all sections of society. "Schools and their work are always subject to public judgment," it said. This is especially true in countries where people care about education and its consequences. Therefore, schools and their administrators must be aware of the sensitivity of school-community relations to public opinion and public reaction. ”The first priority of education today is to have capable and talented principals to run schools effectively. To this end, identifying talents can facilitate effectiveness. Therefore, based on such initial intuitions and concerns, the researcher tried to study talent management in the statistical community of teachers and principals of gifted schools in Tehran. In this regard, conceptualization was discussed.
A total of 9 dimensions in this study as effective dimensions on talent management in gifted schools in Tehran include; Rules supporting regulations for talent, having a vision for talent management, financial resources for talent management, creating an atmosphere of talent support, establishing soft infrastructure, establishing hard infrastructure, scientific and academic system, quality of teaching and learning and talent management were identified.

Background: The decrease in physical activities following increased usage of computer and digital games has led to serious health consequences in children. This study investigates the prevalence of obesity, cellphone and computer usage and physical activity levels and their relationship with spirometry indicators in Iranian children.
Materials and Methods: This is a cross-sectional study during 2013 to 2014 on high-school students in Isfahan, Iran. Sample size determined 1,690 students and sampling performed from 10 girls and 10 boy's high schools, based on multi-staged cluster randomized scheme. A research-made questionnaire was developed to complete by students interview and also contact with teachers and parents and measuring height and weight by researchers. If the asthma was probable (based symptoms and examined by physician) the spirometry was performed.
Results: Overall 1,622 high-school students with the average age of 12.9 (+ 1.1) years were recruited. Prevalence of obesity and overweight were significantly higher in boys (P<0.05). The prevalence of obesity was 9.5% in boys and 5.3% in girls, and sever underweight was 4.2% and 2.5%, respectively. Cell phone, computer, and digital game usage were significantly higher for boys (P<0.05); while watching television (TV) was not (P=0.400). Of total, 423 students (26.1%) witch asthma was probable and the average of FVC and FEV1 was significantly higher in boys than girls (P=0.001).
Conclusion:The overall prevalence of obesity in 12 to 14-year-old students was more than 7% and its prevalence was influenced by physical activity. Spirometry indicators were correlated with physical activity and lifestyle.

- چکیده انگلیسی: Background: Common variable immunodeficiency (CVID) is a primary immune disorder associated with hypogammaglobulinemia, recurrent infections and autoimmune diseases. CVID patients are frequently in contact with infectious pathogens leading to the activation of innate immunity through Toll-like receptors (TLR) affecting adaptive immunity. The aim of the present study was to test the immunomedulatory effect of small molecule G2013, a novel designed non-steroidal anti-inflammatory agent in CVID.
Materials and Methods: After blood sampling from 16 CVID patients and 16 age- and sex-matched healthy controls, peripheral blood mononuclear cells (PBMCs) were isolated and treated with/without lipopolysaccharide (LPS), lipopolyteichoic acid (LTA), and G2013. Assessing the immunomodulatory effect of G2013, flowcytometry was done for quantify the protein expression of TLR2 and TLR4. Gene expressions of signaling molecules involved in the TLR2 and TLR4 pathways were assessed by real-time PCR. ELISA performed assessing the production of IL-1b and IL-6.
Results: G2013 significantly decreased the intensity of TLR2 expression in CVID PBMCs (p=0.001) also G2013 decreased significantly the NF-kB gene expression in PBMCs of CVID patients (p=0.006).
Conclusion: These results indicated that G2013 had immunomodulatory effect at least in part via TLR2 and NF-kB expression. G2013 by decreasing MFI of TLR2 expression and NFkB gene expression provide the possibility of designing new drugs for preventing or controlling autoimmunity in CVID patients.- انتشار مقاله: 23-02-1396

- چکیده انگلیسی: Background: Although sleep disorders are common problems among families and they affect the learning, memory processes and academic performance of children, there is no evaluation of these disorders in Iran. The aim of this study was to assess the prevalence of sleep disorders and its association with academic performance of school age children.
Materials and Methods: A cross-sectional study was conducted on 1,100 middle school students of Isfahan city of Iran during 2012-2013. Multi-stage random cluster sampling method was performed and five girl’s schools and five boy’s schools were selected. The data gathered with a validated questionnaire to evaluate the academic performance and sleep disorders.
Results: The mean duration of nocturnal sleep was 8.38±1.17 which was significantly higher in the group with excellent academic performance (8.86±1.18 hours), than the other two groups (8.14±1.17 hours for average academic performance and 7.90±1.15 hours for poor academic performance). Academic performance was significantly associated with age, gender, parental occupation, nocturnal sleep time, sleep latency and sleep disorders (P<0.05).
Conclusion: This study revealed that sleep disorders negatively affect the academic performance and highlighted the importance of proper sleep among children and students.- انتشار مقاله: 26-10-1395

Bronchiectasis is defined by permanent and abnormal widening of the bronchi. Although this process occurs in the context of chronic airway infection and inflammation, since there is no accurate estimation of the etiology of the disease. This study aimed to determine the most important cause of bronchiectasis in Tehran, Iran.
Materials and Methods
In this retrospective cohort study we used the information of 91 patients admitted to two subspecialty lung hospitals in Tehran-Iran, where a wide range of bronchiectasis patients from around the country referred during 2013to 2014 period. Patients referring with the manifestation of chronic productive cough who had not responded to conventional treatment with the evidences of bronchiectasis on high resolution computed tomography were included. Data were analyzed using SPSS-16.0.
Results
The etiology of bronchiectasis was diagnosed in 73 of 91 patients (80.2%), the most important of which included cystic fibrosis, post infectious, and primary ciliary dyskinesia (PCD). The most common causes of bronchiectasis in the children group (Age ≤ 18 years), were cystic fibrosis (57.1%), allergic bronchopulmonary aspergillusis (14.3%) and PCD (9.5%), respectively. In the adults group (Age >18 years), the most common causes were post infectious (22.6%), PCD (15.7%) and cystic fibrosis (14.3%), respectively.
Conclusion
Main causes of bronchiectasis in this study were not significantly different from other studies. Special attention should be paid to the probable causes of bronchiectasis in order to effectively execute on-time diagnosis, proper treatment and management of complications.- انتشار مقاله: 23-08-1395

Ataxia telangiectasia (A-T) is a common genetically inherited cause of early childhood-onset ataxia. The infrequency of this disease, vast phenotype variation, disorders with features similar to those of A-T, and lack of definite laboratory test, make diagnosis difficult. In addition, there is no rapid reliable laboratory method for identifying A-T heterozygotes, who susceptible to ionizing radiation (IR), atherosclerosis, diabetes, and cancers. We used SMC1pSer966 (pSMC1) in-cell colorimetric ELISA to diagnosis and screen in A-T families.
Materials and Methods: With informed consent, 2cc peripheral blood was collected from the 15 A-T patients, their parents, and 24 healthy controls with no family history of malignancy, diabetes, and atherosclerosis. Extracted peripheral blood mononuclear cells (PBMCs) were cultured in poly-L-Lysine treated 96-well plate with density of 70,000 cells per well. SMC1 phosphorylation was evaluated with cell-based ELISA kit 1 hour after 5 Gy IR and the pSMC1data normalized with Glyceraldehyde-3-phosphate dehydrogenase (GAPDH).
Results: SMC1 phosphorylation was significantly low in A-T`s PBMC (mean + standard deviation [SD]: 0.075 + 0.034) in comparison to carriers (mean + SD: 0.190 + 0.060) and healthy controls (mean + SD: 0.312 +0.081), but unluckily could only discriminate A-T patients (Area Under the Curve -receiver operating characteristic [AUC-ROC]: 1.00, 1.00-1.00). This method in spite of rapidness and simplicity showed poor imprecision (22.49% coefficient of variation [CV] for intraday imprecision).
Conclusion: It seems pSMC1 assessment by in-cell ELISA can be used for detection of A-T patients, but it may not sensitive enough for identification of carriers. This ELISA test is very simple, rapid, and requires less than 2cc blood. Thus it may be proposed for the early differential diagnosis of A-T as an alternative method.- انتشار مقاله: 24-07-1395

- چکیده انگلیسی: Background: Common variable immunodeficiency (CVID) is one of the most prevalent symptomatic primary immunodeficiencies (PIDs), which manifest a wide clinical variability such as gastrointestinal (GI) disorder.
Patients and Methods: A total of 240 patients with CVID were enrolled in this study. The patients were evaluated for demographic data, clinical manifestations, and immunologic profile.
Results: In demographic data, the frequency of consanguinity and mortality rate were higher in patients with GI manifestation than those without GI manifestation. History of GI manifestations was evident in 147 patients (61.3%). The most common GI manifestation in patients with CVID was chronic diarrhea (29.6%). The prevalence of GI disease was 59 of 102 (57.8%) in female patients and 88 of 138 (63.8%) in male patients. The frequency of recurrent infection was higher in patients with GI manifestation than in those without GI manifestation. Also, CVID patients with GI manifestations had lower WBC and CD4+ T cells than patients without GI manifestations.
Conclusion: CVID patients are at increased risk of infectious conditions in the GI tract; hence GI manifestations are one of the most important presentations in CVID patients which can appear as a first manifestation or appear during the course of disease.- انتشار مقاله: 27-07-1398

- چکیده انگلیسی: Background: The most common symptoms of agammaglobulinemia are recurrent respiratory and gastrointestinal problems. The purpose of this study was to define the prevalence and type of gastrointestinal (GI) manifestations in patients with agammaglobulinemia.
Methods: A total of 147 patients with agammaglobulinemia were entered into this study. For each patient clinical, immunological and laboratory data were documented.
Results: The GI manifestations as the first presentation of the immunodeficiency diseases were reported in 14.5% of patients mostly due to chronic and recurrent diarrhea. History of GI manifestations was evident in 67 patients (45.6%). Recurrent, chronic or bloody diarrhea (36.1%), vomiting (10.2%), and gastroenteritis (6.1%) were the most common GI manifestations in patients with agammaglobulinemia, respectively.
Conclusion: The bowel is exposed to multiple antigens including gut microbiota and pathogens from the environment. Mucosal antibodies play a major role in protection and homeostasis; hence, decreasing level of IgA can provide the opportunity for infectious diarrhea and more pathogen to attack.- انتشار مقاله: 28-04-1398

- چکیده انگلیسی: Primary immunodeficiency (PID) is a group of more than 400 distinct genetic disorders affecting both children and adults. As signs and symptoms of PID are usually heterogeneous and unspecific, based on the available human resources and laboratory facilities, diagnosis and follow-up of these patients can be challenging. In line with this notion, management and treatment of PID are practiced divergently in different PID centers. In order to reach a distinct national protocol and given the little evidence on appropriate or universal guidelines to improve the current status of management of the disease, the Iranian PID network designed a consensus suitable for regional resources. This review summarizes this PID guideline based on the importance of different clinical complications and the level of medical authority visiting those at the first line. Further, for each complication, appropriate interventions are mentioned to improve the approach.
