- چکیده انگلیسی: Background and Purpose: Invasive candidiasis (IC) is a significant cause of morbidity and mortality in patients with hematologic disorders and bone marrow transplant recipients. Rapid, specific and sensitive test for the timely accuracy in immunocompromised patients to reduce mortality rates and prevent IC progress is necessary. We established a real-time PCR assay on blood for the diagnosis and differentiation of the causative Candida species. Materials and Methods: Whole blood samples were collected twice, from 72 patients for Real Time PCR and blood culture assays. The primers and hybridization probes were designed to potentiate the specific sequence of 18S rRNA genes using Light Cycler system and Fluorescence Resonance Energy Transfer (FERT). The patients with hematologic malignancies and bone marrow transplant recipients were evaluated for IC based on the revised European Organization for Research and Treatment of Cancer/ Mycoses Study Group (EORTC/MSG) criteria.
Results: From 2009 to 2011, 72 patients with hematologic malignancies and bone marrow transplant recipients were evaluated for IC. The female to male ratio was 27:45; the mean age was 32.1 years. The most common malignancy in this patient was acute myeloid leukemia (AML) (27.8%) and acute lymphoblastic leukemia (ALL) (26.4%). Out of 72 patients, 11 patients (15.3%) had positive real time PCR /probe results. Based on the melting temperature (Tm) analysis, 5 (45.4%) C. krusei, 3 (27.2%) C. tropicalis, 2 (18.1%) C. parapsilosis and 1 C. albicans (9%) were identified. According to the revised EORTC / MSG, 1 patient (9%) and 10 patients (91%) were defined as proven and possible groups of IC, respectively. The mortality rate in proven and possible IC patient was found 54.5%. Conclusion: The established Real-time PCR/FRET probe assay is an appropriate diagnostic tool for the detection of Candida species DNA and the management of patients suffering from hematologic malignancies and bone marrow recipient are at risk for IC.- انتشار مقاله: 18-04-1398

- چکیده انگلیسی: Objective: Colorectal cancer is one of the most common causes of death in the world. Despite of remarkable
advances in medical sciences, cancer is an important disease and the second cause of death after cardiovascular diseases.
The present study was aimed at determining the survival rate of colorectal cancer in Iran. Methods: The present study
is a systematic review of national and international electronic databases. Studies that had the inclusion criteria were
included in the study, electronically published articles over December 2007 and March 2015 were retrieved. The collected
data were analyzed by meta-analytic method through stata 11.0 Software, and the survival rate was measured. Results:
The 1-, 2-, 3-, 4-, and 5-year survival rates of colorectal cancer in Iran were respectively calculated as 85, 75.10, 65,
55.40, and 52.The results indicated that there is a significant relationship between anatomic location of tumor and
survival rate. According to the results of this examination, survival rate of the patients with rectal cancer was 41.9
times higher than those with colorectal cancer. Conclusion: Due to the relative high prevalence of this cancer among
young people in Iran and the low survival rate, early diagnosis of colorectal neoplasms is necessary before they become
symptomatic through more effective diagnosis programs of enhancing the patients’ health and survival rate. Moreover,
it is necessary to conduct more specialized and relevant studies in order to determine genetic or environmental causes of
cancer such as diet and cultural and behavioral habits at the national level and with different ethnicities.- انتشار مقاله: 06-12-1396

Background: Several studies have demonstrated roles of interleukins in the pathogenesis of multiple myeloma (MM). Objective: Here we considered correlations among serum levels of IL-10, stage of disease and clinical laboratory disease markers in Iranian MM patients to investigate whether the interleukin might have prognostic significance. Materials and Methods: In this cross-sectional study, a total of 60 subjects (40 patients and 20 controls) were recruited. After preliminary laboratory tests, disease stage was evaluated and serum levels of IL-10 were measured using an enzyme-linked immunosorbent assay (ELISA). Results: The mean concentration of serum IL-10 in patients (2.39±0.82 ng/ ml) was significantly higher (p<0.0001) than that in healthy controls (0.34±0.15 ng/ml). A positive and significant correlation (p<0.0001) was observed with the disease stage. The highest plasma cell proportions were recorded for MM stage III patients (68.8±9.21%), differing significantly from those of stage I patients (50.0±10.0%; p=0.011). The Beta-2 microglobulin value in stage III patients (7.7±1.13mg/l) was significantly higher than in those with stage II (4.31±0.64 mg/l; p<0.0001) and stage I (2.8±0.4 mg/l; p<0.0001). There was also a positive and significant correlation (p=0.002) between IL-10 levels and B2M. A trend (p=0.06) for positive correlation was observed between IL-10 levels and plasma cells. Conclusions: The correlation of IL-10 with disease stage and markers of disease activity indicates important roles in MM pathogenesis and progression. Therefore, measurement of serum IL-10 might be helpful for predicting stage and clinical management of MM.- انتشار مقاله: 06-12-1395

Background: Deviation of host immune response by engagement of inhibitory receptors is one of the well-known mechanisms of tumor cells for immune evasion and survival. PD-1/PD-L1 and Tim-3/Gal-9 axes are two major pathways in this area which their contribution has been documented in a variety of malignancies. In this study, Gal-9 and PD-L1 expression was investigated in leukemic cells from patients with Chronic Lymphocytic Leukemia (CLL). Methods: Peripheral blood mononuclear cells (PBMCs) were obtained from 25 untreated CLL patients and 15 sex- and age-matched healthy controls. CLL patients were classified into different clinical stages based on the Rai staging system. Total RNA was extracted from all samples and applied for cDNA synthesis. Relative expression of Gal-9 and PD-L1 mRNA was determined by Real-Time PCR using β-actin as a housekeeping gene. Results: Gal-9 and PD-L1 mRNA was significantly more expressed in CLL patients compared to healthy controls (ppatients in advanced clinical stages showed higher expression of Gal-9 and PD-L1 in comparison to patients in early clinical stages (pConclusion: Our promising results regarding over-expression of Gal-9 and PD-L1 in CLL patients call future complementary studies to more evaluate and confirm these pathways for immunotherapy approaches of this malignancy.
