- چکیده انگلیسی: Tuberculosis is a systemic disease which involves skeletal and articular system very rarely. Osteoarticular tuberculosis commonly occurs in the vertebral column and more rarely in the sacroiliac joints. In this study, we report a 44-years-old male patient with low-grade fever, malabsorption syndrome, abdominal and pelvic ascites and low-back pain, that underwent 18F-FDG PET/CT for identifying the cause of signs and symptoms after a negative abdominal CT and negative thorax radiography. The study revealed increased tracer uptake at the peritoneal ascites and at the right sacroiliac joint in absence of bone alteration suggesting a sacroiliitis. Staining of the ascitic fluid was positive for acid-fast bacilli (Ziehl–Neelsen) and in the subsequent abdominal paracentesis Mycobacterium Tuberculosis was isolated; the final diagnosis was abdominal tuberculosis with a sacroiliac joint involvement. The patient started antitubercular therapy for 6 months and the clinical conditions were resolved, in particular both back pain and ascites disappeared.

- چکیده انگلیسی: Pleural epithelioid hemangioendothelioma (EHE) is a rare malignancy of vascular-endothelial origin with non-specific symptoms and an unpredictable outcome. Diagnosis of this condition by imaging modalities is challenging, and no standard therapeutic approaches have been established in this regard. In this paper, we described the case of a patient with a low-grade fever, coughing and chest pain who underwent 18F-FDG PET/CT after a positive thorax CT showing multiple bilateral calcified pulmonary nodules and extensive right-sided pleural effusion. Moreover, PET/CT revealed increased tracer uptake on the nodular pleural thickening and one nodule in the upper lobe of the right lung. A diagnostic thoracentesis was performed to obtain the pleural fluid. However, cytology was not diagnostic, and the subsequent thoracotomy with pleural fluid drainage and pleural biopsy was positive for pleural EHE. The study showed also an abundant non-FDG-avid pleural effusion in the collapsed right lung. Despite chest tube insertion and partial drainage of the volume, patient’s condition deteriorated, and patient passed away six months after the PET scan.

- چکیده انگلیسی: Hepatosplenic candidiasis is a fungal infection, which mostly affects patients
with hematologic malignancies such as leukemia. The pathogenesis of this
infection is not clear yet, and the liver is the most commonly affected organ.
Diagnosis of hepatosplenic candidiasis can be only established via biopsy,
since computed tomography (CT) scan, ultrasonography, and magnetic
resonance imaging (MRI) yield non-specific results. The role of fluorine-18
fluorodeoxyglucose positron emission tomography /computed tomography
(18F-FDG PET/CT) in diagnosis of hepatosplenic candidiasis remains
undetermined, considering a few evidences in the literature. In this case
report, we present the case of a 47-year-old patient, affected by acute myeloid
leukemia, which was treated with three cycles of chemotherapy, resulting in
the development of neutropenia and fever following the last cycle. The 18F-FDG
PET/CT scan showed some foci of intense FDG uptake in the liver and spleen.
The subsequent diagnostic investigations (i.e., abdominal CT scan and biopsy)
were suggestive of hepatosplenic candidiasis. The patient was started on
antifungal treatment with fluconazole. After one month, the clinical conditions
