- چکیده انگلیسی: Objective: This study was designed to evaluate the clinical symptoms and sonographic characteristics of femoral trochlear cartilage in patients with knee joint osteoarthritis (OA). Methods: This cross-sectional study was conducted on 94 patients with painful primary knee OA according to the American College of Rheumatology criteria with a visual analog scale pain score of 30 mm or more. Clinical features were evaluated by a rheumatologist according to the Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) criteria, and a radiologist performed ultrasonography within a week. The thickness and echogenicity grade of three cartilage regions were evaluated. Other findings such as meniscal extrusion, effusion, and Baker’s cyst were recorded. Results: The average age of patients was 60 years. The linear regression model showed a significant association of grades II and III trochlear cartilage injury with the pain index, physical activity, and WOMAC overall index. However, there was no significant association between the trochlear cartilage thickness in these three regions and the WOMAC indexes (including pain, physical activity, and the overall index). There was also no significant association between Baker’s cyst and the WOMAC indexes, but a significant association was found between meniscal extrusion and the WOMAC index (P≤0.001). Conclusion: According to our study, the echogenicity grade determined by ultrasonography of femoral trochlear cartilage affects the clinical symptoms of patients including pain. These findings indicate the role of this diagnostic device in understanding changes due to the disease, and it can help physicians focus on the pathology in order to control clinical symptoms in patients.

- چکیده انگلیسی: Background: Studies show that planning, strategy making, and modeling have greater impact on the achievement of learners than other educational factors. The aim of the present study was to evaluate the changes in clinical competency of radiology residents by teaching and training educational models of radiologic Pattern recognition - diagnostic Approach - Feedback on diagnosis (PAF).
Methods: This semi-experimental study was performed at Mashhad University of Medical Sciences from 2018 up to 2020. The participants were 60 radiology residents divided randomly into two groups of 30 subjects in two educational centers. Eight training sessions were held at one of the two educational centers for teaching this model to assistants. Then, the clinical competency of residents was evaluated by four exams. In addition, residents' satisfaction was also determined by a confidential survey conducted by the university. The parametric tests were used for data analysis. p Results: Comparison of the scores of two groups in OSCE exam showed that in the second and third-year of being assistance, the scores of the intervention group were significantly better than the control group (P=0.02 & 0.05), but no significant difference was found between the two groups in the formative (qualitative) and multiple-choice question exams. About 82.7% of radiology residents were satisfied with this educational model.
Conclusion: Pattern-based learning, including PAF, is a practical suggestion for problem-based learning in medical imaging. It can lead to improvement of the clinical competency in radiology residents, especially levels 2 or 3.- انتشار مقاله: 11-04-1399

- چکیده انگلیسی: Objective(s): Systemic lupus erythematosus (SLE) is an autoimmune disease with a wide range of clinical manifestations. Cognitive dysfunction is one of the manifestations that could present prior to the emergence of any other neuropsychiatric involvements in SLE. Cognitive dysfunction is a subtle condition occurring with a
high frequency. However, there is no data on the correlation of cognitive dysfunction with central nervous system (CNS) imaging findings, in particular single-photon emission computed tomography scan (SPECT) and functional MRI. We decided to perform a systematic review of brain SPECT and fMRI in SLE patients with cognitive dysfunction.
Methods: PubMed, Scopus, and Google Scholar databases were searched until April 2017 with the following keywords: “SLE OR systemic lupus erythematous OR lupus” AND “functional MRI OR functional magnetic resonance imaging OR fMRI OR SPECT or SCAN”. A total of 1,767articles were found. Two rheumatologists
reviewed the articles and finally 14 articles were selected for the final systematic review.
Results: The fMRI and SPECT imaging techniques could provide valuable information regarding the SLE patients with cognitive dysfunction at the early stages of the disease.
Conclusion: Brain SPECT scan and fMRI are used as functional imaging tools in SLE. Both of these diagnostic modalities are sensitive in reflecting the subtle brain damages in SLE patients with cognitive dysfunction. Brain fMRI and SPECT scan could be significantly beneficial in the diagnosis and initial management of cognitive dysfunction in SLE. Nevertheless, prospective studies could be useful in confirming the application of these diagnostic modalities in the clinical setting.- انتشار مقاله: 01-07-1396
