- چکیده انگلیسی: Background: Medical error is defined as an adverse event caused by medical management, which can be prevented. The aim of this study was to investigate the accuracy of diagnosis of oral and maxillofacial diseases made by health care practitioners.
Methods: A descriptive cross sectional study was done between Apr. 2009 and Mar.2010. 372 Patients with oral and maxillofacial lesions who had referred to oral medicine Department of Mashhad dental school or had examined by health care practitioners were examined by two oral medicine specialists and a self established questionnaire was completed. The main aim of this questionnaire was to investigate the accuracy of diagnosis of oral and maxillofacial diseases made by health care practitioners. Histopathologic evaluation was made to achieve a definite diagnosis.
Results: Only 30/6% of initial diagnoses were consistent with the diagnosis made by oral medicine specialists. Lichen planus and inflammatory hyperplasia have been the most common diseases which health care practitioners did not mention a diagnosis for them.Among the most common misdiagnosis lichen planus and abscess each involved 8 percent of misdiagnosis.
Conclusions: Unfortunately in present study there was little compatibility between diagnosis of dentists and practitioners with oral medicine specialist. Sometimes misdiagnosis results in postponing the treatment, patients` suffering and leads to side effects of unnecessary or incorrect treatment. As old people are more susceptible to oral diseases and lesions, there should be a holistic effort to find the cause of present problems and then resolving them through educational promoting and expanding the teamwork among physicians to diagnosis and treatment of the patients.- انتشار مقاله: 27-03-1393

This study aimed to determine the relationship between one of the histocompatibility antigens (HLA DRB1) and its sub-groups with the incidence of recurrent aphthous ulcers in an Iranian population (North East of Iran).
Methods: In this case-control study, a total of 72 patients with recurrent aphthous ulcers and 70 healthy subjects in Northeast Iranian population were included. Genotyping was done by polymerase chain reaction-specific sequence primers (PCR- SSP) for each sample, according to standard kit protocol (BAG- Germany).
Results: In 72 patients with recurrent aphthous ulcers that were included in this study, 26 were male and 46 were female; of the 70 control patients, this difference not significant statistically (P>0.05). The frequency of HLA -DRB1 *16 was 0.7% in the healthy subjects, however frequency of HLA -DRB1 *16 in patients with recurrent aphthous stomatitis (RAS) was 42.36%, and this difference was statistically significant (P=0.03). But, this difference was not observed in other subgroups.
Conclusion: The frequency of DRB1 * 16 in the patients with RAS were higher than the group. Therefore, DRB1 * 16 can be suggested as a Predisposing factor for aphthous ulcers patients.- انتشار مقاله: 30-04-1399

- چکیده انگلیسی: Background: Lichen planus is an autoimmune disorder and is associated with other autoimmune diseases. There is, however, little evidence of the association of oral lichen planus with celiac disease. The aim of this work was to investigate, for the first time, such an association in patients in the city of Mashhad, Iran. Methods: This case-control study was performed during October 2017 to March 2018 in the department of Oral and Maxillofacial Medicine, Faculty of Dentistry, Mashhad University of Medical Sciences, in Iran. All participants were evaluated for Anti-TTG (IgA) and Total IgA, and in some cases for Anti-TTG IgG. Data were analyzed using SPSS software v.20. Results: A total of 96 subjects were considered in the study; 32 in the case group, and the rest in the control group. The mean value of Anti-TTG IgA was 0.12 ± 1.51 Au/ml in the oral lichen planus group, while it was 0.57 ± 1.20 Au/ml in the control group with no significant difference (P=0.167). The mean value of the Total IgA was 134.96 ± 42.86 mg/dl in the lichen planus group, and it was 129.85 ± 55.28 mg/dl in the control group, as they differ negligibly either (P=0.639). Moreover, celiac disease was not present in the population. Conclusions: We showed that there was no celiac disease present in the oral lichen planus patients as well as healthy subjects. Further studies are required to imply or to rule out the association of oral lichen planus and celiac disease.

- چکیده انگلیسی: Background:Oral squamous cell carcinoma (OSCC) is the most common malignancy of the oral cavity. Despite some improvements in treatment, the survival rate is still very low, mainly due to the possible development of secondary malignancy or metastasis. Clinical and pathological features as well as molecular biomarkers might predict the recurrence. In recent years, many studies have been carried out on molecular biomarkers that can predict the prognosis of OSCC. One of these markers is the epidermal growth factor receptor (EGFR), which has led to different results. The aim of this study was to determine EGFR level in OSCC and to analyze its correlation with clinicopathological features. Methods: A total of 62 paraffin-embedded samples from OSCC patients treated in the oncology department of the Omid Hospital in the city of Mashhad, Iran were selected and EGFR staining was performed. The clinical and histopathological data were extracted from the medical records. Results: EGFR expression was positive in 98.4% of the cases. There was a significant difference between EGFR expression in the tumor and control cases in terms of cellularity and intensity (p˂0.001 and p=0.004, respectively). No statistically significant correlation was observed between EGFR and clinicopathological parameters. There was also no significant relationship between the cellularity and intensity expression of EGFR and patient survival (p=0.92 and p=0.42, respectively). Conclusion: In view of the high EGFR expression in squamous cell carcinoma, further studies on the role of EGFR in cell processes such as proliferation, angiogenesis and differentiation of the tumor are recommended.

- چکیده انگلیسی: Statement of the Problems: Lichen planus is a chronic inflammatory disease. Free radicals and reactive oxygen species play important roles in pathogenesis of oral lichen planus (OLP). Isoprostanes show oxidative stress and are formed by free-radical mediated lipid peroxidation of arachidonic acid and cell membrane phospholipids.Purpose: This study was conducted to evaluate the plasma level of 8-isoprostane in patients suffering from erosive and non-erosive forms of OLP.Materials and Method: In this case-control study, 31 patients with OLP and 30 control subjects were enrolled. Plasma samples were obtained and the level of 8-isoprostane was measured with Sandwich enzyme-linked immunosorbent assay (ELISA) in both groups. The data were analyzed by using two-sample t-test, chi-square and ANOVA tests.Results: The results showed significant increase in the plasma level of 8-isoprostane in OLP group compared with the control group. The results of independent t-test revealed no significant correlation between the plasma level of isoprostane and sex, smoking, or previous treatment.Conclusion: Based on the findings of this study, oxidative stress was increased in patients with OLP, reflected by higher concentrations of 8-isoprostane in plasma.Key Words: Oral Lichen Planus; Isoprostane; Oxidative Stress

- چکیده انگلیسی: Adenoid cystic carcinoma (AdCC) is a malignant neoplasm, which accounts for 5-10% of all salivary gland tumors (1). About 50% of these tumors originate from intraoral minor salivary glands usually in the hard palate (1). Three clinically obvious characteristics of AdCC include slow growth rate, perineural invasion and high incidence of distant metastasis (1). The most commonly-affected sites of distant metastasis are bone, liver and brain, followed by lungs (2). Lymph node metastases are rare; The most common sites involved by hematogenous spread are lungs (2). This is a report about a patient with a rare form of AdCC on buccal mucosa with an unusual metastasis to the frontal region after a two-year follow up.

Oral lichen planus (OLP) is a common chronic mucocutaneous disease. Patients with atrophic and erosive types of OLP often have symptoms of soreness, and require proper treatment. The main treatment for OLP has been the administration of topical or systemic corticosteroids. The objective of this study was to compare the efficacy of adcortyl cream (triamcinolone acetonide in orabase) with topical pimecrolimus cream for the treatment of erosive OLP.
Materials and Methods:
Twenty-eight patients with OLP were enrolled in a single blind clinical trial and assigned to either a pimecrolimus 1% cream group or an adcortyl 0.1% cream group. The medication was applied every day for 2 months and patients were assessed every 2 weeks.
Results:
The mean lesion size and mean pain and burning sensation scores did not differ between the pimecrolimus and adcortyl cream groups. The pimecrolimus cream was well tolerated. No clinical drug-related adverse events were observed.
Conclusion:
Topical pimecrolimus cream may be recommended as a safe and effective alternative therapy in the treatment of OLP. Pimecrolimus cream is as effective as adcortyl cream in managing the signs and symptoms of OLP.- انتشار مقاله: 18-12-1392

Oral lesions are among the earliest clinical manifestations of human immunodeficiency (HIV) infection and are important in early diagnosis and for monitoring the progression to acquired immunodeficiency syndrome (AIDS). The purpose of this study was to determine the prevalence of oral lesions and their relationship with a number of factors in HIV/AIDS patients attending an HIV center.
Methods:
A total of 110 HIV-positive patients were examined to investigate the prevalence of oral lesions according to the criteria established by the European Community Clearing House on Oral Problems Related to HIV Infection. An independent T-test was used for correlation of oral lesions with CD4+ count and a χ2 test was used for analysis of the relationship of co-infection with hepatitis B virus (HBV), sexual contact, route of transmission, history of drug abuse, and history of incarceration.
Results:
Most of the cases were male patients (82.7%). The mean age across all participants was 36.2±8.1 years. Rampant carries, severe periodontitis and oral candidiasis were the most notable oral lesions. Oral lesions were more prevalent in patients between 26–35 years of age. There was a significant difference between patients with and without pseudomembranous candidiasis and angular cheilitis according to mean level of CD4+.
Conclusion:
The most common oral presentations were severe periodontitis, pseudomembranous candidiasis and xerostomia.- انتشار مقاله: 22-10-1392

- چکیده انگلیسی: Multiple myeloma is a monoclonal malignant proliferation of plasma cells. The disease is more frequent in men, and the average age of patients at the time of diagnosis of the disease is about 60 years. Solitary myeloma may be the first manifestation of the disseminated form of multiple myeloma, which is characterized by multiple skeletal lesions, general metabolic alterations, impairment of renal function and eventually death.The findings in regards to the present case suggest that oral manifestations may be the first sign of multiple myeloma. This might highlight the important role that a dentist can have in the early diagnosis of the disease.
