- چکیده انگلیسی: Background : Commitment to evidence-based medicine (EBM) means the informed and fair use of the best evidence for accurate diagnosis and treatment decisions for patients. This approach attempts to improve the quality of clinical decision-making. The current study aimed to investigate the effects of EBM training courses on the theoretical knowledge and practical skills of clinical residents. Methods : A quantitative method based on a quasi-experimental single group design was used in this study. Two researcher-made questionnaires and one test were used as the major instruments for evaluating the satisfaction, knowledge and practical capabilities of participants. The content validity of the questionnaires was confirmed by five educational experts in medical education and clinical practices, and the reliability was calculated through Chronbach’s alpha (r = 0.92 and 0.93). The questionnaires were distributed among all 110 junior clinical residents who participated in the EBM workshop; 62 residents completed the questionnaire. The gathered data was analyzed through SPSS version 14 using paired t-test and Pearson’s correlation. Results : Participants took a high level of satisfaction (means of all items were higher than the cut-off points) from the evidence-based medical course. Participants’ knowledge and practical abilities were significantly broadened as a result of attending the EBM course (P < 0.01). In addition, a positive significant correlation was found between the knowledge scores and practical ability scores of residents who participated in the EBM course (r = 84%). Conclusions : More emphasis needs to be placed on developing the practical skills of residents in the EBM course. It is also essential that clinical ward residents be required to make clinical decisions based on evidence-based medicine.

- چکیده انگلیسی: Background: Satisfaction with life indicates an individual’s overall evaluation of life, which is influenced by a range of factors including socio-demographic characteristics, psychological issues, and life experiences. Objectives: The aim of this study was to explore the main determinants of life satisfaction among young Iranian women. Patients and Methods: 391 young Iranian women (18 - 35 years) were recruited through cluster convenience sampling to participate in this cross-sectional study. Satisfaction with life (SwL) was used as the dependent variable. Demographic characteristics together with the individual items of the Persian version of the WHO QoL-BREF (World Health Organization quality of life questionnaire-brief version) were used as the independent variables and/or the determinants of SwL. Results: The analysis showed that young Iranian women’s satisfaction with life was average (19.9 ± 6.4), ranging from 5 to 35. The regression analysis identified higher income, better living conditions, higher level of leisure activities, and satisfactory sexual life as the main predictors of higher satisfaction with life. Satisfaction with life diminishes with increased age, being divorced and/or widowed and being of Lor ethnicity (vs. Fars and Tork ethnicities). On the other hand, those with higher education and/or level of religiosity had a higher satisfaction with life. Conclusions: In transitional countries, particularly those with different social and cultural situations, identifying and enhancing the main determinants of SwL is of importance in order to improve socially cohesive and robust community.

This cross-sectional study was conducted to compare the average costs of breast cancer screening and treatment among women with the age of 25 and over in Shiraz-Iran.
Methods
Three majors hospitals affiliated with Shiraz University of Medical Sciences (SUMS) were selected for data collection. Financial documents and interviews with the hospitals’ financial officers were used for data collection.
Results
Finding shows that the total cost of screening would be 5,847,544.96 US dollars for age groups of 25–34 and 35 and above, demonstrating the huge expense of screening programs. On the other hand, the average cost of breast cancer treatment for each patient would be 3608.47, 996.89, and 311.47 US dollars for mastectomy, radiotherapy, and chemotherapy, respectively. In addition, the total average cost for treatment of 2217 patients would be 1,466,988.9 US dollars, which is much less than screening programs expenses.
Conclusion
It is concluded that although screening can be effective for improving quality of life and treatment effectiveness, considering the high costs of screening, it is not economical in Iran. Screening methods within suitable intervals, and also considering patients’ medical history have been recommended by the present study.- انتشار مقاله: 24-10-1392

- چکیده انگلیسی: Background and Objectives: A career anchors consists of jobs that a person holds in order to achieve a professional goal. Career anchors development management results in job satisfaction, professional commitment, and higher effectiveness. The purpose of this study was to determine different dimensions of career anchors among in the personnel of vice-chancellor offices of Shiraz University of Medical Sciences. Methods: This cross-sectional study was conducted in 2012. The study sample was 236 personnel of the Vice-chancellor offices of Shiraz university of Medical Sciences, selected using ratio estimation method. The 43-item Schein's career anchors orientation inventory was used as the study instrument. T-test, ANOVA and Spearman coefficient were used for data analysis. Findings: The highest mean score of career anchors was observed in service port (3.28 ± 0.43) and the lowest mean was observed in identity port (2.47 ± 0.5). Significant inverse relationships were found between age and work experience in the innovation port. There was also significant difference between genders in service and security ports. Conclusions: The main career anchor was related to services port, which highlights the importance providing quality health care services to the patients. In order to improve work situation it is recommended to hire well-educated managers for job development and provide educational courses for managers and personnel.
