- چکیده انگلیسی: Background: Pain has physiological and psychological effects on a child's life and various body systems. Tactile stimulation affects the central nervous system and releases analgesic drugs resulting in the inhibition of pain.
Aim: The main purpose of this study was effect of touch on the intensity and duration of pain during venipuncture in school-age children.
Methods: This clinical trial was conducted on 60 children of school age in the emergency department of Imam Reza (AS) hospital and doctor Sheikh Hospital in 2014. Children were selected by convenience sampling and randomly divided into two groups, touch and control group. Touch group received intervention for 5 minutes by palms movements and kicks on the site before and during venipuncture by researcher, and then venipuncture was immediately performed by a nurse colleague. The control group did not receive any intervention. FACES instruments were used to assess pain intensity. Pain was measured for 5 min by a chronometer. Data were analyzed by Kolmogorov-Smirnov, Chi-square tests, Mann - Whitney and independent t-test using SPSS software version 11.
Results: In this study, 65% were boys and 35% girls. The average age of children was 8.3±2.3 years. There was a significant difference between touch and control groups in pain of needle insertion into the skin within the first second (p<0.001). Independent t-tests showed that there was no significant difference between touch (242.0±67.7) and control (278.0±82.3) groups in duration of venipuncture pain (p=0.07).
Conclusion: Touch is effective in reducing the intensity of pain during venipuncture in children. This is an easy and practical nursing intervention to reduce negative impact of pain on children.- انتشار مقاله: 01-11-1392

- چکیده انگلیسی: Epilepsy is a major neurological disorder, which may occur in all age groups, including children. Approximately 20% of epileptic children are drug-resistant. Uncontrolled seizures pose variable risks to patients, such as increased mortality rate, trauma, and cognitive and psychiatric disorders. Therefore, effective treatment is essential to controlling seizure attacks. Although many antiepileptic drugs are currently available for clinical treatments, clinicians are concerned with the discovery of safer drugs with enhanced antiepileptic effects and fewer side-effects. Traditional medicine provides strong grounds for modern medicine. Use of some medicinal plants has been shown to reduce or prevent the further progression of epileptic seizures. The present review aimed to discuss the effectiveness of some medicinal plants in the treatment of children with intractable epilepsy. An online literature review was conducted in databases such as IranMedex, Scopus, Medline, and Google Scholar to identify the studies investigating the use of medicinal plants in children with intractable epilepsy. In addition, the files of the authors were reviewed in the reference lists and bibliographies of the retrieved articles. According to the results, herbal therapies could potentially yield new treatment options for children with intractable epilepsy. Using medicinal herbs could be a cost-efficient treatment method in these patients as a culturally acceptable option to their families.

- چکیده انگلیسی: The purpose of this study is to report a case of severe back pain following lumbar puncture (LP), which was associated with an accidental tether cord syndrome (TCS) in MRI. The patient was a 10-year-old boy that was hospitalized in the pediatric ward for severe headache and to rule out meningitis. First, LP was performed for him, but he suffered very severe back pain after LP. Lumbosacral MRI was performed to rule out collection or abscess formation, but the evidence of the TCS was accidentally found; however, it was too unexpected before. In the short term follow up, the symptoms of the patient were completely resolved by using analgesics. It was shown that his pain was as a result of LP. In general, although sometimes severe lower back pain after LP may be overwhelming, if neurologic examination were normal, it is important to assure parents that it would be resolved.

- چکیده انگلیسی: The active form of vitamin D is synthesized in some body organs following sun exposure and dietary intake. Vitamin D exhibits its major and critical effects not only through regulation of calcium and phosphate metabolism but also by influencing on respiratory and immune system. Serum concentrations of 25-hydroxyvitamin D below the optimum limit lead to vitamin D insufficiency or maybe deficiency. These inappropriate concentrations of vitamin D lead to different types of pulmonary diseases such as viral and bacterial respiratory infection, asthma, chronic obstructive pulmonary disease, and cancer. In this review we described the association between vitamin D deficiency and severe therapy resistant asthma. We also reviewed the underlying molecular mechanism of vitamin D deficiency in children with severe- therapy resistant asthma. Based on current information, future clinical trial are needed to study the role of vitamin D supplementation on different groups of patients with severe asthma including infants, children of school age, and ethnic minorities.

- چکیده انگلیسی: Background: Since the treatment of meconium ileus is very different from other types of intestinal obstruction, it is very important to distinguish these two entities. The aim of this study was to assess sonographic findings to differentiate meconium ileus from other types of obstruction.
Methods: This study was performed in Dr. Sheikh and Akbar hospitals, Mashhad Medical University of Science, Iran, during 2017- 2018. The imaging signs of 25 newborns (aged 2-8 days) with delayed passage of meconium were documented. These neonates were suspicious of low type intestinal obstruction. In cases with the simple form of meconium ileus and the small left colon syndrome, ultrasonic guided Gastrografin enema was performed for the treatment. In the patients with the failure of medical treatment and in other surgical cases, the appropriate surgical procedure was performed.
Results: The subjects in this study were diagnosed with small bowel atresia (n=9), a simple form of meconium ileus (n=6), a complicated form of meconium ileus (n=3), small left colon syndrome (n=3), bowel atresia and duplication (n=1), and Hirschsprung’s disease (n=3). The ultrasound findings were as follows: a totally or partially micro-colon in almost all patients, collapsed small bowel in right lower quadrant in all of the patients with bowel atresia, hypoechoic meconium in rectosigmoid in small left colon syndrome and Hirschsprung’s disease, hypoechoic tubular or beaded intraluminal inspissated meconium within terminal ileum, and the floating air bubbles (sonographic soap bubble sign) within fluids on proximal dilated loops in meconium ileus cases.
Conclusion: Although plain abdominal radiography confirmed bowel obstructions in all patients with delayed passage of meconium, it had a low level of specificity in the diagnosis and cause of obstruction. Abdominal ultrasound had a high level of accuracy in the correct diagnosis of meconium obstructions and its differentiation from other intestinal obstructions. Accordingly, it eliminates the need to do a diagnostic contrast enema in order to differentiate these entities.- انتشار مقاله: 26-12-1396

- چکیده انگلیسی: Background: Prematurity is the most common cause of death and disability And Preterm infants, are prone to developmental complications. For this reason this study was designed for follow up of these babies until 2 years by modified DDST-2.
Methods: This study was a prospective longitudinal descriptive study from March 2009 to March 2011 in clinic of sheikh and Imam Reza Hospitals, mashhad, Iran. Sample size with Confidence coefficient of 95% and power 80%, was determined 100 hundred babies. Infants were seen by a pediatrician at a follow up clinic at 1, 3, 6, 9,12,15,18, 24, months.The developmental assessment was done using Denver-2 Developmental Screening Test.
Results: mean age for smiling was 4/6 ± 2/1 months which significantly differed with appropriate age (p = 0.000), mean age for telling two syllables words 11/7± 1/9 months, without significant difference of appropriate age.(p = 0.139). Average age for understanding NO was 10/4± 2/0 months that significantly differed with appropriate age(p = 0.000). The average age for telling 6 word was 17/8± 3/0, without significant difference with appropriate age (p = 0.510).

Asthma is the most common chronic airway disease in children. We aimed to investigate the effect of vitamin C supplementation on improving the asthma symptoms in children.
Materials and Methods: In a double-blind clinical trial: sixty 2-12-year-old children hospitalized with the diagnosis of asthma attack in the Ghaem and Dr. Sheikh hospitals, Mashhad, Iran were randomly assigned into two groups: intervention (receiving vitamin C tablets 500 mg every 12 hours, n=29), and control (receiving placebo, n=31). In each group, at the beginning and end of the hospitalization, one blood sample (3ml) was taken to evaluate the serum level of vitamin C and for other necessary tests. A pediatric resident recorded clinical symptoms at the beginning and every 6 hours until discharged including tachypnea, wheezing, retraction, and hypoxia every six hours until discharge from the hospital. Eventually, the data in the two groups were compared.
Results: The mean age of the hospitalized patients was 33±24 months. There was a significant difference between the intervention and control groups regarding the serum level of vitamin C at the beginning of hospitalization and at the time of discharge. A significant difference was found in tachypnea of the hospitalized patients of both groups at 24, and 36 hours post hospitalization. Existence of wheezing in the patients of the intervention and control groups had a significant difference at 18, 36, and 66 hours post-hospitalization (p <0.05).
Conclusion
According to the results, oral vitamin C prescription in patients with acute asthma symptoms leads to improved respiratory status (tachypnea and wheezing) in 2-12-year-old children. Also, it is effective in reducing the duration of hospitalization of these children.- انتشار مقاله: 24-01-1399
