- چکیده انگلیسی: Anomalous origin of the left coronary artery from the pulmonary artery (ALCAPA) is a rare congenital cardiac malformation. We report three cases of ALCAPA who survived to adulthood. The first case was a 51-year-old woman who complained of typical chest pain that was diagnosed with ALCAPA using cardiac catheterization and coronary computed tomographic angiography (CTA). The second case was a 30-year-old woman with a history of surgery for atrial septal defect at 10 years old who presented with progressive exertional dyspnea. Cardiac catheterization confirmed the diagnosis of ALCAPA. The third case was a 19-year-old man who was brought to our clinic due to aborted sudden cardiac death on the previous day. Cardiac catheterization and coronary CTA confirmed the diagnosis. They underwent the closure of orifice of the anomalous left coronary artery and grafting the left anterior descending artery concomitantly with mitral valve repair. All patients were followed up during a mean of 8.7 months and they were asymptomatic.

- چکیده انگلیسی: Considering the fact that rocks fail faster under tensile stress, rock tensile strength is of great
importance in applications such as blasting, rock fragmentation, slope stability, hydraulic fracturing,
caprock integrity, and geothermal energy extraction. There are two direct and indirect methods to
measure tensile strength. Since direct methods always encompass difficulties in test setup, indirect
methods, specifically the Brazilian test, have often been employed for tensile strength measurement.
Tensile failure is technically attributed to crack propagation in rock. Fracture mechanics has
significant potential for the determination of crack behaviour as well as propagation pattern. To apply
Brazilian tests, cracked disc geometry has been suggested by the International Society for Rock
Mechanics ISRM. Accordingly, a comprehensive study is necessary to evaluate stress field and stress
intensity factor (SIF) around the crack in the centre of the specimen. In this paper, superposition
principle is employed to solve the problem of cracked straight-through Brazilian disc (CSTBD), using
two methods of dislocation and complex stress function. Stress field and SIF in the vicinity of the
crack tip are then calculated. With the proposed method, the magnitude of critical load for crack
initiation in structures can be predicted. This method is valid for any crack of any arbitrary length and
angle. In addition, numerical modelling has been carried out for the Brazilian disc. Finally, the
analytical solution has been compared with numerical modelling results showing the same outcome

Objective: Familial adenomatous polyposis (FAP), an autosomal dominant inherited disorder is characterized by the presence of multiple adenomatous colorectal polyps, which can develop into cancer during early adulthood. Therefore, early diagnosis is essential. Most FAP patients have several extracolonic manifestations, including congenital hypertrophy of the retinal pigment epithelium (CHRPE). Whereas genetic markers may provide the main route to detection of ‘‘at risk’’ subjects , at present this approach is clearly limited and searches for a noninvasive phenotypic marker continue to be high priority.The aim of this study was to describe the pattern of distribution of CHRPE lesions and evaluate their diagnostic value in FAP patients and their family members in a local population. Methods: A total of 23 FAP patients and 26 relatives belonging to 12 families at high risk of developing FAP were subjected to colonoscopic and ophthalmological examination. Result: Retinal examinations demonstrated prevalences of CHRPE in FAP patents and their siblings of 78% and 38%, respectively. We were able to illustrate a significant correlation between FAP disease and the presence of retinal lesions. Sensitivity and specificity of CHRPE as a screening test to detect the presence of FAP are 78.3% and 61.5%, respectively, with a positive predictive value of 64.3% and a negative predictive value of 76.2 %. A "lesion form" significant difference was found between FAP and normal participants.Spearman nonparametric analysis revealed no correlation between age and number or size of lesions. Conclusion: Multiple CHRPE lesions are a diagnostic feature of FAP patients They are specific and sensitive clinical markers of this disease (specificity 60% and sensitivity 77%).- انتشار مقاله: 27-06-1396
