- چکیده انگلیسی: BACKGROUND AND AIM: Nowadays, resin composites are the preferred material for direct posterior restorations. The present study was conducted to assess the performance and problems of general dental practitioners during and after posterior direct composite restorations in Kerman, Iran.METHODS: 160 general dentists (63% men and 37% women) participated in this analytic-descriptive study. Data were collected using a researcher-made questionnaire. The questionnaire consisted of 13 close-ended questions about the performance and problems of dentists associated with composite restorations in Kerman, in 2016. Data were analyzed by chi square and t-test using SPSS software.RESULTS: The most frequent complaint of patients after composite restorations was food impaction (45.0%), and the most clinicians' problems during composite restorations were achieving tight proximal contact (59.3%) and proper isolation (59.3%). Most of the dentists used traditional metal matrix systems (70.0%), wet polishing technique (81.2%), light-emitting diode (LED) light curing unit (62.5%), packable composites (51.2%) with incremental technique (83.1%), and two-step total etch bonding (70.0%). The main criteria for selecting composite as restorative material was patient request (55.0%), and most of the dentists preferred to restore the small or moderate class I cavities (67.5%), and after it, class V cavities (57.5%) with composite. Moreover, secondary caries (58.1%) was the most common reason for replacement of composite restorations; and in all questions, there were statistically significant differences between the most prevalent answer and the other answers (P < 0.05).CONCLUSION: The most frequent problems of dentists (achieving proper contact) and the most frequent complaints of patients (food impaction) are related to the use of traditional metal matrix systems.

- چکیده انگلیسی: BACKGROUND AND AIM: Inflammatory Periapical lesions are the most common periapical lesions. These lesions are often diagnosed based on clinical and radiological examination, which may be different from the histological finding. The present study was carried out with aim to assess the rate and to compare clinical and radiographic features with histological diagnosis of periapical pathology.METHODS: This retrospective study was conducted on dental school pathology records in Kerman, Iran, from 1996-2014. Cases with incomplete records and without net histology diagnosis were excluded. Age, gender, site of lesion, clinical diagnosis, and histopathological diagnosis were extracted from the records. The data were analyzed using chi-square test in SPSS software. A P value of 5% was considered as the significance level.RESULTS: 1854 cases met the inclusion criteria. 129 of the cases were periapical inflammatory lesions. The mean age of individuals was 29.39 ± 15.14 years. Periapical cysts were the most incident lesions in clinical and histopathologic diagnosis. An overall of 98.9% of histopatologic diagnoses were compatible with clinical diagnosis. Inflammatory periapical lesions in maxilla were greater and this difference was significant (P = 0.001). Inflammatory periapical lesions were higher among men compared to women, with significant difference (P = 0.001).CONCLUSION: This study showed that periapical cysts were the most incident inflammatory lesions followed by periapical granuloma.

- چکیده انگلیسی: BACKGROUND AND AIM: Oral diseases are very important because they impose economic and social burden on societies. Given the resource scarcity, it is necessary to devise cost-effective and scientific strategies to prevent and control oral diseases. This study aimed to estimate the potential cost-savings for dental caries treatment associated with Community Water Fluoridation Program (CWFP) in Kerman, Iran.METHODS: An economic model to compare the costs of CWFP with treatment savings achieved through averted tooth decay was developed. Direct cost of prevented caries was taken equal to treatment savings. Implementation cost and the associated savings was estimated for Kerman in 2016. We obtained required data and parameters for costs and savings estimation through published documents and other sources.RESULTS: Annual cost-savings associated with implementing the water fluoridation in Kerman was estimated about $11160415.5 to $44350544.11. About $34.9 to $136 could be achieved per each dollar spent. Annual cost and benefit per capita was $0.66 and $23.4-$91.09 respectively. Net benefit per capita was $22.7 to $90.4.CONCLUSION: This study indicates significant annual savings from CWFP; additional savings could be achieved if this program is implemented in other regions. We could also receive even more if this program is integrated with other public oral health programs such as screening school children, community dentistry and oral health education.

- چکیده انگلیسی: BACKGROUND AND AIM: Pain control is an important part of pediatric dentistry. The purpose of this study was to evaluate pain and behavioral reaction who receive an injection with conventional and telescopic dental needles.METHODS: A total of 50 healthy children aged 4-8 years were included to this study to get a dental injection with the telescopic or the conventional dental needles. Two observers scored videos of children at the time of injection procedures based on sound, eye, motor (SEM) scale and distress reaction to evaluate the observed pain-related behavior. Children completed a face version of visual analog scale (VAS) after injection. Reliability of observer’s opinion evaluated and was established at 96%. Independent t-test and chi-square tests were used for statistical analysis. Statistical significance was defined at P < 0.0500.RESULTS: This study was conducted among 23 girls and 27 boys with mean age 5.3 ± 1.4. The pain scores according to VAS for the telescopic, and the conventional dental needles were 40.20 ± 10.50 and 56.40 ± 14.63, respectively, which was statistically significant between the two groups (P = 0.0001). The difference of SEM values for the telescopic and the conventional groups were statistically significant in totals as well as individual parameters (P = 0.0001). According to mean distress scores, patients showed less muscle tension, less verbal protest and less movement when receiving the telescopic needles (P < 0.0500).CONCLUSION: Telescopic dental needles with the ability of using topical anesthesia before needle insertion and covering needle sight out of patient’s eyes may be a good intervention to reduce pain and anxiety of children during dental injection.

- چکیده انگلیسی: BACKGROUND AND AIM: Dental caries is a multifactorial disease that affects people of any age, sex, and race. Significant caries index (SiC index) has been defined by World Health Organization (WHO) to evaluate caries status in different societies. The aim of this study was to determine the SiC index in 15-year-old children in Kerman. METHODS: This descriptive cross-sectional study was carried out on 300 of 15-year-old children that had been selected by systematic clustering sampling in Kerman. Data were collected by questionnaire (parents’ educational level, parents’ occupation, birth rank, number of children in family, tooth brushing, fluoride use, and regular dental visits) and clinical examination. Teeth were examined by dental mirror, according to WHO criteria. Data were analyzed in SPSS by using ANOVA, t-test, and Kruskal-Wallis tests. RESULTS: Thirty-two and nine brushed their teeth twice daily, 49.8% had never used dental floss, 47.8% had regular dental visit, 80.66% did not use fluoride mouthwash, and 34.4% were caries free. The mean of decayed, missing, and filled teeth (DMFT) was 1.83 ± 1.26 and SiC index was 3.80. There was a significant correlation between the SiC index and parents’ job and birth rank of student. There was also significant correlation between regular dental visit and use of fluoride mouthwash and SiC index. CONCLUSION: The prevalence of dental caries in 15 years student in Kerman is still high. Using DMFT and SiC indices together may help to show oral health status better than DMFT index alone. Further studies are recommended.

- چکیده انگلیسی: Background & Aims: Repair of defective restoration is more conservative than replacing it. Veneering the amalgam restorations with opaque resin materials can satisfy the esthetic demands. Evaluation of interfacial bond quality between amalgam and composite surface is one of the important factors in assessing the quality of repaired amalgam which can be achieved by micro leakage test and bond strength test. The aim of this study was to assess the effect of introduced amalgam and also the bonding agent on shear bond strength of amalgam and composite interface.
Methods: First, 72 acrylic models (20*33 mm) were provided and categorized into two groups of A and B. we used Tytin (kerr,USA) amalgam in group A and Cinalux (Shahid faghihi, Tehran) amalgam in group B. Amalgam surface was scratched by using a diamond bur. All the groups were etched and rinsed. In A1 group, nothing was done after etching and rinsing. In A2 group, single bond and in A3 group Excite was used. B1, B2 and B3 groups were respectively prepared like A1, A2 and A3 groups. DenFil composite was used in all groups. Shear bond strength was assessed by using Universal testing machine. All the specimens were seen under stereo microscope (Olympus, Germany) with 75% magnification. Tukey HSD test was used for statistical analysis.
Results: In the groups which were filled with Tytin, A3 group had significantly higher bond strength than A1 group (P=0.001). In groups which were filled with Sinalux, B3 group had significantly higher bond strength than B1 and B2 groups (P=0.001 and 0.03 respectively). Bond strength was significantly higher in A1 compare to B1, A2 compare to B2, and A3 compared to B3 (P=0.001).
Conclusion: According to the obtained results, the type of introduced amalgam and also the bonding agent can affect shear bond strength of amalgam and composite interface. Tytin amalgam compared to Cinalux amalgam can create more shear bond strength. Also, excite bonding causes higher bond strength compared to single bond.- انتشار مقاله: 25-11-1394

- چکیده انگلیسی: Statement of the Problem: Direct pulp capping (DPC) is a proven method in which the exposed pulp is coated with a suitable material to prevent further damage and to help its repair and healing.
Purpose: The purpose of this study was to compare the response of human dental pulp after DPC with calcium enriched mixture (CEM), mineral trioxide aggregate (MTA) cement and Biodentine.
Materials and Method: In this clinical trial study, class V cavities were prepared on the buccal surfaces of 30 human premolar teeth, until the pulps were mechanically exposed. Then, teeth were randomly pulp capped with MTA, CEM cement and Biodentine, followed by Resin modified glass ionomer filling. The fourth group was control (n = 10), which were extracted without any intervention. Six weeks after the intervention, the teeth were extracted and prepared for histological evaluation in terms of the type and degree of pulp inflammation, dentin bridge formation and the presence of necrosis. Data were analyzed using Kruskal-Wallis and Mann Whitney U tests.
Results: In all groups, Necrosis was not observed and inflammation was chronic. The Biodentine group exhibited significantly more pulpal inflammation compared with the other groups (p = 0.001). There were no significant differences among CEM cement, MTA and Biodentine in terms of dentine bridge formation. The thickness of dentin bridge formed in the Bidonetine group was significantly higher than MTA and control groups (p = 0.035 and p = 0.011, respectively).
Conclusion: Although dentin bridge formation and the thickness of dentin bridge formed in the Biodentine group were higher than the other groups, pulp showed greater inflammation compared to CEM cement and MTA. - چکیده انگلیسی: compare the effect of different ceramic thicknesses on degree of conversion (DC) of 3 light-cured resin cements. Methods: In this experimental in-vitro study, the degree of conversion of three light-cured resin cements, Variolink Veneer (Ivoclar, Liechtenstein), RelyX Veneer (3M ESPE, USA) and Choice2 (Bisco, USA) were evaluated beneath feldespatic ceramic discs (Vita VMK Master) with a same shade, 2m2, in different thicknesses (0.5, 1, 2 and3 mm) using FTIR. The light curing unit used was Optilux 501, with an intensity of 600 mW/cm2 and exposure duration of 40 seconds. Three specimens of each cement group were examined in each condition. The obtained data was submitted to Kolmogorov-Smirnov and also checked for absence of skewness and kurtosis for normal distribution. After that, ANOVA test was used for comparison between experimental groups (Tukey HSD). Results: In all the three used cements, DC decreased as ceramic thickness increased. This reduction was not significant when using 0.5 and 1 mm ceramic discs, however, it was significant between 1, 2, and 3mm discs(p<0.05). There was no difference within the cements’ DC when exposed through ceramic discs of 0.5, 1, and 2mm, but the Rely X Veneer cement had a lower DC compared to Variolink Veneer and Choice2 when the thickness increased to 3mm(p<0.05). Conclusion: It is not advisable to use light-cured resin cements beneath 3 mm thick ceramics and the use of Rely X Veneer is not recommended as the ceramic thickness increases.

- چکیده انگلیسی: Introduction: Resin composites are an important part of restorative treatment in modern dentistry. There are many trademarks available in Iran market and physico-chemical properties of many of these resin composites are not evaluated yet. This study was carried out to evaluate the physico-chemical properties of four available resin composites and also to assess the effect of aging on their properties. The findings of this study help clinicians to choose the best materials available. Methods: Four types of resin composites available in Iran market including Filtek Z350 XT, Filtek Z250, Herculite XRV Ultra and Herculite XRV were evaluated. Five samples were made of each composite using a metal mold (2* 10 * 10 mm). Two areas of each sample were light cured for 40 seconds. Samples were kept in saline for 24h and then underwent polishing process. Samples of each group were randomly allocated to five groups held in different conditions as follows: room temperature, distilled water in 37˚C, Distilled water plus hydrochloric acid, distilled water and thermocycling (5000 rounds of 5-55˚C), distilled water, thermocycling, and hydrochloric acid. At the end of one month, indentation was applied on each sample using nano-indentation technology (TriboIndenter, Hysitron, Minneapolis,MN).
Results: The results of this study revealed that the type of composite and the environmental conditions affect elasticity and nano-hardness. - چکیده انگلیسی: Aim: the aim of this study was to compare cusp deflection, infraction and fracture in teeth filled with three temporary filling materials. Materials & method: Forty five extracted human premolar teeth were chosen. After root canal therapy and mesio-occluso-distal cavity preparation, samples were randomly divided into three groups , each contained 15 teeth and filled with three temporary filling materials: Cavisol (Golchai-Iran), Coltosol F (Coltene,Swiss) and Coltene (Ariadent,Iran). Teeth were kept in normal saline at room temperature and every day the intercuspal distance was measured under stereomicroscope for 20 days. Infractions as well as fractures were also noted. Data were analyzed in SPSS 17 using Repeated measurement ANOVA test to evaluate the intercuspal distance and expansion of each sample every day. Results: Intercuspal distance increased in all three groups but was significantly more in Coltosol F group. On the days 10 and 16 two teeth filled with Coltosol F had cusp fracture. Conclusion: Temporary filling materials have hygroscopic expansion and cause cusp deflection which may lead to cusp fracture, so it is recommended to use them in short period of time.
