- چکیده انگلیسی: Background: Appropriate diagnosis and treatment of latent tuberculosis infection (LTBI) play the most important role in the control of tuberculosis. This study aimed to determine the prevalence of LTBI among healthy tuberculosis unexposed children vaccinated with BCG using the tuberculin skin test (TST) and QuantiFERON TB Gold In-Tube (QFT-GIT) and comparing the agreement between the two tests. Methods: A cross-sectional study was carried out between October 2009 and March 2010 in 24 schools and 11 daycare centers. A total of 967 children were divided into 15 age groups, with a minimum of 64 children per group.Results: The prevalence rates of LTBI with TST were 3.8%, and 2.2% with QFT-GIT. One case was positive in TST and QFT-GIT, 20 cases were QFT-GIT positive, but TST negative and 36 cases were TST positive, but QFT-GIT negative, and finally, 910 cases were negative in both. There was poor agreement between TST and QFT-GIT (1.8%, 95%, CI: 0%-5.3%, k=0.007). The specificity of QFT-GIT in the BCG vaccinated, children aged 1-15 years old, was 97.8% (97.8%, 95% CI: 96.8%-98.8%). After three months, 2/17 (11.8%) of those initially QFT-GIT negative converted, and 10/15 (66%) of those initially QFT-GIT positive reverted.Conclusion: It seems that TST and QFT-GIT are not appropriate tests for the diagnosis of LTBI among healthy tuberculosis unexposed BCG vaccinated children. There was a low reproducibility rate of QFT-GIT. The cause of the the poor agreement requires further studies.

- چکیده انگلیسی: Background: Pseudoexfoliation syndrome is one of the most common identifiable causes of open angle glaucoma. It has unknown etiology and pathogenesis. Infection, possibly viral, is one of the proposed pathogenic mechanisms in this condition. In the present study the presence of herpes simplex virus (HSV) in specimens of anterior lens capsule of patients with pseudoexfoliation syndrome has been assessed. Methods: The presence of HSV- DNA was searched by using polymerase chain reaction method in specimens of anterior lens capsule (5 mm diameter) of 50 patients with pseudoexfoliation syndrome (study group) and 50 age-matched patients without the disease (control group) who underwent cataract or combined cataract and glaucoma surgery during a one-year (2006-2007) period in Khalili Hospital, Shiraz, Iran. The results were compared statistically with Chi-square test and independent samples t test using SPSS software (version 11.5). Results: HSV type I DNA was detected in 18% of the patients in the study group compared with 2% in the control group (Chi square test, P = 0.008). The difference between the ranges of intraocular pressure in the two groups was not statistically significant. Conclusion: The presence of HSV type I DNA suggests the possible relationship between the virus and pseudoexfoliation syndrome. It may be a treatable etiology in this multi-factorial disorder and may help to future management of patients; especially to prevent some of the complications in this syndrome.

- چکیده انگلیسی: Background: Pandemic flu had at least two waves in Iran. Knowing how many of the general population were already exposed to this infection has a major impact on na-tional preventive measures. As of December 30, 2009, a total of 3672 confirmed cases of human infection with a novel Influenza A (2009 H1N1) virus had been reported in Iran with 140 deaths.
Objective: In this study we aim to measure, as a pilot study, the seroprevalence of positive antibody titer (humoral immunity) against 2009 H1N1 virus in Iranian population in Shiraz, Southern Iran.
Methods: Through cluster random sam-pling of families residing in Shiraz, 2553 subjects were selected and after a medical in-terview blood samples were taken and checked for polyclonal antibody against 2009 H1N1 antigen using hemagglutination inhibition assay. An antibody titer of more than 1:40 dilution was considered positive. Data were analyzed considering the demographic characteristics of the population and were compared among different age groups.
Results: 1504 (58.91%) samples were tested positive for the presence of polyclonal an-tibody against 2009 H1N1 virus. The prevalence of positive titers were significantly higher in 60 to 64 years old group and significantly lower in 20 to 24 years old group (p<0.05). Data did not differ based on other demographic characteristics or the history of flu like illnesses in the past 6 months.
Conclusion: High seroprevalence of antibody against 2009 H1N1 in the sera of our subjects describes either a high level of pre-existing immunity against H1N1 in Iranian population or a high rate of asymptomatic infection in our area compared to other countries.- انتشار مقاله: 16-05-1395

- چکیده انگلیسی: NEMO (NF-κB essential modulator) is a regulatory factor involved in signaling pathways of innate and adaptive immune system. Hypomorphic mutation of IKBKG gene on X chromosome leads to X-linked recessive anhidrotic ectodermal dysplasia with immunodeficiency. The affected boys presented developmental phenotype with hypotrichosis, hypohidrosis; hypodontia with conical incisors; and susceptibility to pyogenic bacteria, mycobacteria, and viruses. Most of them have impaired antibody response to polysaccharide antigens. Here, we presented the case report of 7 years old Iranian boy with NEMO-deficiency and unusual Aspergillus infection.
